Subacute, tetracycline-responsive, granulomatous osteomyelitis in an adult man, consistent with Q fever infection.

    Osteomyelitis due to Coxiella burnetii infection is a rare condition in adults. We report the case of a healthy young man presenting with subacute osteomyelitis of the left cheek bone, evolving gradually after an episode of acute febrile illness. Histological evaluation confirmed subacute granulomatous inflammation. Despite antibody titres not reaching the standard cut-off for chronic Q fever (phase I IgG 1/160, phase II IgG 1/2560), osteomyelitis was radiologically and histologically confirmed. A 6-month course of doxycycline/hydroxychloroquine brought clinical and radiological cure while various conventional antibiotic treatments had failed to improve the clinical condition. Currently, at 6-month follow-up, no relapse has occurred and antibody titres have declined. A shorter course of doxycycline/hydroxychloroquine than that used for chronic Q fever osteomyelitis may be sufficient to treat subacute Q fever osteomyelitis in some cases.

Bacterial osteomyelitis in a 3-week-old broiler chicken associated with Enterococcus hirae.

    Enterococcus hirae infections are reported to cause growth depression, encephalomalacia, endocarditis, and septicemia in chickens. This report describes osteomyelitis in the proximal femur of a 3-week-old broiler chicken that also suffered from valvular endocarditis and liver necrosis. Histologically, clusters of gram-positive coccoid bacteria were found in many organs, including bone lesions. In tissues from 5 of 6 examined chickens from the same flock, E hirae was isolated in large numbers. To the authors' knowledge, this is the first report of spontaneous bacterial osteomyelitis where E hirae was cultured from bone and where coccoid bacteria consistent with Enterococcus spp were simultaneously demonstrated within bone lesions.

Fluorescent tetracycline labeling as an aid to debridement of necrotic bone in the treatment of chronic osteomyelitis.

    The authors have found that several months of preoperative tetracycline administration will label living bone so that it fluoresces under a blacklight during surgery. This allows intraoperative differentiation of viable bone from necrotic sequestra, which require removal in the treatment of chronic bone infection.

Histology and histomorphometry of ethmoid bone in chronic rhinosinusitis.

    Mucosal changes have been well described in chronic sinusitis, yet little is known about the underlying bone, despite clinical and experimental evidence suggesting that bone may be involved in chronic sinusitis. Techniques of undecalcified bone analysis were used for detailed histologic examination of ethmoid bone in chronic sinusitis compared with controls. Bone synthesis, resorption, and inflammatory cell presence were specifically assessed. Additionally, histomorphometry techniques were used to determine ethmoid bone physiology in individuals undergoing surgery for chronic sinusitis. Overall, individuals undergoing surgery for chronic sinusitis were found to have evidence of marked acceleration in bone physiology with histologic changes including new bone formation, fibrosis, and presence of inflammatory cells. These findings are compared with osteomyelitis in long bone and the jaw. The suggestion that underlying bone may serve as a catalyst for chronic sinusitis is supported and implications for therapy are discussed.

Ureaplasma urealyticum chronic osteomyelitis in a patient with hypogammaglobulinemia.

    Mycoplasma species are recognized as important pathogens in patients with hypogammaglobulinemia. In this article we describe, for the first time, a patient with hypogammaglobulinemia who developed osteomyelitis of the hip caused by Ureaplasma urealyticum. This article emphasizes the need for considering infection with Mycoplasma species in patients with antibody deficiency.

Osteomyelitis-like bone lesions in acne fulminans.

    Two young males with acne fulminans (AF) are described. In addition to severe skin lesions, they had musculoskeletal symptoms including bacteriologically negative osteolytic lesions in the clavicles. Both patients received antibiotics for suspected septic infections. The response was unsatisfactory in both patients, and combination treatment with prednisolone was started. A favourable response was observed, and a relapse occurring in the first patient was controlled by steroids alone. The possible pathomechanism of bone lesions in AF is discussed.

    Dr. Miller's discussion of the diagnosis and management of soft tissue infections is probably one of the most important topics in this issue. We are all faced with the potential for relatively minor infections becoming serious problems. Having a sound understanding of the infectious process and treatment regimens is extremely important in our general practices. Few things can make a patient as unhappy or destroy good surgical results as quickly and easily as an infection. I recommend reading the articles suggested by Dr. Miller and add that I believe that if the infection has not responded as quickly to therapy as is expected, an infectious disease consultation should be requested.

Brucella osteomyelitis of a closed femur fracture.

    A 19-year-old man incurred a closed femoral fracture complicated by hematogenous dissemination of Brucella osteomyelitis. Repeated limited incision and drainage were ineffective in eradicating infection. Wide debridement, delayed wound closure, and vigorous antimicrobial therapy with streptomycin and tetracycline, along with cephalosporin for secondary staphylococcal infection, were necessary measures before the infection was eradicated. A constant awareness of brucella musculoskeletal infection is advisable when caring for patients frequently exposed to all kinds of livestock, including domesticated and wild animals.

Rifampicin-containing antibiotic combinations in the treatment of difficult infections.

    Combination of rifampicin with trimethoprim, erythromycin, tetracycline or fusidic acid have some desirable features in the treatment of difficult infections. They are active against a very wide range of possible pathogens. Resistance to rifampicin is rare. Such combinations may be bactericidal and may be usefully synergistic. They may prevent or delay the emergence of bacterial resistant seen when some single agents are used. They can be used in patients with penicillin hypersensitivity. A series of life-threatening infections has been treated with rifampicin-containing combinations. The infections included endocarditis, meningitis, pneumonia, Legionnaire's disease, and head and neck sepsis. A major reason for the choice of drug was often penicillin hypersensitivity. A second reason was the presumption (mostly subsequently confirmed) that streptococci and/or staphylococci were implicated. The clinical outcome of these infections was generally satisfactory, with few side effects and little evidence of the emergence of antibiotic resistance.

[Enterobacter-osteomyelitis in two neonates (author's transl)].

    Two boys aged up to 2 weeks suffered from enterobacter-sepsis. In both cases osteomyelitis developed in spite of treatment with Gentamycin or Gentamycin combined with Chepazolin. Both children were, taking accont of the risks, then treated with Chloramphenicol (100 mg/kg body weight/24 hours) and the first patient also, for a short time, with tetracyclin. In the second patient we saw a marrow depression dependent on Chloramphenicol and its dosage which disappeared rapidly, when the drug was withheld.

LINCOMYCIN: A NEW ANTIBIOTIC ACTIVE AGAINST STAPHYLOCOCCI AND OTHER GRAM-POSITIVE COCCI: CLINICAL AND LABORATORY STUDIES.

    Preliminary results suggest that the antibiotic lincomycin (a product of Streptomyces lincolnensis var. lincolnensis) possesses certain valuable properties which include good in vitro activity against many strains of hospital staphylococci resistant to many other antibiotics. During a study of this agent, a selected series of severe staphylococcal infections due to resistant organisms were treated with lincomycin, with encouraging responses. Favourable results were also noted in seven cases of osteomyelitis. Lincomycin may be administered by the oral or parenteral routes to adults and infants and satisfactory serum blood levels obtained. So far as the authors' limited experience enables them to conclude, and at the dose range tested, this antibiotic promises to be one of low toxicity.
