[A man with a rare disfiguring cutaneous manifestation of sarcoidosis].
A 42-year-old man with no relevant medical history presented with a painless, red, disfiguring skin lesion located on the nose. He was diagnosed with lupus pernio, a rare, cutaneous subtype of sarcoidosis which is relatively resistant to therapy and is associated with systemic sarcoidosis. [Sarcoidosis in and around the nose: a diagnostic and therapeutic guideline].
In addition to its mucosal pathology, sarcoidosis of the nose may cause a range of skin abnormalities and destruction of the osteocartilaginous structures of the nose. Clinical presentation can mimic other diseases and a biopsy is often necessary. We propose a classification of nasal sarcoidosis based on disease location to help with recognition of the symptoms and, subsequently, treatment. Here we describe 4 patients with nasal sarcoidosis. In the first patient, a 53-year-old woman with a granuloma in the nasal vestibule, the lesion was excised. The second patient, a 38-year-old man with intranasal crusts, synechiae and atrophic mucosa, was treated with corticosteroid nasal drops. The third and fourth patients, a 36-year-old man with progressive widening of the nasal dorsum and a 60-year-old woman with a saddle nose, respectively, were treated with a rhinoplasty. Sarcoidosis is a multisystem inflammatory disease that manifests as noncaseating granulomas, commonly in the lungs and intrathoracic lymph nodes. Subcutaneous manifestations of sarcoidosis that are caused by granulomas are referred to as specific for sarcoidosis, whereas other lesions are considered nonspecific. Lupus pernio--a patient case study.
An 80 year old female patient was seen in the Otolaryngology department with a skin manifestation of her sarcoidosis, known as lupus pernio. Around 25% of patients with sarcoidosis have skin disease and the presence of lupus pernio may suggest a more aggressive form of the disease. Photography of this relatively rare presentation not only helped us diagnose her underlying medical condition but also served as a baseline during treatment. Disseminated cutaneous coccidioidomycosis masquerading as lupus pernio.
Coccidioides immitis, a pathogenic fungus endemic to arid regions, is the etiologic agent of cutaneous coccidioidomycosis. Primary cutaneous coccidioidomycosis is rare. The majority of cutaneous coccidioidomycosis infections are caused by dissemination of the fungus from the lungs to the skin. Diagnosing cutaneous coccidioidomycosis often can be difficult because it can mimic a variety of other clinical conditions. We present a case of a 45-year-old man presenting with cutaneous coccidioidomycosis on the tip of his nose. This patient had pulmonary symptoms in addition to his cutaneous findings, leading to a diagnosis of disseminated coccidioidomycosis. A case of sarcoidosis mimicking rhinophyma.
A 54-year-old man presented with symmetric, bulbous, nodular enlargement of his nasal tip and ala. At first inspection, the changes appeared to be consistent with rhinophyma, but biopsy revealed noncaseating granulomata consistent with sarcoidosis. His laboratory, radiographic, and functional studies were consistent with systemic involvement of his disease. Cutaneous sarcoidosis presenting as a testicular mass.
Urologic involvement in sarcoidosis is rare. We report a patient who presented with a testicular mass and, shortly thereafter, manifested nasal lesions of the papular variant of cutaneous sarcoidosis. Histologic examination from both organ systems yielded sarcoidal granulomas. The patient was successfully treated using oral corticosteroids. Sarcoidosis continues to replace syphilis as "the great imitator of the 1990s," and clinicians must be increasingly aware of its variable presentations. Mutilating facial sarcoidosis.
We report a patient with massive facial sarcoidosis. While skin involvement is a common manifestation of sarcoidosis, it is unusual to see it in the dramatic form of cutaneous tumors with mutilation of the central face. Granulomatous lesions in nasal biopsies.
A series of 19 cases has been reviewed in which biopsy of an intra-nasal lesion revealed a granulomatous pathology. These have been classified on an aetiological basis. They include infections, Wegener's granuloma and neoplasms with a granulomatous stroma. One patient with sarcoidosis first presented with lesions in the nasal cavity. Cholesterol granulomata were seen in four lesions removed from the paranasal sinuses. In six cases clinical and histological examination failed to show a cause for the granulomata; in all of these patients the nasal cavity was free from disease at a subsequent examination. Thermography of the nose and ear in relation to the skin lesions of lepromatous leprosy, tuberculosis, leishmaniasis, and lupus pernio.
The nasal and aural temperature patterns of 100 normal subjects have been investigated by infrared thermography, paying particular attention to possible errors of instrumentation and technique which may arise in such areas of complex morphology. Although by no means invariable, the pattern of themograms confirms that certain areas which are relatively cool are often affected in lepromatous leprosy, tuberculosis, leishmaniasis, and lupus pernio. In lepromatous leprosy, low temperature appears to govern the localization of disease in most parts of the body, and the possible reasons for this are discussed. Thermography may have a place in the investigation of other skin diseases in which the distribution of lesions on the body surface is unexplained. Nasal sarcoidosis.
Nasal sarcoidosis may affect nasal skin, mucosa, or bone separately or simulataneously. Its incidence in patients with systemic sarcoid was once thought to be low, but this may be due to lack of proper intranasal examination and awareness of its existence by physicians who are more preoccupied with lung and other visceral involvement. The otolaryngologist should be aware of nasal sarcoidosis because nasal obstruction or drainage secondary to nasal sarcoidosis may be the first and only manifestation of systemic sarcoidosis. The otolaryngologist can diagnose this disease earlier in its course by being aware of its existence.
