Clinical and in vitro efficacy of colistin plus vancomycin and rifampin against colistin-resistant Acinetobacter baumannii causing ventilator-associated pneumonia.
We present the case of a patient with ventilator-associated pneumonia (VAP) caused by a pan-resistant Acinetobacter baumannii successfully treated with the combination colistin plus vancomycin plus rifampin, whose in vitro activity was investigated by checkerboard method and killing testing. Furthermore, the serum bactericidal activity (SBA) was assessed. Our case shows that an innovative regimen consisting of colistin plus antimicrobials active only against Gram-positive microorganisms might represent a valid therapeutic option for severe infections caused by colistin-resistant A. baumannii. [Brucellar meningitis complicated by aneurysmal subarachnoid hemorrhage].
Brucellosis is a rare disease in which neurological complications, and more particularly, intracranial aneurysmal hemorrhages were rarely reported.. A brucellar meningitis was diagnosed in a 29-year-old man, who had been suffering from an unusual headache for about two months. Three days after beginning treatment, the patient developed an intracranial right frontal hemorrhage that led to the discovery of a ruptured anterior communicating artery aneurysm. The aneurysm was clipped surgically and doxycycline and rifampin were given for three months. The outcome was excellent.. Clinical manifestations of brucellosis are variable. Neurovascular complications are rare and probably linked to delay in diagnosis of CNS invasion or inadequate initial treatment. Appropriate and prompt medical, and if necessary, surgical management can lead to excellent outcome. Brucella-related cerebral aneurysms/subarachnoidal hemorrhage: a short review featuring a case report.
Brucellosis is a multisystem disease that may present with a large spectrum of clinical manifestations. Only five cases of intracranial aneurysm formation and/or subarachnoidal hemorrhage associated with brucellosis have been reported. In this paper, we take the opportunity to review these reports and present a new case of basilar artery aneurysm and subarachnoidal hemorrhage due to brucellosis.
