Quetiapine and cardiac muscle disorders.
Several detailed case reports have described cardiac muscle disorders (cardiomyopathy and myocarditis) in patients treated with quetiapine, some of which have been fatal. The symptoms included shortness of breath and oedema. The disorders sometimes resolved on withdrawal of quetiapine. Quetiapine is chemically similar to clozapine and olanzapine, which are known to sometimes provoke this type of adverse effect. In practice, a patient who develops dyspnoea or other signs of heart failure during quetiapine therapy may benefit if the drug's role is recognised and quetiapine withdrawn. [Edema related to treatment with olanzapine].
In comparison to metabolic syndrome, edema seems to be a rare occurrence in antipsychotic treatment.. In this paper, we report on two subjects with psychotic disorders who were found to have peripheral edema during olanzapine therapy.. The theoretical background and the relevance of this observation are examined. Furthermore, edema as a side effect of antipsychotic treatment is reviewed. In particular, the importance of olanzapine, a commonly used neuroleptic drug, as a possible causative substance is evaluated. Topics: Adult; Antipsychotic Agents; Benzodiazepines; Dibenzothiazepines; Drug Therapy, Combination; Edema; Female; Humans; Male; Olanzapine; Psychotic Disorders; Quetiapine Fumarate; Risperidone | 2009 |
