    A number of pharmacological agents can induce hyperuricaemia, and sometimes gout, usually by interfering with the renal tubular excretion of urate but also in some instances by increasing the formation of uric acid. Alcohol is well known to have this property and in recent years diuretic-induced hyperuricaemia has become a global phenomenon. Other drugs which can cause hyperuricaemia are salicylates, pyrazinamide, ethambutol, nicotinic acid, cyclosporin, 2-ethylamino-1,3,4-thiadiazole, fructose and cytotoxic agents. A special type of 'drug-induced gout' can follow the rapid lowering of serum uric acid by allopurinol or uricosuric drugs.

[The uric acid-lowering effect of diflunisal].

    20 patients with elevated serum uric acid levels and osteoarthritis of the hip or knee were given diflunisal 500 mg b.i.d. The joint complaints were influenced good to excellent in all patients. The uric acid levels were decreased from 7.37 to 6.22 mg/dl for the duration of this eight week open study. Diflunisal can be advocated also for the treatment of gout.

Comparison of the urate lowering effects of allopurinol and diflunisal.

    The urate lowering effect of diflunisal, a fluorinated salicylate with an antiinflammatory action, was compared with that of allopurinol in a crossover study. When patients took diflunisal the mean serum urate was significantly greater than on allopurinol but still was within the optimal range in 6 of 8 patients studied. This effect was consistent with its uricosuric action. No inhibition of xanthine oxidase activity by diflunisal was found. Diflunisal could be particularly useful in patients who require a drug with both a uricosuric and an antiinflammatory effect.

Hypouricemic action of diflunisal in gouty patients: in vitro and in vivo studies.

    Sixteen hyperuricemic gouty patients were treated with diflunisal, a novel salicylate, 500 mg BID. Serum and urine uric acid along with uric acid clearances were studied before and after a 7-day treatment. A clear hypouricemic action was observed, but 2 different mechanisms of action were seen when overexcretor patients and normoexcretor patients were analyzed separately. In overexcretors an allopurinol-like action was evident, whereas in normoexcretors a uricosuric action occurred. In vitro experiments showed a competitive inhibition of xanthine-oxidase exerted by diflunisal at low concentrations.

[Treatment of painful muscular disorders of various degrees of severity. Comparison of 2 new suppository preparations].

    In clinical rheumatology, the hazardousness of mixed preparations containing cortisone is being stressed even more, their use is only recommended in very special cases and only for short periods. Dolo Visano Supp. sine codeino and Dolo Visano Supp. are remedies which are indicated, like other mixed preparations not containing steroids, in cases of painful muscular overstrain and psychomuscular kinesalgia (symptom range of non-articular rheumatism). In our study, they were used in acute and severe pain conditions in intermediate or short-term therapy until complaints improved. Dolo Visano Supp. sine codeino and Dolo Visano Supp. (with codeine) can contribute in limiting the use of the risky preparations containing cortisone, which have hitherto been applied so frequently.

Genetic studies in primary gout. Investigations on the plasma levels of the urate-binding alpha 1-alpha 2-globulin in individuals from two gouty kindreds.

    The plasma levels of the urate-binding alpha(1)-alpha(2)-globulin, as determined by its urate-binding capacity, have been recorded in 19 individuals from two gouty kindreds. A significantly reduced binding capacity, accounting for 13-30% of the mean value obtained in healthy, unrelated control subjects, was found in all cases of gout and in the single case of essential hyperuricemia included in the present study. In addition, six apparently healthy members of one of these kindreds also exhibited this characteristic. The distribution of the characteristic in three subsequent generations from this kindred further supported the hypothesis that the reduced binding capacity was inherited as an autosomal trait for which affected subjects were heterozygous. Based on the present observation, the mechanisms of inheritance in primary gout are discussed with special emphasis on the possible cooperation of genetic and environmental factors.
