Two different desmopressin (DDAVP) regimens in enuretic monozygotic twins with allergic diathesis.
Primary monosymptomatic nocturnal enuresis is a common complaint in paediatrics. Treatment with desmopressin nasal spray is one of the most often used pharmacological approaches, but concerning therapy interruption, to date no guidelines have yet been established. The aim of this study was to report the use of desmopressin in two monozygotic twin sisters to establish if tolerability and efficacy of treatment can be affected by different modalities of therapy interruption. Hyponatraemia and cerebral convulsion due to short term DDAVP therapy for control of enuresis nocturna.
Desmopressin (DDAVP) is frequently used in the treatment of primary isolated enuresis nocturna if other approaches have failed. We report a further case of hyponatraemia and cerebral convulsion due to water intoxication after intranasal DDAVP application by a 6 year-old boy with enuresis.. Although adverse reactions in DDAVP (e.g. hyponatraemia) are rare, it should not be considered as the first choice treatment of enuresis nocturna and only be used with caution. Acquired von Willebrand disease in twins with autoimmune hypothyroidism: response to desmopressin and L-thyroxine therapy.
Because of menorrhagia, a 13-year-old girl was found to have type I von Willebrand disease and then chronic autoimmune thyroiditis with hypothyroidism. All clinical and laboratory evidence of von Willebrand disease resolved transiently after infusion of desmopressin, and permanently with L-thyroxine therapy. We recommend investigation for hypothyroidism in patients with newly diagnosed acquired von Willebrand disease. Treatment of neonatal diabetes insipidus with desmopressin.
