Medication-induced esophagitis in children.
Clinical and endoscopic features of two pediatric cases of esophageal ulcers caused by capsules of oxytetracycline and doxycycline are described. Several cases of medication-induced esophageal injury in children have been reported until now, all of which were in association with tablets or capsules. Antibiotics are known to be responsible for medication-induced esophagitis in adults. In this study, 4 cases were caused by emepronium bromide and 3 cases, including the present patients, by antibiotics. All cases but one complained of chest pain and/or dysphagia. Although the interval between the onset of the symptoms and the diagnosis varied among cases, the clinical courses were relatively uneventful, without any long-term sequelae. Ulcerative and papillomatous digital dermatitis of the pastern region in dairy cattle: clinical and histopathological studies.
The study was carried out on a single herd of 500 Holstein breed dairy cattle. Only 50 dairy cows (10%) were found to be suffering from focal ulcerative and papillomatous digital dermatitis of the pastern region. The cows were subjected to clinical and histopathological examinations. Early ulcerative stage was seen in 30 cows and late papillomatous stage was observed in 20 cows. The topical application of oxytetracycline solution or benzathine penicillin powder appeared to be effective for the treatment of the ulcerative lesions. The histopathological findings showed signs of epidermopoiesis and papilloma formation. The dermal reaction revealed signs of fibroplasia and perivascular aggregation of inflammatory cells. Silver stained sections indicated the presence of longer filamentous spiral spirochetes, cocci and large-sized anaerobic bacilli invading the stratum spinosum. In conclusion it can be said that bovine digital dermatitis was observed in ulcerative and papillomatous form at the pastern region bordering the dewclaws. Spirochetes are frequently associated with and may be responsible for pathological changes in the lesion. Tetracycline induced esophageal ulcers. a clinical and experimental study.
Medication with oral drugs has not been considered as a cause of esophageal lesions in the general literature of esophageal disease. This study demonstrates 40 patients with complaints of sudden onset of intense retrosternal pains and odynophagia during treatment with oral tetracyclines. All patients had distinct circumferential ulcers in the esophagus. Medical history, barium swallows, esophagoscopy, biopsies and esophageal manometry revealed no other apparent etiology but a local corrosive effect of the tetracyclines. Experimental tests on the esophagus of the cat verified a severe local corrosive effect of the tetracyclines. Another tetracycline, lymecycline, not reported previously to induce esophageal lesions in man, was significantly less ulcerogenic than doxycycline and oxytetracycline. Drug induced esophageal ulcerations are likely to be more numerous than previously suspected. The experimental model used appears to be sound for investigating ulcerogenic potentials of orally administered drugs. [Tetracycline ulcer of the esophagus].
On the basis of 3 observations the author deals with acute ulcers in the oesophagus after oral application of oxytetracycline capsules. Sudden retrosternal pains, beginning in most cases during night, are in the foreground of the clinical picture. Discontinuation of the medicament and prescription of an antacid lead to the regression of the ulcer. For the development of such ulcers probably several factors play a role (pH-changes disturbances of motility and cytolysis). Oxytetracycline capsules should be taken always with abundant fluid and not immediately before bedtime.
