[Meconial disease in a newborn].
A case of meconial disease not associated with mucoviscidosis registered in 0.08% of perinatal autopsies is described in a premature girl who died 46 hours after her birth. Productive exudative meconial peritonitis that had developed due to intestinal wall perforation caused by meconium plug pressure was detected on the section. Microscopically, there were abundant squamous epithelial scales in the fibrinous exudate and commissures, whereas in the submucosal layer of the intestinal wall there were productive aseptic meconial granulomas whose components are squamous epithelial scales in addition to cellular elements. Searches for granulomas are made concurrently with examination of great quantities of histologic sections, but their detection is valuable for differential diagnosis of newborn ulcerous enterocolitis. Symptomatic maternal intraperitoneal meconium granulomata: report of two cases.
We present two cases of histologically proven maternal intraperitoneal granulomata following cesarean section delivery. One patient presented during the immediate postpartum period with clinical symptoms consistent with subphrenic abscess. The other patient required operative intervention for a suspected ruptured tubo-ovarian abscess. In both cases, microscopic examination of the tissue obtained at the time of reoperation showed granuloma reaction, lanugo hair, and fetal squamous cells consistent with meconium deposition. The authors conclude that meconium granuloma should enter the differential diagnosis in cesarean section patients with unexplained postoperative pain. Meconium granulomas of the tunica vaginalis.
Meconium peritonitis results from perforation of the gastrointestinal tract during fetal life. Involvement of the tunica vaginalis may be the sole presenting clinical manifestation of the disease in the unusual event of the gut perforation resolving spontaneously. In such instances radiologically detectable calcification in the abdomen and scrotum is an essential diagnostic point. A case is described in which a baby had hydroceles and bilateral intrascrotal nodules but in which calcification was radiologically undetectable, presumably owing to its having undergon resolution. The typical histology of the nodules provided the diagnosis in this otherwise clinically undiagnostic case.
