[Curbing of arrhythmias under outpatient polyclinic conditions].
The material has been collected in the course of two years and covers 48 cases with paroxysms of supraventricular paroxysmal tachycardia (28), auricular fibrillation (4) ventricular tachycardia (2), group and polytopic ventricular extrasystoles (14), organic cardiac diseases (30) and 18--inorganic heart diseases. Paroxysms were interrupted by isolamid, tachmalin lidocain via slow intravenous infusion with constant ECG, auscultation and hemodynamic control, monitor follow up. The paroxysms successfully coped were 43. It proved possible to interrupt the rhythm disorders under the conditions of non-integrated polyclinic provided the functional consulting room is well equipped. The problem of early and effective treatment of rhythm disorders even under polyclinical conditions is solved, thus avoiding the unnecessary hospitalization of some of those patients and delay in the effective treatment of the other part of the patients. The early diagnosis and proper therapeutic behaviour of the physician towards the patient from the very first moment of contact to the transportation and hospitalization of the patient is of vital importance. The interruption of rhythm disorders must very cautiously be performed--the ventricular tachycardia and polytopic extrasystoles in particular, that considered only an introductory treatment. The paroxysms were coped with isolanid , tachmalin and lidocain , sow intravenous infusion under constant ECG, auscultatory and hemodynamic control and monitors. The number of the successfully coped paroxysms is 43. It proved to be possible to interrupt the rhythm disturbances under the conditions of non-integrated polyclinic in case of properly equipped functional consulting room. In this way the problem of early and effective treatment of rhythm disturbances under polyclinical conditions is solved, hence--avoiding both the unnecessary hospitalization of part of those patients and the delay in the initiation of effective treatment of the other part of them. The early diagnosis and adequate therapeutic behaviour of the physician towards the patient at their first contact till the moment of their transportation and hospitalization proved to be of particular significance. The out-patient department interruption of rhythm disorders should very cautiously be performed, particularly in case of ventricular tachycardia and polytopic extrasystoles, that being only their initial treatment. Cortical control of neurally-mediated arrhythmogenic properties of desacetyl lanatoside C: the role of the posterior hypothalamus.
Functional ablation of the cerebral cortex by cortical spreading depression (CSD) significantly increased the dose of desacetyl lanatoside C required to induce A-V block and ventricular fibrillation. To examine the role of the posterior hypothalamus in the increased resistance of decorticated rats to arrhythmia induced by toxic doses of desacetyl lanatoside C, four groups of animals were injected with this drug: group 1 rats had a craniotomy; group 2 rats had a craniotomy and functional decortication; group 3 rats had a craniotomy and a hypothalamic lesion; and group 4 rats had a craniotomy, hypothalamic lesion and functional decortication. The dose of drug required to induce A-V block and ventricular fibrillation was significantly less in group 1, than in groups 2,3 and 4, and there was no statistically significant difference between these last three groups. These results are consistent with the hypothesis that the increased resistance to arrhythmia induced by desacetyl lanatoside C in decorticated rats is mediated by the posterior hypothalamus. Interaction of serotonin and deslanoside on cardiac rhythm in the cat.
The present study was performed to determine whether increases in the tissue content of serotonin creatinine SO4 in the periphery would influence the arrhythmogenic effect of deslanoside. This was accomplished by infusing serotonin into anesthetized cats exposed to a subarrhythmic dose of deslanoside, determining doses of deslanoside required to produce ventricular tachycardia and ventricular fibrillation, and determining ventricular pacemaker rate (obtained during vagal-induced sinus node suppression). It was found that animals receiving serotonin creatinine SO4 plus deslanoside exhibited a greater increase in ventricular rate during sinus node suppression than with 5-HT infusion alone. No corresponding increase in ventricular pacemaker rate during sinus node suppression was observed with creatinine SO4 plus deslanoside. In addition, the dose of deslanoside to produce ventricular fibrillation in these animals was significantly correlated with the increase in ventricular pacemaker rate seen during th 5-HT infusion in the presence of deslanoside. Studies were also performed to determine whether the arrhythmogenic interaction of serotonin with deslanoside was associated with alterations in either cardiac tissue, blood or plasma levels of serotonin and 5-hydroxyindoleacetic acid. The data revealed a significant correlation between serotonin content in the left ventricle and the dose of deslanoside required to produce ventricular fibrillation. These results suggest that exogenous serotonin interacts with deslanoside to enhance the arrhythmogenic action of deslanoside. [Clinical and pharmacokinetic studies in patients with digitalis intoxication (author's transl)].
6 patients with severe digitalis intoxication were studied while hospitalised in a coronary care unit. 2 and 4 patients had ingested high doses of lanatosid C and digoxin, respectively. In three cases ventricular arrhythmias, in one of these and two further cases SA blocking and an additional A-V-block in one case were observed. Maximum blood levels of digoxin between 3.4 and 20 ng/ml were determined several hours after the ingestion. The maximal blood levels were achieved in one patient only 52 h after ingesting lanatosid C and in one patient taking digoxin after 12.5 h. Potassium concentrations in plasma were elevated in 4 patients. Antiarrhythmic and electrolyte therapy is discussed and the usefulness of a stomach lavage for diminishing the quantity of absorbed lanatosid C is shown in one patient who had a maximal blood level of 3.4 ng/ml after taking 23.7 mg of lanatosid C. Cumulative urinary excretion of this patient was 0.68 mg within 5.5 days. This result confirms the minimal enteral absorption under the therapy chosen.
