Diflunisal ('Dolobid') once-a-day in the treatment of rheumatoid arthritis.

    A 4-week, double-blind, controlled multi-centre study was carried out in 235 patients with active rheumatoid arthritis to compare the efficacy and tolerance of 500 mg or 1000 mg diflunisal per day administered once daily, in the evening, or in divided, twice daily dosage. The results showed that diflunisal given once daily was at least as effective as diflunisal given twice daily. Day pain, morning stiffness, average grip strength and erythrocyte sedimentation rate improved similarly in both groups. Significant differences favouring the once-daily regimen were observed for improvement in night pain, Ritchie index and overall assessments by patient and investigator. Adverse experiences were slightly more common in patients taking diflunisal once daily (24% vs 19%) but this difference was not significant. It is concluded, therefore, that diflunisal once-a-day is an alternative regimen for the treatment of rheumatoid arthritis. It is at least as effective as the twice-daily regimen and may provide additional convenience to the patient and potential enhancement of compliance.

Reduced faecal blood loss in patients receiving choline magnesium trisalicylate ('Trilisate') when compared with aspirin.

    A study was carried out in 16 healthy male volunteers to compare the effects on faecal blood loss of comparable daily doses of choline magnesium trisalicylate (3 g) and acetylsalicylic acid (2.4 g) given over a period of 7 days. Faecal blood loss was measured by 51Cr labelling of red cells. Haematological parameters were also monitored. Faecal blood loss and the reduction in haemoglobin levels were significantly less in the choline magnesium trisalicylate group than in the acetylsalicylic acid group. No significant differences were detected in the partial thromboplastin time, prothrombin time or bleeding time in either group.

[Upper gastrointestinal haemorrhage in the patients aged over 65 years. The contribution of endoscopy (author's transl)].

    One hundred and eighty four patients aged over 65 years and hospitalised for gastrointestinal haemorrhage of high origin underwent emergency oesophago-gastro-duodenoscopy. It was thereby possible to make an aetiological diagnosis in 94.5% of cases. The study demonstrated certain special features: the preponderance of acute gastro-duodenal lesions, possibly explained by the high consumption of gastrotoxic drugs in the elderly subject; the relative frequency of oesophagitis (11%), one out of two cases being responsible for the bleeding; the discovery of a hiatal hernia in 59 patients (1/3 of cases), 25% of them bleeding from a lesion directly related to the latter. Bleeding was in large quantity in 57% of cases. Mortality, which was quite high (17%) appeared to be more related to the underlying general condition than to the haemorrhage itself. Endoscopy gave rise to complications in 8 patients, including 3 who died.
