Effectiveness of nystatin in polysymptomatic patients. A randomized, double-blind trial with nystatin versus placebo in general practice.

    Antifungal therapy has been claimed to be effective in polysymptomatic patients with diffuse symptoms from multiple body systems and even well defined diseases, traditionally not related to fungi. Hypersensitivity to fungus proteins and mycotoxins has been proposed as the cause.. We conducted a 4-week randomized, double-blind, placebo-controlled study in 116 individuals selected by a 7-item questionnaire to determine whether the antifungal agent nystatin given orally was superior to placebo. At the onset of the study, the patients were free to select either their regular diet or a sugar- and yeast-free diet, which resulted in four different subgroups: nystatin + diet (ND); placebo + diet (PD); nystatin (N); and placebo (P).. Nystatin was significantly better than placebo in reduction of the overall symptom score (P < 0.003). In six of the 45 individually recorded symptoms, the improvement was significant (P < 0.01). All three active treatment groups reduced their overall symptom scores significantly (P < 0.0001), while the placebo regimen had no effect (P = 0.83). The benefit of diet was significant within both the nystatin (ND > N) and the placebo groups (PD > P).. Nystatin is superior to placebo in reducing localized and systemic symptoms in individuals with presumed fungus hypersensitivity as selected by a 7-item questionnaire. This superiority is probably enhanced even further by a sugar- and yeast-free diet.

Association of lymphocytic colitis with linear IgA dermatosis.

    The case of a 66-year-old female patient is presented, who suffered from chronic watery diarrhea. In addition, she developed linear IgA dermatosis after oral treatment of a presumed yeast infection with nystatin. To evaluate the reason for her diarrhea, colonoscopy was performed. The macroscopic aspect of the colon mucosa was described as normal with no specific alterations for chronic inflammatory bowel disease or for bacterial infections. In contrast, the histologic examination revealed the typical characteristics of lymphocytic colitis. This disease is thought to be caused by immunological reactions against as yet unknown luminal antigens. After treatment with steroids and dapsone the diarrhea as well as the skin disease disappeared. To our knowledge, the present report describes for the first time the association of linear IgA dermatosis with lymphocytic colitis after oral treatment with nystatin. A possible causative link between these two disease entities is discussed.

Oral manifestations of Sweet's syndrome.

    A case of a 38-year-old male with the diagnosis of Sweet's Syndrome is reported. The most significant feature of the patient was manifested by serious lesions in the oral mucosa which persisted throughout the course of the disease. They only improved after the administration of prednisone. Special emphasis is made on the characteristics of the oral manifestations of Sweet's syndrome since they have not been described in detail in the literature.

[Frequent dermatologic diseases in the newborn and children].

    Sebaceous gland activity in the newborn and children is extensive at first in the epidermis, and later decreases owing to defective operation of the sweat glands, circulation changes, growth of the hair, and a predisposition to infection, for which significant treatment is required. In addition to the genetic "typus neurodermiticus", pharmaceutical, alimentary and respiratory allergens and psychological factors may play a part in the appearance of atopical dermatitis. Onset is usually in the 2nd to 6th month and the side of the face and the scalp are affected. The clinical picture varies considerably with age and includes sanious foci covered with scabs on the head and eczema flexurarum. Differences in family history, age and the location of sites will distinguish this form from seborrhoeic dermatitis. Gluteal dermitis is secondary to congestion caused by dampness, rugging and soaking, and the action of stools and urine (bacterial amniogenesis). The main complications of neonatal and infantile impetigo are: glomerulonephritis, exfoliative dermatitis, pulmonary abscess, pulmonitis and osteomyelitis; phlyctenular, serpiginous impetigo with large and small bullae is particularly important. Treatment of neonatal and infantile dermatosis must take account of the fact that increased reabsorption and congestion are contraindications for the local application of corticoids and preparations containing salicylic and boric acid.
