Thromboembolic complications after arthroscopy of the knee.

    Arthroscopy of the knee has always been associated with a low risk of complications, including thromboembolism. Therefore, few studies have been concerned with the matter. We present two case reports and a review of the available literature concerning thromboembolic complications after arthroscopy.

[Thrombophlebitis--basic principles of treatment].

    Therapeutic strategies in deep vein thrombosis are yielding possible prevention of life-threatening pulmonary thromboembolism and extension of thrombosis. In order to prevent long term sequelae, i.e., the postthrombotic syndrome, recanalization is necessary before venous valves have been destroyed. Therapeutical guidelines comprise immobilization (except isolated calf vein thrombosis), compression therapy and anticoagulation. Thrombolysis may be performed in patients in whom extension and localization of thrombosis may result in a postthrombotic syndrome, if thrombosis is not older than 1-2 weeks, if there are no concomitant diseases which make it uncertain for the patient to experience postthrombotic syndrome, and in the absence of contraindications.

The duration of oral anticoagulant therapy after a second episode of venous thromboembolism. The Duration of Anticoagulation Trial Study Group.

    A consensus has not been reached about the optimal duration of oral anticoagulant therapy after a second episode of venous thromboembolism.. In a multicenter trial, we compared six months of oral anticoagulant therapy with anticoagulant therapy continued indefinitely in patients who had had a second episode of venous thromboembolism. Of 227 patients enrolled, 111 were randomly assigned to six months of anticoagulation and 116 were assigned to receive anticoagulant therapy indefinitely; for both groups, the target international normalized ratio was 2.0 to 2.85. The initial episodes of deep-vein thrombosis (n = 193) and pulmonary embolism (n = 34), as well as recurrent episodes, were all objectively confirmed.. After four years of follow-up, there were 26 recurrences of venous thromboembolism that fulfilled the diagnostic criteria, 23 in the group assigned to six months of therapy (20.7 percent) and 3 in the group assigned to continuing therapy (2.6 percent). The relative risk of recurrence in the group assigned to six months of therapy, as compared with the group assigned to therapy of indefinite duration, was 8.0 (95 percent confidence interval, 2.5 to 25.9). There were 13 major hemorrhages, 3 in the six-month group, (2.7 percent) and 10 in the infinite-treatment group (8.6 percent). The relative risk of major hemorrhage in the six-month group, as compared with the infinite-treatment group was 0.3 (95 percent confidence interval, 0.1 to 1.1). There was no difference in mortality between the two groups.. Prophylactic oral anticoagulation that was continued for an indefinite period after a second episode of venous thromboembolism was associated with a much lower rate of recurrence during four years of follow-up than treatment for six months. However, there was a trend toward a higher risk of major hemorrhage when anticoagulation was continued indefinitely.

[Role of postoperative anticoagulant treatment following femoro-popliteal venous bypass. Interim evaluation of a controlled clinical study].

    Eighty-eight patients underwent a classical reversed saphenous vein graft for femoro-popliteal occlusive disease and were enrolled in a controlled clinical trial. During the second postoperative week the patients were allocated randomly into one of two groups: group I consisted of 42 patients who received dicumarol and group II (46 patients) served as a control group without any anticoagulant treatment. The mean follow-up time was 24 months. After 12 months the cumulative patency rate was 82%, after 18 months 76% and after 24 months 76%; for patients receiving dicumarol treatment the respective figures were 92%, 86% and 86% and for those with no treatment 70%, 67% and 67%, the difference being significant. When the bypass operation was performed for limb salvage (clinical stages III and IV) the overall results were worse (12 months: 72%, 18 months: 68%, 24 months: 68%) than in stages II, II-III (12 months: 90%, 18 months: 85%, 24 months: 85%). In limb salvage surgery the positive effects of anticoagulant treatment were significant (with treatment: 12 months 87%, 18 months 86%, 24 months 86%; without treatment: 12 months 58%, 18 months 54%, 24 months 54%).

[Hematoma of the abdominal wall as differential diagnosis of cystic pelvic tumor].

    In a woman patient aged 80 years under anticoagulation with dicumarol (Marcumar), abdominal pain suddenly occurred which was located on the right side as well as signs of acute bleeding. Preoperative sonography and computer scan showed a large, cystic tumour, most likely originating from the right ovary with infiltration of the abdominal wall. Intraoperative diagnosis was a large haematoma of the abdominal wall and the retroperitoneum penetrating into the free abdomen.

[Treatment of thrombosis in pregnancy].

    Treatment of venous thrombosis during pregnancy is not without risks. Therefore diagnosis has been made by phlebography. Streptokinase therapy is accompanied with bleedings and needs a precise indication. 4 women have been operated on by thrombectomy. Dicumaroles cause fetal demages and are not suitable for prophylaxis of recurrences. Heparin is the treatment of choice, partly by self injection. During parturition a short time change to low molecular dextran is recommended.
