[Possible aggravation of a Henoch Schoenlein purpura by isotretinoin].

    We report on a 16 year old patient with a hemorrhagic bullous form of a Henoch-Schönlein purpura. The cause for the event could not be determined, but the patient recovered without complications. Since he had begun a therapy with isotretinoin due to an acne inversa 3 weeks before, a possible association between the event and the treatment could not be excluded, and the case was reported to Swissmedic. In the comment, we discuss the pharmacology of isotretinoin, drug-induced vasculitis, and its pathomechanisms. As a general reminder, the criteria for the causality assessment of adverse drug reactions as well as the reporting system of adverse drug reactions in Switzerland are discussed.

Keratoacanthoma centrifugum marginatum: a diagnostic and therapeutic challenge.

    A keratoacanthoma centrifugum marginatum (KACM) may pose a diagnostic and therapeutic challenge. Clinically and histologically, it may resemble mycobacterial or deep fungal infection or halogenoderma. Therapy can be challenging because the lesion can expand to a great size. We report on a patient with multiple lesions of KACM. The diagnostic difficulty and the therapeutic failure of imiquimod, intralesional methotrexate (MTX), and isotretinoin, as well as the therapeutic success of 5-fluorouracil (5-FU) cream, are discussed.

Granuloma annulare following waxing induced pseudofolliculitis-resolution with isotretinoin.

    We report a case of localized granuloma annulare, successfully treated with isotretinoin, which occurred as a consequence of waxing-induced pseudofolliculitis. To our knowledge this is the first reported clearance of localized granuloma annulare with isotretinoin and the first reported case of granuloma annulare as a sequel to pseudofolliculitis.
