Steroid dermatitis resembling rosacea: aetiopathogenesis and treatment.

    Corticosteroids were first introduced for topical use in dermatology in 1951. Since then uncontrolled use (abuse) has caused many different reactions, often with manifestations resembling those of rosacea.. The prolonged use of local corticosteroids (usually 2-6 months) may lead to a clinical picture of severe dermatitis with erythema, papules and pustules that are classified according their localization to three types. The treatment of choice is tetracycline in combination with local application of neutral ointments.. Trivial skin dermatoses, especially on the face, should not be treated with local corticosteroids.

Nodulocystic acne: oral gugulipid versus tetracycline.
    The Journal of dermatology, 1994, Volume: 21, Issue:10

    Twenty patients with nodulocystic acne were randomly allocated to one of two treatment schedules: 1) Tetracycline 500 mg or 2) Tab. Gugulipid (equivalent to 25 mg guggulsterone). Both were taken twice daily for 3 months, and both produced a progressive reduction in the lesions in the majority of patients. With tetracycline, the percentage reduction in the inflammatory lesions was 65.2% as compared to 68% with gugulipid; on comparison, this difference was statistically insignificant (P > 0.05). Follow-up at 3 months showed a relapse in 4 cases on tetracyline and 2 cases on gugulipid. An interesting observation was that the patients with oily faces responded remarkably better to gugulipid.

Perioral dermatitis: etiology and treatment.

    Ninety-five patients with perioral dermatitis were studied from an epidemiological aspect. Consistent clearing of the eruption was obtained with oral tetracycline in combination with a topical sodium sulfacetamide-sulfur-hydro-cortisone lotion. Comparison of the study group of patients to a group of 50 control patients revealed highly significant quantitative differences in the cosmetic preparations used by the two groups. This latter finding would suggest that lubricating and moisturizing products play some part in the etiology of perioral dermatitis.

    A case of extragenital donovanosis in a patient with AIDS is reported from Zimbabwe. Despite the rarity of donovanosis in Zimbabwe it is important that health workers are familiar with this disease since donovanosis increases the risk of HIV transmission and appropriate treatment is often successful even in patients with severe immunodeficiency.

Periocular dermatitis akin to the perioral variety.

Sarcoid-like granulomatous periocular dermatitis treated with tetracycline.

    Two cases of periocular micropapular dermatitis with sarcoid-like granulomatous histology are reported. After 8-12 weeks treatment with tetracycline the cutaneous eruptions of both patients cleared. The condition is equivalent to perioral dermatitis.

Perioral dermatitis.

    Perioral dermatitis is an eruption that has a characteristic perioral localization. It usually responds rapidly to systemic tetracycline and low-potency topical corticosteroids. In a study of eighty-seven patients it was found that systemic erythromycin is also effective and that in some instances topical antibiotics combined with 1 per cent hydrocortisone may be effective.

Adverse effect of topical fluorinated corticosteroids in rosacea.

    Fourteen patients suffering from rosacea treated by prolonged topical applications of fluorinated steroids showed adverse effects. Aggravation and extension of telangiectasia occurred in all, and in most cessation of the treatment was followed by severe rebound inflammatory oedema and acute pustular eruption. The telangiectasia cleared or was much improved within three months of cessation of the use of fluorinated steroids. Hydrocortisone did not produce the same effect, and hence it was used, together with oral tetracycline, for treatment.
