The prophylaxis of septic complications in colo-rectal surgery. A controlled trial of metronidazole and oxytetracycline.

    The prophylactic effect of systemic metronidazole on infectious complications following elective colo-rectal surgery was compared with the prophylactic effect of oxytetracycline. Allocation to receive metronidazole or oxytetracycline was random. A total of 137 patients were accepted for the trial, 67 receiving metronidazole and 70 oxytetracycline. The numbers of abdominal wound infections, intra-abdominal infections, fever and deaths were few in both groups, and the differences statistically insignificant. Clinically significant side effects of the antimicrobial agents were not observed. The influence of the antimicrobial agents on colonic flora was also studied. No significant difference in the effect on colonic anaerobic flora was demonstrated, whereas the aerobic flora was significantly suppressed by oxytetracycline. It is concluded that systemic metronidazole, although acting solely on the anaerobic flora, is as effective as oxytetracycline in preventing postoperative infectious complications following elective colo-rectal surgery. The prophylactic use of metronidazole is advocated.

Septicaemia, encephalitis and abortions in a housed flock of sheep caused by Listeria monocytogenes type 1/2.

    One hundred and ninety-six housed pregnant ewes were fed poor quality silage for two days. Ewes are reluctant to eat the silage and within 48 hours they became dull and developed diarrhoea and lameness. Despite treatment with antibiotics and calcium borogluconate 19 ewes died, more than 60 developed vaginal discharges and at lambing 94 ewes were barren. Six developed nervous signs and two of these died, one with lesions typical of listeric encephalitis. Post mortem lesions and the isolation of Listeria monocytogenes type 1/2 from lung, liver, spleen and kidney are described. L monocytogenes was also isolated from blood samples from live ewes. The estimated gross financial loss to the farmer was 5130 pounds or, for the flock, 26 pounds per ewe.

[R and S-streptococcal infections in swine].

    Some 115 R- and 143 S-streptococcal strains were isolated from 258 pigs that had died or been killed for disease, between 1969 and 1973. They were cultured and biochemically tested for sensitivity to chemotherapeutics. All R- and S-strains were of normal sensitivity to penicillin and, with few exceptions, to chloramphenicol and oxytetracycline as well. Most of the strains caused primary Sc.-infection, such as septicaemia, leptomeningitis, endocarditis, and arthritis. R-Sc.-infections were contracted mainly by store pigs and animals in advenced age, while S-Sc.-infections affected mainly piglets.

Case report: septicaemia due to Yersinia enterocolitica in a non-compromised host.

    A case of septicaemia due to Yersinia enterocolitica serotype 3, biotype 4, in a non-compromised host is described. The patient showed severe signs of gram-negative septicaemia. Treatment with sulphamethoxazole-trimethoprim (Bactrim) was unsuccessful in spite of full sensitivity, bactericidal and synergistic effects in vitro. Tetracycline gave full restitution.
