    Patients with POAG have lower corneal endothelial cell density than healthy controls of the same age. This may be attributed to mechanical damage from elevated IOP and toxicity of glaucoma medications.. Mycophenolic acid was detected in all cats. The dose 10 mg/kg given q12h for 1 week was tolerated (n = 3). The efficacy of MMF as an immunosuppressant and long-term safety in cats of this dosage regimen is unknown.. T

Single-dose penicillin prophylaxis against neonatal group B streptococcal infections. A controlled trial in 18,738 newborn infants.

    Neonatal Group B streptococcal infections may not respond to antimicrobial therapy and have been associated with case fatality rates of 50 per cent or greater. We evaluated the effect on colonization and disease rates of a single intramuscular dose of aqueous penicillin G given at birth in a prospectively controlled study of 18,738 neonates during a 25-month period. The colonization rate in the mothers was 26.6 per cent, with 50 per cent concordance in the untreated infants and 12.2 per cent in the penicillin-treated infants (P < 0.001). There was a significant decrease in the incidence of disease caused by all penicillin-susceptible organisms in the penicillin group (0.64 vs. 2.26 cases per thousand live births, P = 0.005). Disease caused by penicillin-resistant pathogens was increased in the penicillin-treated group during the first year of the study but was unaffected during the second year. Routine administration of parenteral penicillin at birth cannot be recommended until the effect on the incidence of disease caused by penicillin-resistant pathogens is fully defined.

Human chlamydial infections.

    Chlamydiae are obligate intracellular parasites, bacteria with a peculiar biology. They belong to the genus Chlamydia which includes two species: C. psittaci and C. trachomatis. A wide range of hosts, including birds, mammals and man can be infected by chlamydiae. The diseases chlamydiae can produce include psittacosis, lymphogranuloma venereum, trachoma, inclusion conjunctivitis, urethritis, cervicitis, pelvic inflammatory disease, and neonatal pneumonia. The diagnosis of chlamydial infection may be made by visualization of the organism in direct smears, isolation of the agent in cell culture, or by demonstrating a significant rise in antibody titer. Chlamydial infection may be treated with tetracycline, erythromycin, or sulfonamides.

Persistence of chlamydial infection after treatment for neonatal conjunctivitis.

    A high incidence of pharyngeal infection was found in babies with isolation-positive chlamydial conjunctivitis. Chlamydia trachomatis was isolated from the pharynx of 12 (52%) of 23 babies before treatment, and was reisolated from the eyes of 4 (12%) of 34 and from the pharynx of 14 (41%) of 34 after treatment. C trachomatis was reisolated significantly more often from babies treated only with topical tetracycline for 4 weeks (75%) than from those treated with both topical tetracycline and oral erythromycin for 2 weeks (32%). Reisolation from the eyes was associated with only minor clinical signs. Radiological signs of an inflammatory lesion in the chest were found in 2 of 8 babies examined because of persistent cough. These signs were not associated with high or rising titres of serum chlamydial antibody.

Conjunctivitis in the newborn: observations on incidence, cause, and prophylaxis.

    One hundred seventy-one cases of neonatal conjunctivitis seen at Bellevue Hospital during the period 1950--1976 were reviewed. An overall incidence of 3.0 cases per 1,000 live births was found. A comparison of the rates of neonatal conjunctivitis with silver nitrate and tetracycline prophylaxis revealed a 100% increase in the rate overall, as well as the rate of gonococcal conjunctivitis with tetracycline. Using conjunctival cultures and cytology, a diagnosis could be established in 73% of the cases, with 41% being bacterial and 32% chlamydial. Staphylococcus was the single most common organism recovered; gonococcus was relatively rare.

An epidemic of resistant Salmonella in a nursery. Animal-to-human spread.

    A Salmonella heidelberg epidemic in a hospital nursery was traced to infected calves on a dairy farm where the mother of the index patient lived. The Salmonella isolates from all cases were resistant to chloramphenicol, sulfamethoxazole, and tetracycline. Verification of the spread of infection from the farm animals to a hospital population is unusual and raises questions about the hazards of antibiotic animal-feed preparations that may induce infection with resistant organisms in humans.

Purulent ocular discharge in neonates: significance of Chlamydia trachomatis.

    We obtained cultures for bacteria and chlamydiae from 100 infants with conjunctivitis that began during the first month of life. Sixty-nine infants were evaluated during well-child visits (group A); 31 were seen specifically for the ocular discharge (group B). Potentially pathogenic bacteria, predominantly Staphylococcus aureus, were cultured from one third of the infants in each group. Chlamydia trachomatis was recovered from three infants (4%) in group A and from ten (32%) in group B. Three infants with chlamydial conjunctivitis (two in group A, one in group B) had only mild inflammation. Initial treatment with topical antibiotics was unsuccessful in eliminating the organism from seven of 11 infants.

Characterization of Escherichia coli serogroups causing meningitis, sepsis and enteritis. I. Serological properties and animal pathogenicity of O18, O78 and O83 isolates.

    Escherichia coli O78: K80 strains isolated from an outbreak among premature and newborn infants with meningitis, sepsis and enteritis, from sporadic cases of enteritis and from healthy carriers were compared with one another and with different E. coli serogroups. The O78: K80 cultures uniformly failed to give the rabbit intestinal loop test and the guinea pig eye reaction and none of them contained L1 antigen. After intraperitoneal injection into mice, the organisms multiplied in the peritoneal cavity and caused bacteriaemia lasting at least 2 weeks. E. coli strains originating from septicaemia (O78: K80, O18a,c: K?, O83: K?) showed significantly lower LD50 values for mice (9 x 10(3)--7 x 10(5)) than did E. coli serogroups associated with infantile enteritis only (3 x 10(8)--7 x 10(8)). It is assumed that the isolates differ in pathogenicity not only from E. coli strains associated with "cholera-like" disease and with "dysenteriform" infection, but also from L1 antigen-containing cultures described in neonatal meningitis, and constitute a separate group characterized by an ability to cause meningitis, sepsis and enteritis within the same outbreak.

[Enterobacter-osteomyelitis in two neonates (author's transl)].

    Two boys aged up to 2 weeks suffered from enterobacter-sepsis. In both cases osteomyelitis developed in spite of treatment with Gentamycin or Gentamycin combined with Chepazolin. Both children were, taking accont of the risks, then treated with Chloramphenicol (100 mg/kg body weight/24 hours) and the first patient also, for a short time, with tetracyclin. In the second patient we saw a marrow depression dependent on Chloramphenicol and its dosage which disappeared rapidly, when the drug was withheld.
