6-Phosphogluconate dehydrogenase links oxidative PPP, lipogenesis and tumour growth by inhibiting LKB1-AMPK signalling.

    The oxidative pentose phosphate pathway (PPP) contributes to tumour growth, but the precise contribution of 6-phosphogluconate dehydrogenase (6PGD), the third enzyme in this pathway, to tumorigenesis remains unclear. We found that suppression of 6PGD decreased lipogenesis and RNA biosynthesis and elevated ROS levels in cancer cells, attenuating cell proliferation and tumour growth. 6PGD-mediated production of ribulose-5-phosphate (Ru-5-P) inhibits AMPK activation by disrupting the active LKB1 complex, thereby activating acetyl-CoA carboxylase 1 and lipogenesis. Ru-5-P and NADPH are thought to be precursors in RNA biosynthesis and lipogenesis, respectively; thus, our findings provide an additional link between the oxidative PPP and lipogenesis through Ru-5-P-dependent inhibition of LKB1-AMPK signalling. Moreover, we identified and developed 6PGD inhibitors, physcion and its derivative S3, that effectively inhibited 6PGD, cancer cell proliferation and tumour growth in nude mice xenografts without obvious toxicity, suggesting that 6PGD could be an anticancer target.

[An odour of disease and decay: the nose as a diagnostic instrument].

    Infectious diseases and cancer change a patient's metabolism and hence the metabolic compounds produced. The composition of volatile organic compounds (VOCs) in exhaled breath or urine or stool samples can therefore be characteristic of a particular disease. In recent years many studies have been conducted into the training of animals, including dogs, to recognise diseases by smell. Besides trained animals, electronic noses (e-noses) are also being developed. These devices can identify disease-specific odour profiles in VOCs. Although the results of research in the field of scent diagnosis are promising, the medical community remains largely sceptical. We discuss applications of scent detection as a diagnostic tool in modern medicine.

[Coverage of issues of professional malignant neoplasms of the respiratory tract in modern clinical guidelines of the Ministry of Health of Russia].

    Analysis and comparison of clinical guidelines (CG) on malignant neoplasms of the respiratory system, placed in the CG rubricator of the Ministry of Health of Russia, for the information they contain on the role of factors and working conditions of the working environment in the etiology of respiratory diseases, including the upper respiratory tract, and comparison this information with published and regularly updated data from the International Agency for Research on Cancer (IARC).. Clinical recommendations from the heading of the Ministry of Health of Russia "Cancer of the laryngopharynx", "Cancer of the nasopharynx", "Cancer of the oropharynx", "Cancer of the nasal cavity and paranasal sinuses", "Cancer of the larynx", "Cancer of the trachea", "Cancer of the lung", and IARC data according to the List of Classifications by cancer sites with sufficient or limited evidence in humans dated 11/29/2019.. The low level of registration of occupational cancer of the upper respiratory tract in Russia is due to an inadequate system of criteria for detecting this form of occupational pathology, a long latency period of diseases, and insufficient assessment of the history of information on the potential role of occupational carcinogens. The CGs developed to optimize the diagnosis and treatment of malignant neoplasms of the respiratory system do not contain up-to-date information on the role of professional potential carcinogens in the genesis of malignant neoplasms.. Recognition of the professional nature of malignant neoplasms of the respiratory system implies, firstly, strengthening the role of otorhinolaryngologists in early diagnosis and rehabilitation of patients, and secondly, it has a pronounced preventive focus in the form of tasks facing the employer to reduce the carcinogenic load and develop preventive measures aimed to reduce and prevent the development of malignant tumors in workers, including the respiratory system.. Признание профессионального характера злокачественных новообразований органов респираторной системы подразумевает, во-первых, усиление роли специалистов оториноларингологов в вопросах ранней диагностики и реабилитации пациентов, во-вторых, носит выраженную профилактическую направленность в виде встающих перед работодателем задач по снижению канцерогенной нагрузки и разработке превентивных мер, направленных на снижение и профилактику развития у работников злокачественных опухолей, в том числе органов дыхания.

Non-ossifying fibroma with a pathologic fracture in a 12-year-old girl with tricho-rhino-phalangeal syndrome: a case report.

    Tricho-rhino-phalangeal syndrome (TRPS) is a rare autosomal dominant genetic disorder characterized by distinctive craniofacial and skeletal abnormalities, while non-ossifying fibroma (NOF) is a common benign bone tumour in children and adolescents. To date, no case of TRPS coexisting with NOF has been reported. This report presents a 12-year-old girl who had the characteristic features of tricho-rhino-phalangeal syndrome and non-ossifying fibroma with a fibula fracture.. A 12-year-old girl was admitted to the Department of Endocrinology and Diabetes for evaluation of brachydactyly and a right fibula fracture. Clinical examination revealed sparse scalp hair, a characteristic bulbous pear-shaped nose, and brachydactyly with significant shortening of the fourth metatarsal. Neither intellectual disability nor multiple exostoses were observed. Radiography of both hands showed brachydactyly and cone-shaped epiphyses of the middle phalanges of the digits of both hands with deviation of the phalangeal axis. Genetic analysis of TRPS1 identified a heterozygous germline sequence variant (p.Ala932Thr) in exon 6 in the girl and her father. Approximately 1 month before being admitted to our department, the girl experienced a minor fall and suffered a fracture of the proximal fibula in the right lower limb. The pathological cytological diagnosis of the osteolytic lesion was NOF. Ten months following the surgery, the lesion on the proximal fibula of the girl disappeared.. In conclusion, the present study is the first to report a rare case of NOF with a pathologic fracture in the fibula of a girl with TRPS. The identification of a missense mutation, (p.Ala932Thr), in exon 6 of TRPS1 in this kindred further suggested that the patient had type I TRPS and indicated that mutations in this exon may be correlated with more pronounced features of the syndrome. Radiological techniques and genetic analysis played key roles in the definitive diagnosis.

Localized mucosal leishmaniasis caused by Leishmania infantum mimicking cancer in the rhinolaryngeal region.

    The clinical, microbiological, and histopathological findings of six patients with mucosal leishmaniasis are reported. Five of these patients were Spanish with no history of travel abroad, while the other was from Bolivia but had lived in Spain for more than 5 years. Two patients had no underlying disease, while the other four had several other medical conditions. Lesions were located in the nose in three patients and in the larynx in the other three. Symptoms included difficulty in swallowing, nasal obstruction, dysphonia, and polypoid lesions mimicking cancer. The diagnosis was based on the identification of parasites, or on PCR assay or culture. Five patients were treated with liposomal amphotericin B and the other with antimonial compounds.

[Application of an image navigation system in nasal endoscopic surgery in treatment of basicranial tumor].

    To investigate the therapeutic effects of the image navigation system in nasal endoscopic surgery in treatment of basicranial tumor.. Retrospective analysis was carried out in 17 patients with basicranial tumor who underwent nasal endoscopic resection via image navigation system. The accuracy of image navigation system, operation procedure time, curative effect and complications were analyzed and summarized to evaluate the effect.. The image navigation system provided an accurate anatomical localization 0.5-2.0 mm localization error (mean 1.2 mm). The operation procedure time was 1.5-3.0 h. Postoperative complications occurred in 3 of 17 patients including cerebrospinal fluid rhinorrhea and injury of sieve plate. Two patients recrudesce within half a year.. The image navigation system in combination with a nasal endoscope is helpful in the resection of basicranial tumor with minimal trauma, increasing the curative effect and avoiding complications.

Increasing prevalence of nasal and rectal colonization with methicillin-resistant Staphylococcus aureus in children with cancer.

    Infections with methicillin-resistant Staphylococcus aureus (MRSA), in community-settings, especially with strains carrying the Panton-Valentine Leukocidin (PVL) genes, have increased markedly in recent years. Colonization with S. aureus is a risk factor for infection. However, there are few studies that examine colonization and infection with PVL-positive strains of MRSA in cancer patients.. The epidemiology of colonization and infection with MRSA was studied in children with cancer during two time periods: 2000/2001 and 2006/2007. PVL genes were screened and spa typing performed on the isolates.. The prevalence of colonization with MRSA increased from 0.6% in 2000/2001 to 2.9% in 2006/2007 (P = 0.0003). MRSA colonization at admission was associated with infection (P < 0.0001; RR 38.32; 95% CI: 23.36-62.84). The prevalence of infection increased from 0.99% in 2000/2001 to 3.78% in 2006-2007 (P = 0.0002). Of the 32 colonized patients, 18 (56%) had infection. None of the 14 colonized but non-infected patients had dual colonization of nares and rectum, while 8 of the 18 infected patients had colonization of both of these sites (P = 0.004). Ten patients (31%) were colonized with PVL-positive strains. Patients colonized with PVL-positive strains were more likely to be colonized both in the nares and rectum (P = 0.005), and more likely to have infection (P = 0.001). Recurrent MRSA infections were seen in 22% of patients.. An increasing prevalence of colonization with MRSA was observed in children with cancer at our institution. Colonization with MRSA especially with PVL-positive strains was associated with infection.

Chemical genetics reveals a complex functional ground state of neural stem cells.

    The identification of self-renewing and multipotent neural stem cells (NSCs) in the mammalian brain holds promise for the treatment of neurological diseases and has yielded new insight into brain cancer. However, the complete repertoire of signaling pathways that governs the proliferation and self-renewal of NSCs, which we refer to as the 'ground state', remains largely uncharacterized. Although the candidate gene approach has uncovered vital pathways in NSC biology, so far only a few highly studied pathways have been investigated. Based on the intimate relationship between NSC self-renewal and neurosphere proliferation, we undertook a chemical genetic screen for inhibitors of neurosphere proliferation in order to probe the operational circuitry of the NSC. The screen recovered small molecules known to affect neurotransmission pathways previously thought to operate primarily in the mature central nervous system; these compounds also had potent inhibitory effects on cultures enriched for brain cancer stem cells. These results suggest that clinically approved neuromodulators may remodel the mature central nervous system and find application in the treatment of brain cancer.

External nasal dilator strips (ENDS) may improve breathlessness in cancer patients.

    ENDS (external nasal dilator strips) are adhesive bands containing a central elastic strip providing a spring action. The possibility of reducing the cumulated effort of continuous breathing or decreasing the perception of exertion would be of major benefit to oncological dyspneic patients, namely in presence of a cachexia-asthenia complex.. Nine patients complaining of dyspnea were included in this pilot study. The ENDS were applied at 0800 and at 1600. Perception of dyspnea and tolerability was assessed 12 h after the first application of the ENDS. No other treatment changes were performed during the evaluation time.. As a result of the application of ENDS, one patient perceived a good improvement of dyspnea, three moderate, two little, and three none. Tolerability was good for four patients and moderate for another four. Seven out of nine patients decided to carry on with the application of ENDS afterwards.. The application of ENDS may be useful in cancer patients with dyspnea. It could be particularly helpful to patients suffering from cachexia and profound fatigue. However, further research is needed to affirm that ENDS might have an important place in relieving dyspnea.

NTP toxicology and carcinogensis studies of dipropylene glycol (CAS No. 25265-71-8) in F344/N rats and B6C3F1 mice (drinking water studies).

    Dipropylene glycol is found in antifreeze, air fresheners, cosmetic products, solvents, and plastics. We studied the effects of dipropylene glycol on male and female rats and mice to identify potential or cancer-related hazards to humans.. We gave groups of 50 male and female mice drinking water containing dipropylene glycol at concentrations of 10,000, 20,000, or 40,000 parts per million (corresponding to 1%, 2%, or 4%) for two years. Male and female rats received concentrations of 2,500, 10,000, or 40,000 parts per million. Other groups received untreated water and were the control group. Tissues from more than 40 sites were examined for every animal.. The groups of animals receiving 40,000 ppm dipropylene glycol weighed less than the control animals. All the make rats receiving 40,000 ppm dipropylene glycol died before the end of the study, mainly because of kidney disease. All the other animal group survived as well as the controls. No increase in tumor rates were seen in any of the groups of rats or mice.. We conclude that dipropylene glycol did not cause cancer in male or female rats or mice. Exposure to dipropylene glycol did increase the rate and severity of kidney nephropathy and inflammation of the liver and salivary gland in male rats and some atrophy of the epithelial tissue of the nose in male and female rats.

[Staphylococcus nasal carriage and infection of central venous ports in oncology].

    To assess the risk of infection of either subcutaneously implanted central venous access devices or percutaneous central venous catheters inserted via a subcutaneous tunnel in cancer patients with a positive staphylococcal nasal carriage.. Prospective study.. The study included 266 patients undergoing cancer chemotherapy.. A nasal swab was taken prior to insertion of the venous access device and the patients were followed over 30 days for the occurrence of a staphylococcal infection (hemoculture and device or site of insertion).. A nasal staphylococcal nasal carriage was found in 227 patients. Out of the 15 developing a device infection, a staphylococcal nasal colonization was existing in nine patients. Bacteriological screening a sensitivity of 60% and a specificity of 13%.. Bacteriological screening at the time of device insertion of a central venous access device is of no value for the detection of patients at risk of staphylococcal infection of the device.

Epistaxis: study of aetiology, site and side of bleeding.

    The present study comprises 300 cases of epistaxis. The analysis of these cases revealed a higher incidence in young males. Unilateral bleeding was seen in almost 60% each of indoor and outdoor cases. Litte's area was the most common site responsible for epistaxis in 28.8% of the indoor and 26.2% of the outdoor patients. Hypertension was the most common systemic cause among indoor patients (62.2%) and sickle cell disorder among the outdoor patients (37.5%). Atrophic rhinitis with myiasis was the local cause of epistaxis in maximum (27%) of the indoor patients and traumatic epistaxis was the commonest cause (33%) among outdoor patients-fingernail trauma in 75.9% of them. Idiopathic epistaxis contributed for 16.5% indoor and 26.1% of outdoor cases. Intractable epistaxis was seen in one case following accidental facial trauma.

[Bacterial fora of patients treated with bleomycin].

    Microflora of the pharynx, nose, sputum and excrements was investigated. It was found that bleomycin therapy may induce or aggravate the shifts in microbiocenosis of the pharynx and sputum in the direction of pathogenization (increase in the number of Staph. aureus, hemolytic streptococci, enterobacteria). A tendency to elimination of bifidobacteria and appearance of Proteus in the excrements was observed. Complications associated with the treatment are rarely of infectious origin.

Management of cancer of the nasal cavity and paranasal sinuses.

    Cancer involving the antrum can be cured in a greater percentage of cases if the lesions are detected earlier, specifically diagnosed histologically, and radically managed at the time of the initial treatment. The procedure of choice is, first, surgical removal (with a scalpel) of as much of the tumor as is possible without risk of operative death, then limited but specific cancerocidal radiation as indicated by operative findings or regular postoperative observation. The period of convalescence will be less distressing and shorter with this procedure than with others; and, in those cases in which cure is not effected, palliation will be more satisfactory. The cured patients are cosmetically acceptable without functional disability except for unilateral loss of vision in cases in which exenteration of the orbit is necessary.
