Rifampicin-erythromycin combination for the treatment of gonococcal urethritis in men.
A total of 184 men with uncomplicated gonococcal urethritis were treated with a single oral dose of rifampicin 900 mg and erythromycin stearate 1 gm. The cure rate on day 3 was 95.1%, 175 were cured and 9 failed (4.9%). Of all the Neisseria gonorrhoeae isolated, 86 were PPNG (46.7%). There was no difference in failure rate among patients with PPNG or with non-PPNG. With the exception of self-limited gastrointestinal disturbance, side effects were minimal. The value of this combination for the treatment of uncomplicated gonococcal urethritis in men is promising and deserves further study. Treatment of uncomplicated gonorrhoea in women with a combination of rifampicin and erythromycin.
One hundred women with uncomplicated gonorrhoea (in five cases due to penicillinase producing strains of Neisseria gonorrhoeae (PPNG)) were treated with a single oral dose of rifampicin 900 mg and erythromycin stearate 1 g. N gonorrhoeae was reisolated from the oropharynx of one patient, who was infected with a PPNG strain, but was eradicated from the genital tract in 100% of cases. The combination eradicated Chlamydia trachomatis from only 10 (28%) of the 36 patients infected. Side effects were predominantly mild and consisted of transient nausea. The treatment merits evaluation in areas with a high incidence of PPNG strains. [1,200 mg rifampicin(Rimactan) as an oral one-dose treatment of uncomplicated gonorrhea in women].
40 women were treated for uncomplicated genital gonorrhoea, verified by culture, with a single oral dose of 1,200 mg rifampicin (Rimactan). The ages ranged between 15 and 51 years. In 33 patients that could be evaluated, the gonorrhoea was cured. In 8 of 18 patients the preexistent vaginal discharge improved. 4 women experienced six different side effects which were probably related to rifampicin. The 1,200-mg rifampicin oral one-dose treatment is a reliable, efficient, and well-tolerated therapy of uncomplicated gonorrhoea in women. Acute gonococcal urethritis: treatment with a single dose of rifampicin.
A total of 103 patients with acute gonococcal urethritis were treated with a single 1,200 mg dose of rifampicin. A 91 . 3% cure rate was obtained, as proved by the negative bacteriological controls effected on the 7th and 14th days after the initiation of therapy. Three pharyngeal infections and one ano-rectal infection responded successfully to the treatment. No signs of drug intolerance were detected with the stated dose. Reactivity to the VDRL test was not impaired during the duration of the study and three reactive cases were discovered. In previous studies of gonorrhoea we had observed a significant discrepancy between urine cultures and the urethral smears and, in view of this, it was decided to adopt the latter alone as a routine procedure. The proposed dose does not originate resistance to rifampicin in either the Hansen or Koch bacilli. [Rifampicin in the therapy of gonorrheal urethritis in men].
The therapeutic efficiency of benemycin (rifampicin of Polish production), a semisynthetic antibiotic was studied in 96 male cases with gonorrhea urethritis. The antibiotic was used in a dose of 300 mg every 6 hours (2.1--3gm for the treatment course depending on the desease severity). Observation of the patients for 1--2 months showed etiological recovery in 91 (94.8 per cent) out of 96 patients. Postgonorrhea inflammatory processes were observed in 8.7 per cent of the cases. For studying late results of the treatmant 62 patients were observed for 3 to 12 months. Gonococci were isolated from none of the patients. No side reactions were found in the patients treated with rifampicin. Recent developments in the treatment of gonorrhoea.
Rising failure rates with single injections of penicillin in gonorrhea are causing alarm. In the Far East, the failure rate is about 30% after single injections of 2.4 mega units of aqueous procaine penicillin. In London, although there is rising gonoccoccus resistance to penicillin, single injections of 1.2 mega units of aqueous procaine penicillin nets a cure rate of 85.8% to 91.5% in male patients. Other recent experiences in the treatment of gonorrhea in London men are outlined in this article. The failure rate was reduced to 5.8% when a single injection of 2.4 mega units of procaine penicillin was given. When 1 gm probenecid was given orally along with 1.2 mega units of procaine penicillin, the failure rate was 6.6%. Streptomycin is so ineffective now against gonorrhea that it is no longer acceptable even as a first treatment in patients who are allergic to penicillin. In London, the failure rate with streptomycin is 31.7%. Experts think the rate will jump to 85.8% in the next year. Many other antibiotics can be used against gonorrhea, but, as in the case of streptomycin, they may lose their effectiveness. Some recent experiences show that a combination of rolitetracycline by injection and tetracycline by mouth does not give as good results as a single injection of aqueous procaine penicillin but that comparable results can be obtained with single oral doses of rifampicin and superior results (95.7% success) were obtained with 2 oral doses of dementhylchlortetracycline. In vitro susceptibility of Neisseria gonorrhoeae to nine antimicrobial agents.
The in vitro action of nine antibiotics was tested by the agar streak method against 45 gonococcal strains isolated from penicillin-therapy failures. The penicillin susceptibility range of these strains was 0.003 to 1.32 mug/ml, and the tetracycline susceptibility range was 0.125 to 2.0 mug/ml. Minimal inhibitory concentrations of minocycline and doxycycline paralleled the activity of tetracycline and ranged from 0.125 to 1.0 mug/ml and 0.125 to 2.0 mug/ml, respectively. Rifampicin, with a narrow range of 0.5 to 1.0 mug/ml, inhibited 75% of the strains at 0.5 mug/ml. The range for cephaloridine and cephaloglycine was 0.5 to 20.0 mug/ml, but another cephalosporium derivative, cephalexin, exhibited greater activity in its range of 0.25 to 20.0 mug/ml. A semisynthetic penicillin, carbenicillin, with a range of 0.025 to 0.75 mug/ml, displayed more activity against the lower susceptible penicillin G gonococcal strains. Treatment of gonorrhoea with single oral doses of rifampicin.
Of 103 cases of acute uncomplicated gonorrhoea treated with single oral doses of 900 mg. of a new antibiotic rifampicin 89 were followed, and 10 (11.2%) apparent failures were found.Rifampicin gives results which compare favourably with those obtained under like conditions with similar doses of other orally administered antibiotics.
