Prolapse of the neovagina in Mayer-Rokitansky-Kuster-Hauser syndrome. A case report.
Mayer-Rokitansky-Kuster-Hauser syndrome is a rare entity. The creation of a sigmoid vagina was performed in some patients with this syndrome in the past, though it is not widely used now. We report on a patient who developed prolapse of a sigmoid vagina 33 years after the operation.. A 57-year-old woman presented with a "falling-out" sensation in the vagina, pain, leukorrhea and dyspareunia. She had undergone an operation for creation of a sigmoid vagina 33 years earlier in our hospital. She and her husband desired conservation of the ability for sexual intercourse. The transabdominal method of retroperitoneal sacropexy of the sigmoid vagina was performed. The patient has maintained a satisfactory sexual life with her husband since the operation.. There are a few cases of prolapse of a sigmoid vagina in the literature, while the repair methods are not described in detail. To our knowledge, this is the first report of reconstruction of a sigmoid vaginal prolapse. Although the reasons for the neovaginal prolapse were not understood, the retroperitoneal sacropexy was successful in this case. Dhat syndrome. A sex neurosis of the Indian subcontinent.
Dhat syndrome is a culture-bound sex neurosis of the Indian subcontinent. Fifty-two patients with a presenting complaint of passage of 'Dhat' in urine were studied. Diagnosis of neurotic depression, anxiety neurosis, hypochondriacal neurosis, and psychogenic impotence were made in 21, 19, 3, and 1 cases respectively. Seven patients received the diagnosis of pure Dhat syndrome, and one of gonorrhoea. A girl with the Weaver syndrome.
A female with the Weaver syndrome is reported. In addition to the characteristic manifestations of overgrowth and advanced bone age, the facies were typical, with a broad forehead, hypertelorism, a long philtrum, micrognathia, and large ears. Like most other patients with Weaver syndrome, she was developmentally delayed, hypertonic, and had a hoarse voice. Other clinical features included prominent finger pads, narrow hyperconvex nails, small and narrow chest, unilateral dislocated distal ulna, and abnormal thoracic vertebrae. Two cases of "pure" or "primary" erotomania successfully treated with pimozide.
Erotomania is a mono-delusional disorder which, in its "pure" or "primary" form, should probably belong to the diagnostic category of Paranoia. Traditionally, the prognosis and response to treatment are very poor, but one of the present authors has previously proposed that the condition should respond to pimozide. [The puberty syndrome: the acute obsessive crisis].
For a rather long period of time the authors have seen and followed up the evolution of a very characteristic syndrome. It is not frequent and it appears only in puberty, in boys as well as in girls, and it has been called "acute obsessive crisis". It appears suddenly in the form of a very intense anguish with the constant repetition of questions about the possibility of being in mortal sin and with the evidence of poorness of thought. The acute period lasts about 72 hours and then remission begins spontaneously and in one week it is all over. The evolution of this syndrome showed that it is not repeated later and that it does not leave any defect. It is essential to make the differential diagnosis with schizophrenia. The author's hypothesis is that acute obsessive crisis is the extreme manifestation of the painful mental experiences associated to corporal changes so frequent during puberty. Psychiatric status after hysterectomy. A one-year prospective follow-up.
Forty-four randomly selected women who received noncancer hysterectomy were evaluated preoperatively and at one-year follow-up. The findings did not support the hypothesis that the hysterectomy procedure is followed by an excess of psychiatric symptoms. The group, in fact, was less symptomatic at follow-up than presurgically. On the whole, women manifesting psychiatric symptoms during the follow-up had had similar problems before surgery. Many of the symptoms were associated with a presurgical diagnosis of hysteria (Briquet's somatization syndrome), found at a striking excess in the sample (27% vs the 1% or 2% expected). The authors suggest that when hysteria is diagnosed, therapeutic decisions regarding hysterectomy should be based on objective findings rather than on complaints, in that women with hysteria may be placed at risk of hysterectomy on the basis of psychiatric rather than gynecologic illness. Dhat syndrome: a culture-bound sex neurosis of the orient.
The Indian Dhat syndrome is a culture-bound symptom complex. The clinical picture includes severe anxiety and hypochondriasis. The patient is preoccupied with the excessive loss of semen by nocturnal emissions. There is a fear that semen is being lost, and mixed in urine. A study was carried out to investigate the cultural basis of the Dhat syndrome. One hundred and seven respondents from the general public were interviewed. A vignette describing an individual having nocturnal emissions was read aloud to the respondents. Attitudes toward nocturnal emission, its causes, and its management were investigated. A large segment of the general public from all socioeconomic classes believed that semen loss is harmful. Seminal fluid is considered an elixir of life both in the physical and in the mystical sense. Its preservation guarantees health, longevity, and supernatural powers. This belief is more frequent in lower socioeconomic classes. The susceptible individual reacts to the prevalent belief system and to the fears of semen loss. The symptoms usually disappear if the misconceptions about semen loss are effectively dealt with. It is expected that with increasing literacy and progress in sex knowledge the syndrome will become less common.
