Sexual functioning and the physically disabled adult.

    A person's sexual readjustment following a physical disability has traditionally been ignored by health care professionals. Since the occupational therapist often facilitates a person's resumption of activities of daily living, the therapist is in a special position to provide counseling. Understanding, support, and correct information are needed most. As derived from a search of the literature, sexual functioning is discussed in relation to the following disabilities: stroke, heart disease, diabetes mellitus, muscular dystrophy, multiple sclerosis, renal disease, spinal cord injury, pulmonary disease, arthritis, and alcoholism.

Typologies of alcohol consumption on a Saturday night among young adults.

    Alcohol policies and interventions seek to curtail risky single-occasion drinking (RSOD) and the negative health and public order consequences. Yet RSOD behaviors are not easily defined since people can drink excessively at a variety of locations and drink a range of products. The current study examines the presence and correlates of different typologies or classes of drinking behavior on 1 Saturday night to facilitate a nuanced policy response to harmful drinking.. Data from 1,883 adults aged 18 to 30 were collected using an online survey. Latent class analysis was used to categorize respondents into mutually exclusive classes based on the quantity, type, and unit cost of alcohol consumed plus location of alcohol consumption on the past Saturday night. Significant correlates and predictors of latent class membership were then identified using regression analysis.. Seven distinct classes were identified that represent qualitatively distinct profiles of Saturday night drinking behavior among young adults. Multivariate analyses indicated that alcohol risk (measured using the Alcohol Use Disorder Identification Test), age, and recent (past 12 months) stimulant use were strong predictors of heavier drinking. The heaviest drinkers also consumed some of the cheapest alcohol and consumed alcohol at multiple locations over the course of the night.. Given the large degree of heterogeneity among drinking behaviors, policy makers need to be cognizant that alcohol type and drinking location-specific policies may be less effective in targeting some groups of the population.

A population-based analysis of leaving the hospital against medical advice: incidence and associated variables.

    Prior studies of patients leaving hospital against medical advice (AMA) have been limited by not being population-based or assessing only one type of patient.. We used administrative data at the Manitoba Centre for Health Policy to evaluate all adult residents of Manitoba, Canada discharged alive from acute care hospitals between April 1, 1990 and February 28, 2009. We identified the rate of leaving AMA, and used multivariable logistic regression to identify socio-demographic and diagnostic variables associated with leaving AMA.. Of 1,916,104 live hospital discharges, 21,417 (1.11%) ended with the patient leaving AMA. The cohort contained 610,187 individuals, of whom 12,588 (2.06%) left AMA once and another 2986 (0.49%) left AMA more than once. The proportion of AMA discharges did not change over time. Alcohol and drug abuse was the diagnostic group with the highest proportion of AMA discharges, at 11.71%. Having left AMA previously had the strongest association with leaving AMA (odds ratio 170, 95% confidence interval 156-185). Leaving AMA was more common among men, those with lower average household incomes, histories of alcohol or drug abuse or HIV/AIDS. Major surgical procedures were associated with a much lower chance of leaving the hospital AMA.. The rate of leaving hospital AMA did not systematically change over time, but did vary based on patient and illness characteristics. Having left AMA in the past was highly predictive of subsequent AMA events.

Housing retention in single-site housing first for chronically homeless individuals with severe alcohol problems.

    We studied housing retention and its predictors in the single-site Housing First model.. Participants (n = 111) were chronically homeless people with severe alcohol problems who lived in a single-site Housing First program and participated in a larger nonrandomized controlled trial (2005-2008) conducted in Seattle, Washington. At baseline, participants responded to self-report questionnaires assessing demographic, illness burden, alcohol and other drug use, and psychiatric variables. Housing status was recorded over 2 years.. Participants were interested in housing, although a sizable minority did not believe they would be able to maintain abstinence-based housing. Only 23% of participants returned to homelessness during the 2-year follow-up. Commonly cited risk factors--alcohol and other drug use, illness burden, psychiatric symptoms, and homelessness history--did not predict resumed homelessness. Active drinkers were more likely to stay in this housing project than nondrinkers.. We found that single-site Housing First programming fills a gap in housing options for chronically homeless people with severe alcohol problems.

Association between gender and marital status and confidence in remaining abstinent among alcohol abusers in treatment.

    To assess the importance of gender and marital status on self-efficacy expectancies of alcohol abusers in treatment.. Cross-sectional study with a group of alcohol abusers in treatment (n = 259, 56 women) at the Bergen Clinics Foundation, Norway.. The Situational Confidence Questionnaire was applied for the self-efficacy assessment.. Self-reports of significant life problems were associated with low self-efficacy expectancies. Controlling for this, female abusers scored significantly higher than their male counterparts on risk of relapse in "positive emotional state" and "pleasant emotional states with others".. Situations perceived as being at high risk for relapse were associated with gender and significant life problems. The findings have implications for a more differentiated approach in the relapse prevention treatment programmes.

Gender differences in Axis I comorbidity among depressed outpatients.

    The aim of our study was to assess gender differences in Axis I comorbidity in patients with a primary diagnosis of Major Depressive Disorder (MDD), as well as gender differences in age of onset of MDD.. The presence of MDD, including age of onset, and of comorbid Axis I disorders were assessed in 396 depressed outpatients.. Women were significantly more likely than men to meet criteria for comorbid bulimia nervosa and for simple phobia, while men were significantly more likely than women to meet criteria for lifetime history of alcohol abuse/dependence and other substance abuse/dependence. No other significant gender differences in those comorbid Axis I disorders examined were observed. In addition, the age of onset of the first episode of MDD was significantly lower in women than in men.. Our findings are consistent with those of previous studies showing a greater prevalence of alcohol and substance abuse and dependence in men and of eating disorders in women.

Sex, alcohol and violence: a community collaborative action against striptease shows.

    Between September 1988 and February 1989, Anyinginyi Congress, an Aboriginal community-controlled health organisation, facilitated a collaborative community action against striptease shows in public bars in Tennant Creek. This action resulted in changes to the guidelines of the Northern Territory Liquor Act to regulate striptease shows in public bars and began other processes of addressing alcohol related problems in the community. The composition and strategies of both the pro- and anti-striptease lobbies are analysed within the context of changing power relationships between Aboriginal and non-Aboriginal people, as epitomised in the growth and consolidation of Aboriginal community-controlled organisations in Tennant Creek. The role of the media in shaping the course and direction of the debates as well as the response of the Northern Territory government as the final arbiter in the striptease conflict are scrutinised. The use of sex to sell alcohol is a legitimate public health concern and community action for healthier public policy is an important strategy in creating supporting environments for health.

Predicting onset and chronicity of women's problem drinking: a five-year longitudinal analysis.

    Longitudinal studies of adult drinking have typically excluded or sampled only small numbers of problem drinking women, and have measured a limited range of influences on women's drinking behavior.. To study the development of women's problem drinking over time, five-year follow-up interviews were conducted with two groups of respondents from a 1981 national survey of women's drinking: 143 problem drinkers and 157 nonproblem drinkers. Regression analyses examined effects of 1981 predictors on six measures of 1986 problem drinking, for problem drinkers and nonproblem drinkers separately.. Among 1981 nonproblem drinkers, predictors of onset of problem drinking indicators by 1986 included younger age, cohabiting, and lifetime use of drugs other than alcohol. The most consistent predictor of persistent (chronic) problem drinking was sexual dysfunction; other predictor included being employed part-time or never married, and experiencing recent depression. Divorce or separation predicted lower levels of subsequent alcohol dependance among problem drinkers.. Findings suggest that different personal and social factors predict the onset of problem drinking as compared with its continuation, and point to nontraditional life-style, sexual dysfunction, and role deprivation as potentially important variables.

[Changes in the spectral power of the EEG in alcoholics during presentation of stimuli of different semantic modality].

    Using bipolar registration of the EEG from two symmetrical temporal leads in 50 alcoholic patients and 20 normal subjects, the authors examined changes in the spectral power (SP) of the EEG in a range of 0.5-32 Hz in response to the presentation of 6 series of visual stimuli different by semantic modality. It was found that stimuli of an alcoholic nature induced in the patients the maximum increase in SP parameters and led to a change in the SP-dominant hemisphere. In contrast, the controls showed augmentation of SP in response to stimuli of food and sex modality without changes in lateralization. A conclusion is drawn about the dependence of SP parameters on the hierarchy of the individual requirements. From the standpoint of concepts about the pathological development of the personality, the authors analyze the qualitative features of EEG responses in alcoholics toward specific stimuli.

Sexual jealousy in alcoholics.

    A hundred patients referred to an alcoholism service were assessed in terms of sexual jealousy by interview and with a Sexual Jealousy Questionnaire. The overall incidence of sexual jealousy was 35% in men and 31% in women as judged by the interview. Twenty-seven per cent of the men and 15% of the women had morbid jealousy. There was no relationship between sexual jealousy, as measured by the questionnaire, and sexual dysfunction. However, in the men there were inverse relationships between the scores for sexual jealousy and measures of sexual satisfaction and marital stability, and in the women sexual jealousy scores correlated negatively with duration of the marriage and positively with severity of alcohol dependence.

Family therapy with Irish-Americans.

    This paper presents a paradigm for understanding the cultural values and family patterns of Irish-American families and offers suggestions for clinical intervention. Families' correspondence to the model described will depend upon their level of acculturation, socioeconomic status, and other contextual factors. The paradigm suggests a number of issues that may create difficulties for the therapist and for the family in therapy. In general, behavioral techniques, Bowen systems therapy, and strategic interventions may be more useful then techniques emphasizing expression of emotions in therapy or direct attempts to alter communication or family behavior in therapy sessions, as used in structural therapy.
