Improvement of intraoperative frozen section diagnosis in patients with biliary strictures by Levovist injection into the bile duct on color Doppler ultrasonography.
This study evaluates the efficiency of color Doppler ultrasonography-guided intraoperative pancreatic biopsy (CDUS-IPB) using Levovist injected into the bile duct in conjunction with stimulated acoustic emission (SAE) in patients with biliary strictures.. The study was performed on 12 patients. After completing a conventional intraoperative pancreatic biopsy (c-IPB), each subject underwent CDUS with SAE imaging using Levovist. Upon identification of the biliary stricture, the IPB was taken from the area surrounding the stricture on the same imaging setting. Section diagnosis of the CDUS-IPB specimen was compared to that of the c-IPB specimen and resected tissue.. Biliary strictures were identified as enhanced areas of color Doppler signal on CDUS. CDUS-IPB provided adequate specimens from the biliary strictures in all cases and corrected false-negative diagnoses by c-IPB in three cases. Section diagnosis by CDUS-IPB corresponded to the permanent section diagnosis. There were no complications.. CDUS-IPB with Levovist is an accurate diagnostic tool. The method is especially useful for patients with a suspected malignant biliary stricture who show no tumor mass in preoperative images and no evidence of malignancy on cytologic examinations. Contrast-enhanced three-dimensional transcranial color-coded sonography of intracranial stenoses.
Intracranial stenoses are associated with a considerable number of strokes each year. The clinical value of a workstation-based three-dimensional (3D) reconstruction system for transcranial color-coded sonography was evaluated in patients with intracranial stenosis or occlusion.. Twenty-six patients (13 men, 13 women; mean age, 57 years +/- 12 [SD]) with 36 intracranial stenoses or occlusions, as detected at two-dimensional (2D) color Doppler imaging (CDI) and digital subtraction angiography (DSA), underwent Levovist-enhanced power Doppler imaging (PDI), with subsequent 3D reconstruction. A workstation connected to a magnetic sensor capable of spatial localization of the probe was used to reconstruct 3D images of the circle of Willis from serial PDI images.. At DSA, seven (19%) stenoses were estimated to less than 50%, 24 (67%) were 50% or more, and five (14%) were occluded. DSA and 3D-PDI estimates of the degree of stenosis agreed in 33 cases (92%), with a weighted kappa value of 0.86. Disagreement occurred with two subtotal basilar artery stenoses and one subtotal middle cerebral artery stenosis, which were evaluated as being complete occlusions at 3D-PDI. Interobserver agreement between two experienced 3D investigators in estimating the percentage of stenosis was high (correlation coefficient,.98).. 3D-PDI enables the investigator to reconstruct virtually any arbitrary viewing angle. Compared with conventional CDI, 3D-PDI offers easier spatial assessment of intracranial stenoses, and its findings are sufficiently correlated with angiographic findings. Because different investigators can postprocess the same 3D data, improving reproducibility and reducing investigator dependency in transcranial color-coded sonography may be possible. [Follow-up of TIPSS by color-coded duplex sonography using an ultrasonic signal enhancer. First results].
A study was performed to determine the visualization of the transjugular intrahepatic portosystemic stent shunt (TIPSS) and the detection of stenosis by the use of a capillary transversing signal enhancer. In 37 patients 37 colour-coded duplex sonographies were performed before and after intravenous injection of the ultrasound signal enhancer Levovist (Schering, Berlin). The examinations were evaluated using a four-category score. Special attention was paid to the detection of stenoses in the TIPSS. Transjugular portal venograms of the same day were used as gold standard. The use of Levovist provided better colour and flow signals for the portal vein end of the shunt in only 9 of 37 sonograms and for the hepatic vein end of the shunt in 37 of 39 sonograms. Eleven of 13 stenoses requiring reintervention in portal venography could be correctly identified with signal enhancer. Eleven of these 13 stenoses were located in the hepatic vein end of the shunt. Ultrasound signal enhancer can significantly improve the sonomorphological visualization especially of the hepatic vein end of TIPSS in colour-coded duplex sonography. Stenoses which usually occur in the hepatic vein end of the shunt may be better detected.
