The impact of thyroid activity variations on some oxidizing-stress parameters in rats.

    The effect of the thyroid activity on the formation of lipid peroxidation and on liver and heart antioxidant enzyme activities was investigated in Wistar rats. Hypothyroidism and hyperthyroidism conditions were induced for five weeks by the administration of 0.05% benzythiouracile (BTU) and L-thyroxine sodium salt (0.0012%), in drinking water, respectively. No significant effect was observed on the rates of both lipid peroxidation and the vitamin E in hepatic and cardiac tissues of hypothyroidism rats compared to the controls, contrary to the hyperthyroidism rats, which expressed a pronounced increase. The increased glutathione peroxidase activity in rats suffering from hyperthyroidism was associated with a fall of the reduced glutathione in the homogenate and a marked increase in the glutathione reductase activity. An increase in superoxide dismutase and catalase activities was also recorded in hyperthyroidism. Our results explain the thyroid activity variation in relation to the lipid peroxidation and the tissular contents of the enzymatic and the non-enzymatic antioxidants. To conclude, our results show the occurrence of a state of oxidizing stress in relation to hyperthyroidism.

[Benzylthiouracil induced ANCA-positive vasculitis].

    Several cases of vasculitis associated with antineutrophil cytoplasmic antibodies (ANCA) have been reported in patients treated with synthetic anti-thyroid drugs but only 2 cases have incriminated benzylthiouracil.. A 36 year-old woman, 3 years after treatment with benzylthiouracil, rapidly developed progressive kidney failure, related to a pauci-immune extra-capillary glomerular nephropathy and necrotic vasculitis lesions. The search for p-ANCA was positive with anti-myeloperoxidase specificity. She was treated with corticosteroids and 6 monthly intravenous pulses of cyclophosphamide substituted by azathioprine. Renal failure and proteinuria significantly improved. However the high level of p-ANCA.. ANCA vascularities are a rare but serious complication of treatment with synthetic thiouracile-type anti-thyroid drugs. The ANCA must be measured when confronted with a systemic manifestation during treatment.

Effects of thyroid hormone on the renal dopaminergic system.

    This study determined the effects of thyroid hormone on the renal dopaminergic system. Surgical thyroidectomy (Tx) and treatment with 2-thiouracil (Thio) decreased renal cortex Na+/K+ ATPase activity and urinary volume. Tx also decreased urinary Na+ and urinary L-DOPA without changing urinary excretion of Dopamine (DA). Thio treatment decreased slightly urinary L-DOPA and Na+, but increased urinary excretion of DA. In both models of thyroid hormone deficiency, the ratio urinary DA/DOPA increased. Changes after Thio treatment were reversed after one month of drug withdrawal. Treatment with T3 via osmotic minipump increased Na+/K+ ATPase activity and urinary L-DOPA, did not change urinary DA, and increased the ratio DA/DOPA. To further analyze the effects of thyroid hormone deficiency, we administered selective DA1 (SCH-23390), DA2 (Sulpiride), and a non selective (Haloperidol) DA receptor antagonists to Thio treated and control animals. The DA1 antagonist decreased diuresis, natriuresis and urinary L-DOPA in control, but had no effect in Thio treated rats. Sulpiride had no effect in either group. The combination of SCH-23390 plus Sulpiride decreased urinary L-DOPA and urinary volume only in Thio treated animals. Haloperidol decreased urinary volume in Thio treated animals, but had no effect in controls. Our findings suggest that renal DA synthesis is to some extent dependent on thyroid hormone levels, and that the response of DA receptors is altered by thyroid hormone deficiency, indicating a role of this hormone in the regulation of the renal dopaminergic system.

The influence of hypo- and hyperthyreosis on insulin receptors and metabolism.

    Changes in thyroid status affect metabolism not only directly, but influence it also by alterations in insulin secretion and action. Despite several investigations, these effects are, however, poorly characterised or even controversial. The aim of the studies was to investigate the effect of hyperthyreosis (HT) and hypothyreosis (HPT) on insulin binding by rat liver membranes. Some metabolic parameters reflecting insulin and thyroid hormones action were also determined. HT and HPT were developed by daily administration for 3 weeks of thyroxine (T (4) ) and thiouracil (TU), respectively. Experimental hyperthyreosis and hypothyreosis caused deep changes in metabolism. The greatest alterations were observed in body and thyroid glands weight, blood triiodothyronine (T (3) ), T (4), glucose, and insulin levels, liver glycogen amount and number of insulin receptors. HT reflected in rats in slower rate of growth and in smaller thyroid glands weight. In comparison to controls, T (4) concentration in HT was almost doubled and it was reduced by about 30% in HPT. Also, T(3), insulin and glucose levels in HT were heightened. Simultaneously, binding of insulin to liver membranes was elevated in HT and reduced in HPT. In HT the number of high affinity insulin receptors (HAIRs) and low affinity insulin receptors (LAIRs) was increased, whereas in HPT the amount of HAIRs was diminished. HT caused a drastic reduction of glycogen concentration in liver, but no changes were observed for muscle glycogen. Considering lipid metabolism, only free fatty acids (FFA) level in blood was changed (in HPT), but no differences were observed in serum concentration of triglycerides and cholesterol. Several metabolic changes observed in HT and HPT seem to be the dire ct consequence of alterations of thyroid hormone concentrations. These disturbances, together with the direct effect of HT or HPT on insulin secretion, binding and action lead, in turn, to changes in the other metabolic parameters. As a result of these disturbances the adaptive mechanisms appear. One of them is change in the number of insulin membrane receptors taking place even against the well known "down-regulation" theory.

[Treatment of Basedow-Graves disease in pregnancy].

    Thyrostatic treatment of pregnant women with Graves' disease is a special problem. Observation of 46 pregnancies of 35 women suffering from Graves' disease has been summarized. The outcome was successful in 45 cases. Methimazole and propylthiouracil was administered to the patients without thyroxine. Therapy was needed for the two thirds of the mothers. At the end of the second trimester the thyrostatic agent could have been withdrawn in the 77% of the cases. Antithyroid treatment administered in low dose at the time of conception did not affect the outcome. Premature delivery rate and the number of neonates with low weight did not increased. Transient hyperthyrotropinaemia was observed in one case. Likewise, one infant suffered from neonatal thyrotoxicosis. 37% of the mothers had postpartal recurrence of hyperthyroidism.. the free thyroxin level monitoring is essential during thyrostatic treatment. Thyrotropin receptor antibody investigation, having predictive value for neonatal thyrotoxicosis, should be done, too. Postpartal thyroid control is necessary for elucidate a hyperthyroid relapse, the rate of which was almost 40%.

[Treatment of hyperthyroidism in children with antithyroid drugs].

    Sixteen cases of pediatric hyperthyroidism treated by antithyroid drugs are reported. First-line treatment with carbimazole in a mean dose of 19 mg/m2 initially and 7 mg/m2 after one year ensured control of the disease within 6 weeks. Mean follow-up was 4 years. Only two children were able to discontinue the drug, after 9.1 years and 4.6 years. Two other patients had thyroidectomy, after 6.7 years and 3.4 years. The main disadvantages of antithyroid drugs are the need for prolonged treatment and the risk of recurrence. An attempt at treatment discontinuation is warranted in patients with good disease control of 2 to 3 years duration, goiter size reduction, improved exophthalmos, and decreased anti-TSH receptor antibodies.

The pituitary-thyroid axis effects on ocular and orbital tissues: a histological, histochemical, and morphometric study.

    This work tests the notion that the effect of thyroid hormone on orbital and ocular tissues is mediated through its action on their lysosomal enzymes. Hyperthyroidism was produced in guinea pigs by thyroxin and TSH; hypothyroidism was induced by thiouracil. After treatment for 10 to 21 days, several ocular and orbital tissues were taken for histological, morphometrical, and histochemical examinations. High acid phosphatase activity was demonstrated in extraocular muscles, optic nerve and the retinal pigment epithelium of thyroxine- and TSH-treated animals. The findings fit the notion that the effects of thyroid hormones are mediated through lysosomes also in ocular and orbital tissues.

Muscle strength in hyperthyroid patients before and after medical treatment.

    Ten hyperthyroid patients were assessed for muscle strength before and after a period of medical treatment that averaged 12 months. The subjects did not change their habitual level of physical activity between the two test occasions. Maximal voluntary isokinetic knee extensor muscle strength was determined during various concentric, eccentric and isometric conditions. Average increases in strength from before to termination of treatment ranged from 25 to 41% for the concentric and isometric tests (P less than 0.01, n = 10), and from 19 to 35% for the eccentric tests (P less than 0.01, n = 6). The present study demonstrates that medical treatment of hyperthyroid patients results in a marked increase in muscle strength.

Effect of hyperthyroidism on fibre-type composition, fibre area, glycogen content and enzyme activity in human skeletal muscle.

    Seven hyperthyroid patients were studied by repeated muscle biopsies (vastus lateralis) before and after a period of medical treatment which averaged 10 months. The biopsies were analysed with regard to fibre-type composition, fibre area, capillary density, glycogen content and enzyme activities representing the glycolytic capacity (hexokinase, 6-phosphofructokinase), oxidative capacity (oxoglutarate dehydrogenase, citrate synthase) and Ca2+- and Mg2+-stimulated ATPase in muscle. In the pretreatment biopsy (hyperthyroid state), there was a significantly lower proportion of type I fibres (30% vs. 41%), a higher capillary density (23%), lower glycogen content (33%), and higher hexokinase activity (32%) compared with the post-treatment biopsy. No significant changes in the activity of the remaining enzymes were observed. The present study indicates that hyperthyroidism induces a transformation from type I to type II fibres in human skeletal muscle. The increase in hexokinase activity probably reflects a higher glucose utilization by skeletal muscle in order to compensate partially for the reduced glycogen content.

Long-term effects of triiodothyronine and thiouracil on myocardial beta-adrenergic receptor numbers and cyclic AMP concentration in rats.

    Long-term (35 days) effects of thyroid hormone on the number of myocardial beta-adrenergic receptors and the c-AMP concentration were studied in the rat. The ventricles from triiodothyronine-administered rats (T3 rats) showed an increase in the number of beta-receptors compared to controls (54.3 +/- 3.1 vs 39.3 +/- 1.8 fmole of [125I]-iodohydroxybenzylpindolol (IHYP) binding sites/mg protein p less than 0.001) on the 35th day. Conversely, the ventricles from thiouracil-administered rats (TU rats) showed a decrease in the number of beta-receptors compared to controls (31.4 +/- 1.6 vs. 38.4 +/- 2.6 fmole/mg protein, p less than 0.05). The equilibrium dissociation constants (Kd) for the interaction of receptors with IHYP did not differ significantly (0.10 to 0.15 nM). The myocardial concentration of cyclic AMP was not significantly different (T3 rats, 1.09 +/- 0.09 nM/g wet weight tissue; T3 controls, 1.12 +/- 0.07; TU rats, 1.13 +/- 0.07; TU controls, 1.16 +/- 0.12) on the 35th day. On serial effects of triiodothyronine from the first to the 35th day, the number of beta-receptors of T3 rats increased significantly on the 24th and the 35th day, but the c-AMP concentration was not significantly different from that in control rats. These results demonstrated that thyroid hormone affects the number of myocardial beta-receptors in rats, and suggested a different mechanism of action of thyroid hormone on the myocardium from that of catecholamines.

[Drug therapy of hyperthyroidism].

    The possibilities of the treatment of hyperthyroidism are mentioned, in which cases the author especially deals with the medicamentous therapy, taking into consideration the different chemical groups (imidazol derivations, thiourazil derivations, perchlorates) and their therapeutic mechanisms. The indications and contraindications of this treatment and the possible side effects are compiled in form of a table. The in-vitro-parameters which are necessary for the observation of the course are estimated in their significance. The author deals with the problems of the additional treatment. Taking into consideration indications and contraindications as well as controls of the course of functional parameters the medicamentous therapy concerned is a treatment with a good prognosis. It is promising particularly in such cases, when suitable parameters are used for the solution of the question concerning the end of the therapy. For this purpose nowadays two tests are at our disposal: 1. modified Alexander's test (radioiodine-test and suppression test under thyreostatic therapy) and 2. TRH-test with radioimmunological determination of TSH. The treatment should be carried out in adequate endocrinological dispensaries, as there are certain risks in not optimal therapy.

Effect of thyroid status on the thyrotrophin-releasing hormone-degrading activity of rat serum.

    The TRH-degrading activity of rat serum in vitro is five times more potent than that of human serum. In rats, it is significantly reduced in hypothyroidism (thiouracil-induced) and significantly increased in hyperthyroidism (T3 or T4-induced). This suggests a possible role in the regulation of adenohypophysial-thyroid function which is probably, in turn, dependent on thyroid hormone, rather than TSH, levels.

Neonatal hyperthyroidism following intrauterine hypothyroidism.

    An infant, whose mother was treated for thyrotoxicosis during pregnancy, appeared normal at birth, but laboratory data were indicative of hypothyroidism. On the sixth day of life the infant had clinical and laboratory evidence of hyperthyroidism. A plan of management is proposed for infants born to thyrotoxic mothers.

[Effect of MMI on the serum T3 and T4 levels in hyperthyroid patients (author's transl)].

    Effect of 1-methyl-2-mercaptoimidazole (MMI) on the serum triiodothyronine (T3) and thyroxine (T4) was studied in six patients with hyperthyroidism. From 30 to 60 mg of MMI per day were administered to the patients orally. Serum T3 was measured by radioimmunoassay and T4 by radioimmunoassay of Tetrasorb Kit (Abbott Lab.). Serum T4/T3 ratios in these patients were low initially, but increased to normal range in two cases immediately after the administration of MMI. But they were unchanged at first in the other four cases. When high serum T3 values decreased abruptly to normal at a later course of medication, the serum T4/T3 ratios increased to normal range in almost all cases. Even though the serum T4/T3 ratio became normal, serum T3 and T4 values remained high in some cases. Elevated serum T4 values in all cases decreased to normal faster than serum T3 with the administration of MMI. When MMI was administered to the patents concinuously, the T4/T3 ratios, once increased to normal, then decreased again gradually in five cases. And serum T3 which decreased to normal sometimes gave high values. After the discontinuation of MMI, serum T3 increased over the normal range more rapidly than did T4.

The use of antithyroid drugs.

    The preoperative use of antithyroid drugs is mandatory if surgical treatment of complicated hyperthyroidism is contemplated. Six months to a year may be required for suitable preparation. The long-term use of antithyroid drugs is less effective for the "cure" of hyperthyroidism than is operation or the use of radioactive iodine. Propyl and methyl thiouracil are the antithyroid drugs of choice. Either of these thiouracil derivatives is capable of producing leukopenia. The antithyroid drugs exert no favorable effect on exophthalmos. The antithyroid drugs are suitable for the control of hyperthyroidism during pregnancy. The patient probably should not be kept hypothyroid during pregnancy but rather in a state of mild hyperthyroidism.

Substernal goiter and pulmonary emphysema.

    Pulmonary emphysema is not an infrequent complication of tracheal compression due to goiter. In many cases the goiter is in the mediastinum and therefore overlooked. Among 13 patients with this combination of disease, chronic cough, dyspnea and wheezing were the chief respiratory symptoms. Ten of the 13 patients also had hyperthyroidism; in eight of the ten the disease was of the "masked" variety and both the thyrotoxicosis and the mediastinal mass were overlooked. Early recognition and surgical removal of substernal goiter may prevent or retard otherwise irreversible and progressive tracheobronchopulmonary disease. Medical management of such cases with thiouracil preparations may lessen the toxic symptoms but probably will not affect the mechanical compression of the trachea.
