Keratinizing dermatoses. Combined data from four centers on short-term topical treatment with tretinoin.
In four medical centers, 40 patients with keratinizing dermatoses were treated with topical tretinoin (vitamin A acid) 0.1% cream and salicylic acid 2% cream in a short-term, double-blind study. Tretinoin was the more effective treatment for several of the keratinizing dermatoses with the exception of palmar-plantar hyperkeratosis, for which it was not effective in the concentration and method of application used. The most striking clinical responses occurred in patients with lamellar ichthyosis and ichthyosis vulgaris. Local adverse reactions-chiefly pruritus, erythema, burning, excoriation, and irritation-were not severe and could be controlled by modification of the treatment regimen. Stratum corneum changes in patients with senile pruritus.
Generalized pruritus in the elderly is a common and distressing problem; often there is no evidence of skin disease other than xerosis.. The aim of the study was to determine whether any abnormality could be detected in the structure and function of the skin of patients with generalized pruritus.. The skin of 13 elderly patients with generalized pruritus, without skin disease or any underlying cause, was contrasted with that of age- and sex-matched normal control subjects.. The patients had clinically drier skin (mean visual analogue scale score 2.9 [standard deviation +/- 2.2], controls 0.52 [+/- 0.59], p = 0.002). The severity of the pruritus was related to the degree of xerosis (r = 0.66). The patients had decreased skin surface conductance (10.7 mumho [+/- 3.4], controls 16 mumho [+/- 5.3], p = 0.017), and increased intracorneal cohesion (240.5 g [+/- 88], controls 162.7 g [+/- 39.8], p = 0.001). The patients also had statistically significantly diminished parameters of skin surface contour.. The findings of increased intracorneal cohesion and altered skin surface contour parameters suggest that elderly patients with generalized pruritus may have an acquired abnormality of keratinization. [Water content of the skin following salicylic acid and urea treatment].
Water content of the epidermis was measured before and after a 3-h , 4-day, and 10-day treatment using 10% salicylic acid or 10% carbamide ointment. The measurement was performed using the Corneometer CM 420 from the firm "Schwarzhaupt Medizintechnik GmbH". Salicylic acid ointment was applied on one and carbamide ointment on the other back of the hand. Neither salicylic acid nor carbamide seem to influence significantly the water content of the healthy epidermis. In two patients with more severe hyperkeratosis carbamide increased the water content more than salicylic acid did. Familial X-linked ichthyosis, steroid sulfatase deficiency, mental retardation, and nullisomy for Xp223-pter.
Steroid sulfatase (STS)-deficient X-linked ichthyosis was diagnosed in a man with short stature and mental retardation. His generation includes five similarly affected male members. A translocation chromosome is segregating in this Newfoundland kindred. The proband's mother and grandmother have normal skin and are of normal intelligence. From his carrier mother, the proband inherited an X short arm (Xp) to Y long arm (Yq) translocation chromosome, with the entire Y short arm and the X short arm terminal segment deleted (Xp223-pter). His cells are completely deficient in STS activity, confirming assignment of the STS locus to Xp223-pter. Effective management of his ichthyosis included treatment with 6% salicylic acid gel under plastic occlusion and removal of the scales by scrubbing. A group of genodermatoses that differ in clinical appearance and cellular kinetics are included in the classification of ichthyosis. This condition is characterized by the formation of thick fish-like scales over the skin surface and may cause significant disability. Ichthyosis in two dogs.
Ichthyosis in 2 dogs was characterized by extreme hyperkeratosis on all or part of the skin and by extreme thickening of the digital and metapodial pads. The skin disorder closely resembled human lamellar ichthyosis, in which the histologic features include a thickened granular layer, mitotic figures, and dense collections of coarse keratohyaline granules. The disease was believed to be inherited, probably autosomal recessive, inasmuch as none of the parents was affected. Treatment of 1 dog with 5% lactic acid in hydrophilic ointment reduced the hyperkeratosis and decreased the shedding of scales.
