[Skin cancers in Madagascar: where do we stand?]
Skin cancers are malignant keratinizing tumors of epidermal, melaninic or adnexal origin. These cancers are still little known. However they affect people living in Madagascar where the amount of sunshine remains very high with an impact that varies depending on the skin type. The purpose of our study was to determine the epidemio-clinical profile of skin cancers in Antananarivo.. We conducted a retrospective descriptive study of skin cancers in the Department of Oncology of the Joseph Ravoahangy Andrianavalona Hospital, Antananarivo over a period of seven years. It included patients with tumoral skin lesions confirmed by histological examination.. We collected data from 47 cases of skin cancers (male predominance; sex ratio 1.13). The average age of patients was 49.3 years. Farmers were the most affected. The lesions mainly occurred in the head, neck (44%) and lower limbs (42%). Localized skin cancers were detected in 61.7% of cases. Squamous cell carcinoma was the most common skin cancer in Madagascar (37%), followed by the melanoma (21%) and the basal cell carcinoma (11%). Treatment was mainly based on surgery (74.19%).. Squamous cell carcinoma is the most common cancer in Madagascar, followed by melanoma and basal cell carcinoma. Skin cancers affect mainly the young population and require appropriate management. Two antiproliferative triterpene saponins from Nematostylis anthophylla from the highlands of Central Madagascar.
Investigation of the endemic Madagascan plant Nematostylis anthophylla (Rubiaceae) for antiproliferative activity against the A2780 ovarian cancer cell line led to the isolation of the known triterpene saponin randianin (1), and the two new bioactive triterpene saponins 2"-O-acetylrandianin (2) and 6"-O-acetylrandianin (3). The structures of the two new compounds were elucidated based on analysis of their 1D- and 2D-NMR spectra, and mass spectrometric data. The three isolated triterpene saponins displayed moderate but selective antiproliferative activities, with IC(50) values of 1.2, 1.7, and 2.2 μM, respectively, against the A2780 ovarian cancer, but only weak inhibitions of the proliferation of A2058 melanoma and the H522 lung cancer cell lines. [Immunomodulating properties of an extract isolated and partially purified from Aloe vahombe. 3. Study of antitumoral properties and contribution to the chemical nature and active principle].
An immuno-modulator fraction (Alva) extracted from an endemic plant, in the south of Madagascar, the Aloe vahombe, significantly protects mice against bacterial, parasitic and fungal infections. Wishing to verify whether the fraction Alva was active in tumour reduction, we studied its effect on the development of experimental fibrosarcoma and melanoma in mice by intravenous and intracutaneous injections and injections directly into the tumour of the immunostimulant fraction. We have observed cures, only in the case of the McC3-1 tumour but it is encouraging to note that under different experimental conditions the rate of growth of tumours in animals which were treated is slower than in those not treated. The Alva fraction is a substance which is hydrosoluble, thermostabile, having a molecular weight exceeding 30 000 and is a polysaccharide. The predominant sugars are glucose and mannose in 3:1 ratio. Preliminary studies of its action seem to indicate that the Alva fraction acts upon non-specific response and could possibly stimulate the phagocyte activity of the peritoneal macrophagus.
