Charles Bonnet syndrome versus Occipital Epilepsy, a diagnostic challenge.
Charles Bonnet Syndrome (CBS) is a disorder with visual deficit and complex recurrent visual hallucination in conscious patients, described for the first time by Charles Bonnet. It has been found in association with variable pathologic conditions of the eyes, central visual pathways and occipital lobe. Occipital lobe lesion is an important cause of visual field deficit associated with elementary simple hallucinations, whereas complex hallucinations are related to occipitotemporal and occipitoparietal visual association neocortex damage. Similarity of symptoms between transient epileptic amnesia and Lewy body disease.
Epilepsy with the main symptom of amnesia is known as transient epileptic amnesia (TEA). Dementia with Lewy bodies (DLB) is the second most common form of neurodegenerative dementia. The concept that Lewy body disease includes Parkinson's disease with dementia and dementia with Lewy bodies was proposed in the 2005 revision of the Clinical Diagnostic Criteria. Here, we describe a woman with cognitive impairment, olfactory dysfunction, and reduced Topics: 3-Iodobenzylguanidine; Aged; Amnesia; Anticonvulsants; Cognition Disorders; Electroencephalography; Female; Hallucinations; Humans; Levetiracetam; Lewy Body Disease; Myocardial Perfusion Imaging; Parkinsonian Disorders; Piracetam; Seizures; Tomography, Emission-Computed, Single-Photon | 2017 |
A young woman with seizures and psychosis.
We present a case of a 24-year-old woman with abnormal behaviour of recent onset. She had been diagnosed previously with epilepsy and had been started on antiepileptic medication. Clinical examination confirmed features of psychosis including paranoid delusions and auditory hallucination. Neurological examination showed nystagmus and dysmetria. Further evaluation revealed the underlying cause for her symptoms. She responded promptly to appropriate therapy with complete resolution of psychosis. Charles Bonnet syndrome disappearing with carbamazepine and valproic acid but not with levetiracetam.
Although Charles Bonnet syndrome (CBS) appears to be more frequent than historically reported, there are very few reports about possible pharmacological treatments. We report the case of a CBS disappearing completely with successive monotherapies of carbamazepine and valproic acid but worsening with levetiracetam.
