[Treatment of diffuse peritonitis in children].
Improving the treatment of advanced peritonitis via use in therapy antihypoxant Reamberin and hepatoprotector Remaxol, nutritional support, sanitation laparoscopic abdominal cavity.. A total of 232 children aged 1 to 15 years with generalized purulent peritonitis treated at the children's surgical departments of Samara from 2001 to 2014. A study group comprised 148 patients who used the optimized pathogenetic therapy. In the study group was allocated two groups: 64 patients in the pathogenetic therapy that used antihypoxant reamberin, and 84 children in the treatment of which reamberin and hepatoprotector remaxol. All the children of the main group received nutritional support (trophic feedings), used in the surgical treatment of abdominal laparoscopic sanitation. Comprehensive survey includes the study of the dynamics of the level of white blood cells, leukocyte index on Kalf-Caliph, erythrocyte sedimentation rate, temperature, total albumin concentration, transaminase levels.. Comparison of the studied parameters in the study and control groups, showed a more rapid decrease in the symptoms and signs of intoxication (leukocytosis, LII, body temperature), relief of enteric disease, recovery of protein-synthetic function of the liver, a decrease of cytolytic and mesenchymal-inflammatory syndrome in the study group, especially in the subgroup in which therapy was included remaxol.. Optimization of treatment involving the application of the combined drugs--antihypoxant reamberin, hepatoprotector remaxol, nutritional support and implementation of laparoscopic abdominal sanitation led to improved results of therapy common purulent peritonitis in children.. Цель исследования - совершенствование лечения распространенного гнойного перитонита с помощью применения в терапии антигипоксанта Реамберина и гепатопротектора Ремаксола, нутритивной поддержки, лапароскопической санации брюшной полости. Материал и методы. Обследованы 232 ребенка в возрасте от 1 до 15 лет с распространенным гнойным перитонитом, находившихся на лечении в детских хирургических отделениях Самары с 2001 по 2014 г. Основную группу составили 148 пациентов, получавших оптимизированную патогенетическую терапию. В основной группе были выделены 2 подгруппы: в патогенетической терапии которых применяли антигипоксант Реамберин (64 пациента) и Реамберин плюс гепатопротектор Ремаксол (84 ребенка). Все дети основной группы получали нутритивную поддержку (трофическое питание), в хирургическом лечении использована лапароскопическая санация брюшной полости. В контрольную группу вошли 84 ребенка с распространенным гнойным перитонитом, получавших стандартную патогенетическую терапию. Комплексное обследование включало изучение динамики уровня лейкоцитов, лейкоцитарного индекса интоксикации по Я.Я. Кальф-Калифу, скорости оседания эритроцитов, температуры, общей концентрации альбумина, уровня трансаминаз. Результаты. Сравнение исследуемых показателей в основной и контрольной группах выявило более быстрое снижение динамики симптомов интоксикации (лейкоцитоз, ЛИИ, температура тела), купирование энтеральной недостаточности, восстановление белково-синтетической функции печени, уменьшение цитолитического и мезенхимально-воспалительного синдромов в основной группе, особенно в той ее подгруппе, где в терапию был включен Ремаксол. Выводы. Лечение, предусматривающее применение комбинированных препаратов - антигипоксанта Реамберина, гепатопротектора Ремаксола, нутритивной поддержки и внедрение лапароскопической санации брюшной полости, позволило добиться улучшения результатов терапии распространенных гнойных перитонитов у детей. [The immunoregulating action of reamberine and its use in the treatment of duffuse peritonitis].
Influence of reamberin on dynamics of functional condition of mitogen-induced immunocompetent blood cells at patients with widespread purulent peritonitis is studied. It is established that investigated immunocompetent cells have regulating effect on migration of neutrophil leukocytes at widespread purulent peritonitis during all postoperative period. PHA-activated mononuclear cells don't cause inhibition of migration of neutrophil granulocytes, observed in norm. At the same time the LPS-activated monocytes lead to migration strengthening of neutrophil granulocytes, in comparison with the data received in norm. Usage of reamberin at patients with widespread purulent peritonitis renders corrective action on immunocompetent cells, changing their properties of regulation of migratory activity of neutrophil granulocytes. The revealed effects at use of reamberin most expressed by the 7-10th days of the postoperative period. [Reamberin in combined therapy of general peritonitis].
Randomized studies of combined treatment of 138 patients with toxic phase of general peritonitis were accomplished. We demonstrated a significant decrease in electric activity of the intestine and alteration of free-radical processes in patients with generalized purulent peritonitis. Administration of reamberin which contains succinic acid promoted restoration of the intestine's peristalsis to the 3rd day of postoperative period and effective correction of imbalance in free-radical processes system. Usage of reamberin in combined treatment of general peritonitis let lethality to be diminished from 16.49 to 9.76%. [The effectiveness of vacuum therapy and meglumine sodium succinate in unformed fistulas of the gastrointestinal tract (clinical observations)].
The incidence of intestinal fistulas after laparotomy for various reasons (peritonitis, acute pancreatitis or trauma) is 1.5%. Fistula formation in patients with chronic gastrointestinal diseases has a rapid onset, severe course, and poor prognosis. Against the background of a long course of the disease and depletion of the body, there is a decrease in the activity of reparative processes, which leads to the manifestation of postoperative complications: the formation of fistulas, insolvency of intestinal anastomoses, peritonitis. Vacuum drainage is a treatment method aimed at eliminating exudate, reducing the area of the wound and its epithelization. The inclusion of a succinate-containing solution in the treatment regimen improves metabolic processes and improves the prognosis of the disease. As an illustration, a description of the clinical observation of patients with similar pathology and different treatment regimens is given.. Частота возникновения кишечных свищей после лапаротомий по поводу разных причин (перитонит, острый панкреатит или травма) составляет 1,5%. Образование свищей у пациентов с хроническими заболеваниями желудочно-кишечного тракта имеет стремительное начало, характеризуется тяжелым течением и неблагоприятным прогнозом. На фоне длительного течения заболевания и истощения организма происходит снижение активности репаративных процессов, что приводит к проявлению послеоперационных осложнений: образованию свищей, несостоятельности кишечных анастомозов, перитонита. Вакуумное дренирование является методом лечения, направленным на элиминацию экссудата, уменьшение площади раны и ее эпителизацию. Включение в схему лечения сукцинатсодержащего раствора способствует улучшению метаболических процессов и прогноза заболевания. [Prophylaxis and correction of inflammatory complications in peritonitis, depending on severity of enteral insufficiency].
There were examined 54 patients, in whom peritonitis of various etiology was diagnosed. Clinical, biochemical, immunological and other investigations were conducted for estimation of the enteral insufficiency syndrome severity. Basing on the analysis of the data obtained and criterions of the patient common state there was determined the occurrence risk for inflammatory complications (IC). Complex of prophylactic and therapeutic measures for reduction of the IC rate was proposed. [Opportunities of correction of rheological properties of blood in intensive therapy in patients with peritonitis and acute bowel obstruction].
An analysis of efficacy of 1.5% reamberin solution usage was made in 80 patients for correction of blood rheological properties disorder in the case of peritonitis and bowel obstruction. It was established, that reamberin, which was administered in the dose of 500 ml after 12 hours of the beginning of intensive care during 48 hours, efficiently corrected the rheological disorders. It was because the erythron and reticulocyte production were activated, the hyposmolarity was removed, the intensity of free radical oxidation and peroxide oxidation of lipids was decreased and the antioxidant status was restored. This facilitated the hydrophily and erythrocyte volume decrease, the haemolysis reduction and qualitative gas exchange at the level of tissues. [Effect of reamberin on the lipid spectrum of the liver mitochondria in experimental widespread purulent peritonitis].
Lipid spectrum of liver mitochondria has been studied in a group of 46 Chinchilla rabbits with experimental widespread purulent peritonitis. It is established that the drug reamberin that contains succinic acid produces a positive effect on the protein spectrum of liver mitochondria under these conditions, which is manifested by a decrease in the content of lysophosphatidylcholine and increases the levels of polyglycerophosphatides and sphyngomyelin.
