The efficacy of oral beraprost sodium, a prostaglandin I2 analogue, for treating intermittent claudication in patients with arteriosclerosis obliterans.

    This study aimed to evaluate the effect of oral beraprost sodium, a prostaglandin I2 analogue, on symptoms of intermittent claudication in patients with arteriosclerosis obliterans. The research design consisted of a before and after treatment study without comparison groups. The subjects comprised arteriosclerosis obliterans patients who experienced intermittent claudication. Furthermore, this study aimed to assess the mechanism of action of beraprost sodium via blood sampling and measurements of flow-mediated vasodilatation before and after treatment.. The study was performed prospectively in 7 patients with arteriosclerosis obliterans. Beraprost sodium (40 microg) was orally administered to 7 patients at study entry, followed by administration of 120 microg/day for 12 weeks. Blood sampling and measurements of flow-mediated vasodilatation were performed before and after treatment at study entry, 4 weeks, and 12 weeks after treatment. Treadmill exercise tests were performed three times at study entry, 4 weeks, and 12 weeks after treatment. The ankle-brachial index (ABI) was measured at rest and after exercise.. Pain-free walking distances increased by 138% at 12 weeks after treatment. Maximum walking distances increased by 133%. The ABI was significantly increased at 4 weeks and 12 weeks after treatment at rest. Endothelin-1 levels tended to be decreased at 1 h after administration of 40 microg beraprost sodium. N(G),N(G)-dimethyl-L-arginine, nitrate ions, and flow-mediated vasodilatation.. Beraprost sodium tended to decrease endothelin-1 levels and improved symptoms of intermittent claudication in patients with arteriosclerosis obliterans.

[Observation on effect of integrated Chinese and Western medicine in treating patients with peripheral atherosclerosis obliterans].

    To observe the therapeutic effect of integrated traditional Chinese and western medicine (TCM-WM) and its influence on plasma endothelin-1 (ET-1), nitric oxide (NO) and apoproteins.. Ninety patients of peripheral atherosclerosis obliterans (ASO) were divided into three groups randomly and treated with Salvia injection (SI), prostaglandin E1(PGE1) and SI + PGE1 respectively. The clinical manifestation of patients was observed dynamically, plasma level of ET-1, NO and apoprotein were tested before and after treatment by radioimmunoassay, Griss method, immuno-transmission method and enzyme method.. The cure rate of the SI, PGE1 and TCM-WM group was 26.67%, 33.33% and 46.67% respectively, the effective rate of them was 26.67%, 40.00% and 43.33% respectively. The therapeutic effect of the TCM-WM group was better than that of the SI and PGE1 group, P < 0.01. The plasma level of ET-1, NO and apoprotein was positively correlated with the stage altering of ASO, it was normalized gradually when ASO was alleviating effectively.. The TCM-WM therapy has good clinical effect in treating ASO. To observe the change of ET-1, NO and apoprotein level before and after treatment dynamically is helpful to judge the severity, progress of disease and effect of treatment, and has high clinical significance.

Clinical Significance of Endothelin-1 And C Reaction Protein in Restenosis After the Intervention of Lower Extremity Arteriosclerosis Obliterans.

    This study aims to investigate the role of endothelin-1 (ET-1) and C-reactive protein (CRP) in restenosis after intervention of lower extremity arteriosclerosis obliterans.. The present prospective observational study included a total of 251 patients with arteriosclerosis obliterans in the lower extremity. All patients were treated with balloon dilatation, stent-assisted angioplasty or balloon dilatation, and stent-assisted angioplasty. Furthermore, these patients received a CTA examination at one and three months after surgery. The serum ET-1 and CRP levels were determined using a commercial enzyme-linked immunosorbent assay (ELISA).. In non-restenosis patients, both the CRP and ET-1 levels were significantly upregulated after surgery, reached a peak level at one week, and decreased at one month after surgery. However, for restenosis patients, the serum ET-1 and CRP levels did not decrease to the baseline at one and three months after surgery, but were remarkably higher than the levels for non-restenosis patients. Serum ET-1 levels were positively correlated with serum CRP levels at both one and three months after surgery. Both ET-1 and CRP levels after one week and one month, and CRP at three days, one week, one month and three months after surgery were risk factors for restenosis after intervention surgery of arteriosclerosis obliterans.. Both serum ET-1 and CRP levels were elevated after one and three months of intervention for lower extremity arteriosclerosis obliterans in patients with restenosis. These might be the risk factors for restenosis of lower extremity arteriosclerosis obliterans patients.

[Taohong Siwu Decoction regulated functions of endothelial cells and treated arteriosclerosis obliterans: an experimental study].

    To discuss the effect of Taohong Siwu Decoction (TSD) in regulating functions of endothelial cells and treating arteriosclerosis obliterans (ASO).. The ASO model was prepared by using high-fat diet plus intimal injury. They were randomly divided into the model group (n = 10), the normal control group (n = 9), the low dose TSD group (group A, n = 12), the middle dose TSD group (group B, n = 10), and the high dose TSD group (group C, n = 9). Eight weeks after modeling, the limb blood perfusion was observed using laser Doppler flowmetry. The arterial morphology was observed using light microscope and transmission electron microscope. The number of circulating endothelial cells (CECs) was determined using Percoll density gradient centrifugation method. Serum levels of TNF-alpha, IL-1, ET-1, and NO were detected using double antibody sandwich assay of enzyme linked immunosorbent assay (ELISA).. The ASO rat model was successfully established. Blood lipids levels significantly increased, the blood perfusion of left hind limbs significantly decreased, the number of CECs in the peripheral blood significantly increased, the arterial lumen was irregularly narrowed, the ultra-structure of vessel walls was damaged, serum levels of TNF-alpha, IL-1, and ET-1 significantly increased, and the serum level of NO significantly decreased in the model group, showing statistical difference when compared with the normal control group (P < 0.01). Compared with the model group, significant improvement in the aforesaid indices was shown in group B and C (P < 0.05, P < 0.01).. The injury and abnormal functions of endothelial cells is an important pathological process of ASO. As an effective recipe for treating ASO, TSD could protect vascular endothelial cells and improve the secretion function of vascular endothelial cells.
