Internal jugular vein thrombosis following carotid endarterectomy.

    Internal jugular vein thrombosis (IJVT) is an uncommon condition, which is often not correctly diagnosed clinically. Diagnosis is readily made by duplex scan, computerized tomography with contrast or magnetic resonance imaging. A case of IJVT following carotid endarterectomy with intra-operative retraction of the internal jugular vein is reported.

Fusiform Left Carotid-Ophthalmic Artery Aneurysm Presenting with Central Retinal Artery Occlusion.

    Central retinal artery occlusion (CRAO) is a devastating ocular emergency characterized by acute painless visual loss in the ipsilateral eye. We describe the case of acute non-arteritic CRAO associated fusiform internal carotid-ophthalmic artery aneurysm with intraluminal thrombus. Despite the rarity of this condition, we suggest that carotid-ophthalmic artery aneurysm should be included in the differential diagnosis of CRAO.

    Acute bilateral blindness is an emergent condition that may signal life-threatening disease. This article details an emergency medicine approach to the patient with acute bilateral blindness, and reports a case of an elderly woman whose sole presenting complaint was acute loss of vision. She was ultimately diagnosed as having pulmonary embolism. The approach focuses on identification of life-threatening disease processes, while differentiating between ocular, psychogenic, and cortical etiologies.

Massive subretinal hemorrhage and anticoagulant therapy.

    A 66-year-old woman receiving anticoagulant therapy experienced a spontaneous massive subretinal hemorrhage that led to blindness and intractable glaucoma in the involved eye and necessitated enucleation. Clinical and histopathological observations suggested that the bleeding originated from blood vessels within an area of disciform macular degeneration. If the patient had not been receiving anticoagulant therapy the hemorrhage would presumably have been more limited.

Procainamide-induced SLE and lymphoreticular disorders.

    A 56-year-old male patient diagnosed as a case of procainamide-induced systemic lupus erythematosus (SLE) was found to have a lymphoproliferative disorder at postmortem examination.Contrary to other immune disorders, the association of SLE with neoplasia is a rare occurrence. The present case raises the question of whether a relationship exists between the lupus diathesis and lymphoreticular neoplasia. The study of the incidence of neoplasia in families of patients with SLE may prove helpful in establishing this relationship.
