Lyme disease: the latest great imitator.
    The Pediatric infectious disease journal, 1988, Volume: 7, Issue:6

    Lyme disease is caused by the spirochete B. burgdorferi. Like its counterpart syphilis, it causes multisystem disease particularly affecting the skin, nervous system, heart and musculoskeletal system. It is endemic in several areas of the United States as well as in Europe. The prompt recognition of this disease and its diverse manifestations should lead to early treatment and resolution. Prevention is aimed at avoidance of the tick vector.

Lyme disease: a review.

    Lyme disease has protean manifestations. The causative agent is Borrelia burgdorferi, a recently discovered spirochete. The disease has been found on three continents. The initial major clinical feature is a characteristic skin eruption known as erythema chronicum migrans. Subsequent main clinical manifestations are meningopolyneuritis and arthritis. However, clinical expressions of the disease vary widely. Some patients have very mild disease and others develop severe and prolonged illness. Specific laboratory tests for the detection of antibody to the organism are now available. The sensitivity of these tests approaches 100% when the sera are collected from patients having the later clinical manifestations. Penicillin and tetracycline are the therapeutic drugs of choice.

Antibiotic treatment of venereal disease and Reiter's syndrome in a Greenland population.

    To assess the effects of antibiotic treatment of urethritis or cervicitis on the incidence of recurrences of articular symptoms in Reiter's syndrome patients.. Retrospective evaluation of the medical charts of 109 patients living in Greenland.. Thirty-seven percent of the episodes of genitourinary tract inflammation that were not treated or were treated with penicillin were followed by arthritis, compared with 10% of those treated with tetracycline or erythromycin.. Antibiotics active against Chlamydia trachomatis reduced the risk of postvenereal arthritis in the population studied.

Bowel-associated dermatosis-arthritis syndrome.

    A 48-year-old woman, who had undergone reversal of a jejunal loop, presented with skin eruptions, myalgia and arthralgia. Delayed gastric emptying was associated with a gastric phytobezoar. The clinicopathological features and response to therapy were typical of the dermatosis-arthritis syndrome, and probably were related to bacterial overgrowth.

[Extragenital gonococcal infections. A review].

    In many cases gonorrhea is overlooked and not treated due to an unusual course of the disease. The longer this infection exists the greater is the danger of dissemination of the gonococci. Gonorrhea, if not diagnosed, may cause a series of local and general clinical manifestations, such as: the dermatitis-arthritis syndrome whose pathogenesis is not yet completely clarified: the Fitz-Hugh-Curtis syndrome which is often seen in females with chronic gonococcal adnexitis;gonococcal pharyngitis which nowadays occurs more often due to less conventional sexual practices of contemporary society; gonococcal conjunctivitis which represents the best known extragenital gonococcosis. This paper deals with the clinical manifestations and their treatment.

Intestinal bypass arthritis: association with circulating immune complexes and HLA B27.

    Two patients are described in whom arthritis following intestinal bypass surgery for obesity was associated with the presence of circulating immune complexes (CLC) and HLA B27. The arthritis was characteristically intermittent and controlled by low dose prednisone, indomethacin, and tetracycline therapy. The findings suggest that immune complexes play a role in the pathogenesis of arthritis associated with condition, and that perhaps the association with HLA B27 may predispose to the development of this complication.

Arthritis after jejunoileostomy for intractable obesity.

    Joint symptoms developed in six of 33 patients (18%) who had jejunoileostomy as treatment for intractable obesity. These consisted of single or recurrent attacks of polyarthritis or arthralgias, primarily in large joints. The histocompatibility antigen, HL-A B27, was not present in these patients. Most attacks responded to simple analgesic agents, but a trial of tetracycline in one patient failed to relieve joint symptoms. This arthropathy seems to differ from that which has been reported after jejunocolostomy.

Detection by electron microscope of rod-shaped organisms in synovial membrane from a patient with the arthritis of Whipple's disease.

    Rod-shaped organisms identical to those present in the jejunal mucosa have been found in the synovial membrane of a patient with Whipple's disease. These probably caused inflammatory changes which sere reflected in an increase of the cellular content and high enzyme levels (acid phosphatase and 5-nucleotidase) of the synovial fluid. Tetracycline was effective in controlling the bowel lesion but only had a temporary effect upon the arthritis. Erythromycin controlled both the bowel lesion and the arthritis.

Reiter's disease in three boys.

    Three cases of Reiter's disease occurring in boys under the age of 16 are reported. One of these presented with a Salmonella enteritidis diarrhoea. This conforms to the 'dysenteric' form of Reiter's disease usually seen in Europe and rarely reported in England. Another presented with a monarticular arthritis of the knee, and the third has developed a chronic relapsing erosive arthritis as a result of sexually acquired Reiter's disease--an occurrence not previously reported in this age group. We draw attention to the frequency of diarrhoea in these children and the sex incidence of 1 female to 4--5 males, which agrees more with Reiter's disease of dysenteric origin than that acquired venereally.

Vascularity of the femoral head: 18fluorine scintigraphy validated with tetracycline labeling.

    Blood flow to the femoral head was assessed by 18F positron scintigraphy and tetracycline fluorescent labeling in 17 patients undergoing replacement of the femoral head with an endoprosthesis. Radioactivity was absent in the femoral head of 10 patients, normal in 5, and increased in 2. Normal or increased radioactivity indicated presence of blood flow to the femoral head; decreased or absent radioactivity indicated absence of blood flow. Results of 18F scintigraphy correlated with those of tetracycline labeling.
