[Duodenal ulcer: clinical significance of adaptation defects in the system of neurohumoral regulation].
Radioimmunoassay was used to determine prostaglandin (PGE-2) and cyclic nucleotides (cAMP and cGMP) concentrations in the serum before, 1 and 2 hours after standard test breakfast (5066 kJ) in 65 patients with duodenal ulcer. Basal hyperproduction of prostaglandins and cyclic nucleotides were interpreted as an adaptation-compensatory mechanism. The unbalance of their aftermeal reactions persistent even in ulcer healing may belong to factors of the disease progression. On the biochemical background of cysteamine-induced duodenal ulceration in the rat.
In cysteamine-induced duodenal ulceration the endogenous prostacyclin level initially showed an elevation. This elevation later disappeared, and the endogenous prostacyclin level dropped to zero--that is, below the detection limit. During ulceration the mucosal DNA level decreased, a phenomenon that was directly proportional to ulceration. In the course of the ulcerative process in the duodenal mucosa a de novo protein synthesis took place. This process was most probably a mucus secretion and served as a protective reaction against damaging noxae. The mucosal cyclic adenosine 5'-monophosphate (cAMP) content increased in the so-called pre-ulcerative phase but later returned to the normal (physiologic) level. The changes in the cyclic guanosine 5'-monophosphate (cGMP) level were more pronounced than those of cAMP, and the final result was a decrease in the mucosal cAMP/cGMP ratio. In accordance with our previous results we conclude that a positive cAMP/cGMP 'shift' indicates antiulcerogenic, cytoprotective, reparative processes in the mucosa, whereas its decrease is connected to damaging noxae. [Cyclic nucleotide levels in the gastric juice of patients with recurrent duodenal ulcer].
Cyclic nucleotide content in the gastric juice of patients with duodenal ulcer recurrences was studied before and after reconstructive surgery and in remote periods. The basal secretion phase was characterized by an increased cyclic nucleotide concentration in gastric juice; histamine and insulin stimulation caused decrease in their concentration. After reconstructive surgery, acid production in the basal period decreased, while cyclic adenosine monophosphate concentration increased by more than 100 per cent. The long-term period was characterized by clear intensification of cyclic adenosine monophosphate excretion under the action of histamine. It has been suggested that the release of this nucleotide from gastric mucosa cells is a subsidiary mechanism for maintenance of its necessary intracellular concentration.
