Facial edema as an adverse drug reaction to olanzapine in a patient with cancer receiving palliative care.
Atypical neuroleptics such as olanzapine are indicated for the treatment of various psychiatric disorders and have been used in the palliative care setting also for several clinical indications. Peripheral and facial edema are a rare side effect of the treatment with olanzapine. We report a case of an advanced cancer patient cared receiving palliative care who developed severe facial edema after initiating a low dose of olanzapine in monotherapy.. A patient with advanced cancer who presented with severe facial edema after initiating olanzapine for the treatment of her opioid use disorder.. After excluding other differential diagnosis for facial edema, olanzapine was discontinued with complete resolution of the edema.. To the best of our knowledge, this is the first case reporting facial edema due to olanzapine treatment in a patient with advanced cancer. Our report will help clinicians recognize the possible role of olanzapine in cases of rapid onset of facial edema, allowing its rapid resolution. Lessons of the month 2: Olanzapine-induced hypothermia and hand oedema.
We describe a case of olanzapine-induced hypothermia and hand oedema in an older adult with behavioural and psychological symptoms of dementia (BPSD).. An 82-year-old woman with hypothyroidism and dementia was reviewed by the geriatric team at a nursing home in view of lethargy and an unrecordable oral temperature. She was noted to have a bilateral hand oedema and right basal crackles. Investigations revealed high white cell count and inflammatory markers. She was treated as per hypothermia and communityacquired pneumonia protocols. The patient did not have the expected response to treatment. Olanzapine was tailed down and stopped with good effect as it was suspected to be a contributory cause to both the hypothermia and oedema.. Potentially inappropriate polypharmacy can be specifically targeted with effective deprescribing. Treatment review should be encouraged on a regular basis, especially in frail older adults with polypharmacy. Olanzapine as a cause of peripheric edema in an elderly man.
Edema can be observed as side-effect of many medications, of which calcium channel blockers are the best known. Elderly people use many more medications than their younger counterparts and are usually more prone to developing medication-induced side-effects. Atypical antipsychotics have occasionally been shown to induce peripheric edema. Age is put forward as a risk factor for olanzapine-induced edema. We present here the case of an elderly man who developed upper- and lower-limb edema during use of olanzapine to emphasize its relatively frequent association with edema in the elderly. In comparison to metabolic syndrome, edema seems to be a rare occurrence in antipsychotic treatment.. In this paper, we report on two subjects with psychotic disorders who were found to have peripheral edema during olanzapine therapy.. The theoretical background and the relevance of this observation are examined. Furthermore, edema as a side effect of antipsychotic treatment is reviewed. In particular, the importance of olanzapine, a commonly used neuroleptic drug, as a possible causative substance is evaluated. Peripheral oedema in patients taking olanzapine.
Premarketing trials of olanzapine reported an incidence of peripheral oedema of 3%. Outpatients, who were actively receiving treatment with olanzapine, were evaluated for peripheral oedema. Twenty-eight (57%) out of 49 subjects exhibited oedema, including five (10.2%) to a severe degree. There were no significant differences regarding gender, time/dose on olanzapine, concomitant diagnoses or psychotropic medication. There was a tendency (P < 0.10) for older age and greater frequency of thyroid abnormalities in those individuals with oedema. In the oedema group, there was a positive correlation (P < 0.05) between age and severity. Our finding, far from demonstrating that olanzapine causes oedema at this frequency, suggests that there maybe patients who are vulnerable to developing this side-effect.
