Persistent ascites resolving with gonadotropin-releasing-hormone-agonist 18 months after hospitalization for severe ovarian hyperstimulation syndrome.
Ovarian hyperstimulation syndrome (OHSS) is a life- threatening complication of controlled ovarian stimulation. One of the main symptoms of OHSS is ascites. Treatment is symptomatic with resolution of the symptoms over days to weeks. We report a case of severe OHSS with persistent ascites 18 months after the diagnosis. Persistent ascites secondary to OHSS was diagnosed and single dose leuprolide acetate depot 11.25 mg was administered. At follow-up, no ascites was observed. Massive ascites and hydrothorax after leuprolide acetate administration in a down-regulated woman undergoing assisted reproduction.
To present an atypical case of massive ascites and hydrothorax after leuprolide acetate administration in a down-regulated woman undergoing assisted reproduction.. Case report.. Centre for Reproductive Medicine, Department of Obstetrics, Gynaecology, and Neonatology, University of Parma, Parma, Italy.. A 41-year-old, nulliparous, white woman who developed massive ascites and hydrothorax after administration of 0.50 mg/day of subcutaneous leuprolide acetate, beginning at the midluteal phase.. Down-regulation with the gonadotropin-releasing hormone analogue was discontinued, and therapy was started with furosemide 50 mg/day for 10 days.. Successful medical reduction of ascites and hydrothorax.. Resolution of symptoms.. A comprehensive MEDLINE search revealed this to be the first reported case of massive ascites and hydrothorax after leuprolide acetate administration (0.5 mg daily) in a down-regulated woman undergoing assisted reproduction. This case can be explained by an increase in capillary permeability, which resulted in a rapid fluid shift from the intravascular space into the third space. We believe that ascites in our patient resulted from an increase in estradiol in the ovaries, due to a direct action of the gonadotropin-releasing hormone analogue on the corresponding ovarian receptors in the first few days after the start of therapy. Pleural fluid accumulation due to intra-abdominal endometriosis: a case report and review of the literature.
A case is presented of massive ascites and right sided pleural effusion caused by endometriosis. The final diagnosis was not made for a considerable time. Massive ascites and a right sided pleural effusion caused by endometriosis is rare, with fewer than 10 reports in the literature worldwide. Physicians should be aware of this potentially tentially treatable cause, having excluded other possibilities such as malignancy and tuberculosis. Massive ascites after leuprolide acetate administration for the treatment of leiomyomata uteri.
The administration of GnRH-a for decreasing the size of uterine leiomyomata is tolerated by most of our patients with a minimum of side effects and difficulty. An unusual case of pseudo-Meigs' syndrome developed after a single dose of depot LA that resulted in an emergent operation in this young woman. The treatment of large leiomyomata uteri with GnRH-a warrants careful clinical surveillance. Ascites and pleural effusions associated with endometriosis.
We report a case of ascites and bilateral pleural effusions associated with an endometrioma in a 26-year-old woman of Chinese descent. She had a right salpingo-oophorectomy and partial omentectomy performed, and she received leuprolide acetate depot injections after the operation. We believe this is the first description of use of a GnRH agonist to treat this rare condition.
