    We report the case of a 28-year-old resident doctor with no past history of having taken rifampicin, who presented with thrombocytoapaenic purpura occurring after the initiation of anti-tuberculosis therapy (isoniazid, rifampicin, pyrazinamide and ethambutol) for tubercular lymphadenopathy.

    Drug-induced immune thrombocytopenia (DITP) can be triggered by a wide range of medications. Although many cases of DITP are mild, some are characterised by life-threatening bleeding symptoms. In the treatment of tuberculosis there are special therapeutic problems related to adverse effects of drugs, compliance to treatment and microbial resistance. Thrombocytopenia is an uncommon but potentially fatal adverse effect of certain antituberculous drugs when the incriminating drug is taken by a susceptible individual. Here the authors report a case of rifampicin-induced thrombocytopenia, which although rare, needs attention.

Acute thrombocytopenic purpura in childhood brucellosis.

    Two children who presented with fever, thrombocytopenic purpura and mucosal haemorrhages proved to have brucellosis. Large platelets in the peripheral smear and megakaryocytic hyperplasia in the bone marrow suggested increased peripheral destruction as the primary mechanism of the thrombocytopenia. There was a prompt clinical and haematological response to specific anti-brucella chemotherapy. The nature of this association and its implications for brucella-endemic areas are discussed.

["Immuno-allergic" complications of rifampicin therapy].

    From 8 cases of immuno-allergic accidents attributed to Rifampicine, the authors review the literature on the subject. The "flue like" syndrome is the most frequent and characteristic of those accidents, worsened by haematological and renal involvements. These accidents probably belong to a pathology of immuno-complexes in relaiton with the production of anti-Rifampicine antibodies. Among the different means of in vitro diagnosis, the test with the anti-complement antiglobulin is the most reliable, though not always in good correlation with clinical signs. Though rare and most often benign, these accidents should lead to prudence in treating recurrent tuberculosis or reusing Rifampicine after a momentary interruption.
