Majocchi's granuloma in a 3-year-old boy.

    A 3-year-old boy presented with a 15 x 15-cm papulonodular lesion on his left shin of 8 weeks' duration. The lesion was considered a Majocchi's granuloma caused by Trichophyton tonsurans. He was treated with griseofulvin and topical clotrimazole and hydrocortisone cream and responded well after 8 weeks of treatment.

Tinea of the face caused by Trichophyton rubrum with histologic changes of granuloma faciale.

    We report a case of tinea of the face caused by Trichophyton rubrum in which the histologic changes were consistent with granuloma faciale. We recommend that local infection with dermatophytes be considered in all cases of histologic granuloma faciale, especially when the clinical presentation is atypical for this dermatosis.

Unusual aspects of granulomatous dermatophytosis.

    Described are three male patients showing bizarre manifestations of granulomatous dermatophytosis, all in the hand region. One had a small infiltrated nodular lesion near the thumb which was mistaken for a common insect bite. The second had a violaceous-yellowish plaque, also on the thumb, composed of 2-3 small subcutaneous nodules. The third case had five reddish intracutaneous nodules on the dorsum of the hand. Puncture of these nodules revealed the presence of fungal elements and culture yielded T. rubrum in all cases. Each patient was also found to have a fingernail affected by the same agent. Histologic sections in each case showed granulation tissue in the dermis and hypodermis, mainly in the vicinity of hair fragments with spores. These lesions appeared to be variants of Majocchi's granuloma trichophyticum.

    Tinea capitis with granuloma formation in the scalp of an adult that was caused by Tr. tonsurans infection is presented. The lack of symptomatic findings emphasizes the occasional difficulty in making a clinical diagnosis of this condition. Moreover, the possibility of beauty parlors being the source of scalp infection in adults, as well as children, must be considered.
