Comparison of cinnarizine, cyproheptadine, doxepin, and hydroxyzine in treatment of idiopathic cold urticaria: usefulness of doxepin.
Randomized double-blind trials using doxepin and several conventional antihistamines were carried out for treatment of patients with idiopathic cold urticaria. In the first double-blind trial, eight of nine patients preferred doxepin (10 mg three times daily) to cinnarizine (10 mg three times daily). In the second double-blind trial, the results of ice cube tests suppressing the effect of cyproheptadine (4 mg three times daily), doxepin (10 mg three times daily), and hydroxyzine (10 mg three times daily) did not statistically differ. However, doxepin was subjectively the most effective and it had fewer side effects than other treatments that were compared. Doxepin effectively suppressed the wheal and itching responses and shortened the duration of the wheal response in the ice cube test in all patients with cold urticaria who were studied. Treatment of chronic urticaria with an inhibitor of complement activation (cinnarizine).
Twenty-three patients with chronic urticaria were treated with cinnarizine, dexchlorpheniramine and placebo in a double-blind, crossover trial. Compared to placebo treatment both cinnarizine and dexchlorpheniramine caused a statistically significant improvement in the clinical symptoms (p less than 0.01). The two active drugs were found to be equally effective; however, cinnarizine was better tolerated. Both drugs appear to be useful in the treatment of chronic urticaria. [Combined pizotiphen and cinnarizine treatment in urticaria factitia].
30 patients suffering from urticaria factitia were treated with pizotiphen and cinnarizin for 6 weeks. In 13 patients, the effect of treatment was controlled by means of the dermographism test. Placebo administration failed to affect the subjective and objective symptoms. During treatment with pizotiphen and cinnarizin, the flare significantly decreased within the first week and finally did not differ from that observed in healthy persons after 6 weeks of therapy. 19 out of 30 patients were totally free of symptoms. In 11 cases, treatment resulted in significant reduction of the urticaria, but only 7 patients still complained of itching. After repetition of the therapy, 5 patients were free of symptoms; 2 patients still reported on slight itching one year after treatment.
