Miliary tuberculosis and choroidal tuberculoma in a three-month old baby girl: Diagnosis and follow-up of a case.
A three-month old baby girl presented with fever of unknown origin and with signs of worsening of this episode. Funduscopy showed a solitary choroidal lesion in her left eye, as well as extraocular lesions suggesting disseminated tuberculosis. A favourable outcome was achieved after quadruple antibiotic course and cortisone therapy. Disseminated tuberculosis should be considered in cases of fever of unknown origin in children with an unsatisfactory evolution. Ocular examination is mandatory, due to the possible posterior uveitis signs that can help with early diagnosis and treatment of some diseases. Clinical presentations and therapeutic effect of presumed choroidal tuberculosis.
To present the clinical findings and to evaluate the therapeutic response to systemic antituberculosis (anti-TB) therapy in 18 cases of presumed choroidal TB.. Retrospective noncomparative interventional case series. Eighteen patients underwent a complete ophthalmologic examination and relevant laboratory and radiologic investigations. All patients were treated with systemic anti-TB therapy accordingly.. In 18 cases of choroidal TB, there were 8 men and 10 women ranging in age from 8 years to 52 years. Ten patients also had evidence of pulmonary TB and among of them, two patients had coexistent central nervous system TB, one patient had lymphoid TB, and one patient had abdominal TB. Those cases had varied clinical presentations including tubercle, tuberculoma, multifocal choroiditis, serpiginous like choroiditis, and/or diffuse choroiditis. Two cases (11.1%) had bilateral presentation. All patients were referred to a TB specialist to initiate a full standard course of anti-TB therapy according to the guideline of Centers for Disease Control and Prevention. Aside from one case, which worsened to panophthalmitis, the others responded to anti-TB therapy very well. None of the patients had human immunodeficiency virus and none was immunocompromised.. Choroidal TB may present as variable clinical manifestations and occasionally be misdiagnosed as intraocular tumor or metastatic tumor. In our case series, the prognosis of this disease was well by giving systemic anti-TB therapy. So all ophthalmologists especially in developing countries should have a high degree of clinical suspicion to maximally maintain visual acuity. [Ocular bartonellosis: report of three clinical cases].
The characteristic clinical presentation of cat scratch disease is subacute regional lymphadenopathy; nevertheless, 5-25% of Bartonella henselae infections may present an atypical or systemic form, with potential eye involvement. We describe three clinical cases of ocular bartonellosis in two adolescents and one young adult, who had close contact with cats; all of them presented persistent fever ranging from 15 to 21 days, and two of them developed a sudden unilateral loss of visual acuity associated with optic neuritis. The other patient presented retinal choroiditis and unilateral retinal microgranulomas, with normal visual acuity. Patients received macrolides as sole antimicrobial or in association with rifampin, and one patient was additionally treated with systemic corticoids. The outcome was favorable in two patients; one patient developed a permanent visual deficit. Ocular bartonellosis must be suspected in patients with close contact to cats or with cat scratches whom develop persistent fever and sudden loss of visual acuity.
