Toxicity

Lithium is the first-line treatment for bipolar disorder (BD) But there is a large variation in response rate and adverse effects. This paper describes the case of a 74 year old patient who became lithium toxic after 15 years of lithium therapy.

Pharmacokinetics

Co-administration of nefazodone did not modify pharmacokinetic parameters of lithium at steady-state. Results support the view that ethnic differences in blood levels found with other psychotropic drugs are due to altered drug metabolism.

Lithium, alone and in combination with any of the tested antidepressants, led to a consistent increase of IL-1ß, IL-6 and TNF-α levels. Maintenance therapy with quetiapine significantly increased the time to recurrence of a mood event (mania or depression) versus placebo.

Dosage Studied

The feasibility of single daily dosing of lithium carbonate was tested in eight recurrent manic-depressives being treated with lithium prophylaxis. Dosing with tap water as soon as possible within 30 min after ingestion seemed a useful first aid measure to prevent esophageal injury after accidental swallowing of a lithium battery.
