Technical approach to iliac crest biopsy.

    Bone histomorphometry has been the gold standard in the evaluation and diagnosis of renal osteodystrophy. The recent new definition of renal osteodystrophy as chronic kidney disease-mineral and bone disorder has once again highlighted the use of bone biopsy as a powerful and diagnostic tool to determine skeletal abnormalities in chronic kidney disease. The procedure of iliac crest bone biopsy has been proved safe and associated with very minimal morbidity. In this review, the clinical indications, preparation, instrumentation, and potential complications are discussed. Because current biochemical markers are poor predictors of bone turnover, volume, and mineralization, a wider use of bone biopsy and histomorphometry will lead to a better understanding of the bone and mineral disorders that are associated with chronic kidney disease.

Hepatorenal failure induced by tetracycline in dogs with portacaval shunt.

    The aim of the present investigation was to determine whether tetracycline accelerated the hepatic toxicity of portacaval anastomosis and whether this could be reflected by changes in pharmacokinetics of the drug. Side-to-side portacaval shunts were constructed and converted to end-to-side by ligating the hepatic side of the portal vein in dogs. The results of this study showed that the i.v. infusion of tetracycline (50 mg/kg) to shunted animals caused rapid deterioration in hepatic and renal functions followed by the eventual transition of these animals from stage I to stage II liver disease. This was reflected by a 3- to 6-fold increase in the serum level of bilirubin, alkaline phosphatase, serum glutamic-oxalacetic transaminase, blood urea nitrogen and creatinine as compared with the levels of those produced in serum of dogs before and after the construction of the shunt before the administration of tetracycline. Kinetic analysis revealed a significant prolongation in the elimination half-life (38.2 +/- 4.2 hr) of the shunted dogs as compared with controls (14.5 +/- 1.5 hr) after the i.v. administration of tetracycline. This was accompanied by an appreciable reduction of elimination rate constant. In contrast to shunted animals, control animals exhibited no behavioral side effects after the administration of tetracycline.

Intrarenal abscess. Report of 14 cases.

    Six of 14 patients with renal abscess had prior history of urinary tract infection; initial symptoms included fever and flank pain in 12. A drip-infusion intravenous pyelogram was the most sensitive radiologic test, but selective renal arteriography was most specific. Urine cultures were positive in all 14 patients; blood cultures were positive in nine. Six patients were treated with antibiotics alone and eight required surgery. Of the eight, five had pus-filled cavities, one had multiple stones, one had a renal infarct, and one had a resolving abscess. Of six treated with antibiotics alone, one died of unrelated complications and five have demonstrated no pathological renal condition after three to six years.

Tetracycline-induced aplastic anemia.
    Southern medical journal, 1979, Volume: 72, Issue:3

    A 54-year-old man developed mild renal insufficiency after treatment with gentamicin and cephalothin. He then received tetracycline for eight days, 500 mg orally four times daily, for a total dose of 15 gm. Twelve days after discontinuation of the drug, a severe but reversible aplastic anemia developed. We propose that bone-marrow-toxic concentrations of tetracycline resulted from the preexisting renal insufficiency. Because of the temporal relationship between tetracycline administration and aplastic anemia, and the lack of any other explanation, we conclude that this case illustrates a rare but potentially fatal complication of this drug.

The effects of peritoneal dialysis on the physiological disposition of oxacillin, ampicillin and tetracycline in patients with renal disease.

    Seven patients with acute or chronic renal failure who were receiving intermittent peritoneal dialysis and who required parenteral oxacillin, ampicillin or tetracycline were studied to determine the disposition of these antibiotics in severe renal disease and the effects of peritoneal dialysis. While severe renal impairment markedly prolongs persistence in the serum of ampicillin and tetracycline, there is little effect on oxacillin. Whereas required doses of ampicillin and tetracycline are lower in the presence of severe renal disease, oxacillin should be given in doses equivalent to those used for patients with normal renal function. Peritoneal dialysis does not alter these dosage requirements.Four patients receiving ampicillin or tetracycline in the infusing solution during peritoneal dialysis were studied to determine the amount of systemic absorption. Local prophylaxis alone is not achieved with this method of administration, since small amounts of both antibiotics are absorbed systemically from the infusing solution. The serum concentration of tetracycline attained is inadequate for treatment of systemic infections but is probably significant, with repeated use in intermittent dialysis, in causing adverse effects. Tetracycline should be abandoned in the local prophylaxis of peritonitis during peritoneal dialysis.

Chemotherapy of experimental leptospiral infection in mice.

    A strain of Leptospira zanoni was used to produce chronic renal infections in young white mice. A variant of this strain produced an acute disease with over 50% mortality. The responses of both forms of disease to chemotherapy were studied. When treatment of the acute disease was initiated before jaundice occurred, suitable single doses of streptomycin, chlortetracycline, tetracycline, erythromycin, oxytetracycline and oxytetracycline (in oil) prevented death and chronic renal infection in a high percentage of mice. Bicillin, a long-acting penicillin preparation, was more effective than other penicillins, but it prevented the development of chronic renal infection in only half the treated mice. Streptomycin was the only antibiotic of which a single administration regularly cured the chronic renal infections: chlortetracycline, tetracycline and oxytetracycline (in oil) were partially effective. Oxytetracycline, chloramphenicol, Bicillin, fortified penicillin, procaine penicillin and potassium penicillin had no permanent action. The suitability of mice as laboratory animals in the study of experimental leptospirosis and the need for complete cure of carriers of chronic renal infection are emphasized. The above findings indicate that streptomycin is the drug of choice for the treatment of leptospirosis in animals, and that it is worthy of further trial in man.
