Long-term corneal endothelial cell loss after cataract surgery. Results of a randomized controlled trial. Oxford Cataract Treatment and Evaluation Team (OCTET).

    Three hundred thirty-three eyes were randomly assigned to three treatments for cataract: group A, intracapsular extraction plus contact lens; group B, intracapsular extraction plus Federov implant (Federov I lenses were made by Rayners Ltd, United Kingdom, based on a design by Professor Fyodorov); and group C, extracapsular extraction plus iridocapsular implant. Endothelial cell loss estimated at one and six months and yearly up to four years showed significantly greater loss in the two implant groups from one year onward. Continuing loss slowed down after the third year, except for the intracapsular extraction plus implant group, in which cell loss appeared complication related. Of operative factors, use of alpha-chymotrypsin significantly reduced cell loss, and pseudophakos contact increased cell loss. Postoperative complications associated with significantly greater cell loss were hypopyon, uveitis, subluxation, shallow anterior chamber, and cystoid macular edema. To date, five and four cases of bullous keratopathy were derived from implant groups B and C, respectively.

The effect of timolol and acetazolamide on transient intraocular pressure elevation following cataract extraction with alpha-chymotrypsin.

    Elevated intraocular pressure (IOP) early after cataract extraction with alpha-chymotrypsin is a well-described, common occurrence. To study the incidence of pressure increase and the efficacy of medical therapy, IOP was monitored every 12 hours beginning on the first postoperative day in 68 patients after otherwise uncomplicated intracapsular cataract extraction performed with one milliliter of alpha-chymotrypsin and wound closure with multiple sutures. Patients were randomly assigned to one of three groups: treatment with oral acetazolamide, treatment with topical timolol, or no treatment. Without prophylactic treatment, the prevalence of pressure elevation at 24 hours postoperatively was 69% with IOP greater than or equal to 25 mm Hg; 29% with IOP greater than or equal to 40 mm Hg. This was independent of the type of sutures used (10--0 nylon or 9--0 silk). The patients of four different surgeons had similar rates of occurrence of postoperative glaucoma. Both timolol and acetazolamide were essentially equally effective in lowering IOP in these patients. No adverse side effects were observed. With these effective treatments available if needed, we believe that routine monitoring of the IOP by applanation tonometry during the early postoperative course provides a good opportunity for reducing the risk of complications from excessive intraocular pressure.

This study was combined with a clinical 'double-blind' trial of the drug Chymoral.

Förster Resonance Energy Transfer-Based Self-Assembled Nanoprobe for Rapid and Sensitive Detection of Postoperative Pancreatic Fistula.

    Postoperative pancreatic fistula (POPF) is the most serious and challenging complication following gastroenterological surgery. Activated pancreatic juice leaking from the organ remnant contains proteases that attack the surrounding tissue, potentially leading to severe inflammation, tissue necrosis, and fistula formation. However, it is difficult to observe pancreatic leakage during surgery and to evaluate the protease activity of leaked fluid at the patient's bedside. This report describes a protein nanocage-based protease ratiometric sensor comprising a pancreatic protease-sensitive small heat-shock protein (HSP) 16.5, which is a naturally occurring protein in Methanococcus jannaschii that forms a spherical structure by self-assembly of 24 subunits, and a chemically conjugated donor-acceptor dye pair for Förster resonance energy transfer (FRET). The HSP-FRET probe was constructed by subunit exchange of each dye-labeled engineered HSP, resulting in a spherical nanocage of approximately 10 nm in diameter, which exhibited very high stability against degradation in blood plasma and no remarkable toxicity in mice. The efficiency of FRET was found to depend on both the dye orientation and the acceptor/donor ratio. Pancreatic proteases, including trypsin, α-chymotrypsin, and elastase, were quantitatively analyzed by fluorescence recovery with high specificity using the HSP-FRET nanoprobe. Furthermore, the HSP-FRET nanoprobe was sufficiently sensitive to detect POPF in the pancreatic juice of patients using only the naked eye within 10 min. Thus, this novel nanoprobe is proposed as an effective and convenient tool for the detection of POPF and the visualization of activated pancreatic juice during gastroenterological surgery.

Adequate enzymatic substitution in treating exocrine pancreatic insufficiency.

    The chymotrypsin in the stool test was used to monitor adequate enzymatic substitution in treating exocrine pancreatic insufficiency with 18 patients (16 suffering from chronic pancreatitis and 2 having passed duodenopancreatectomy due to pancreatic cancer). This test helps to identify pancreatic insufficiency and can be successfully used in monitoring the adequate amount of pancreatic substitute, which, we have found, differs from patient to patient. The dosage can be higher in cases of chronic pancreatitis than in those required after duodenopancreatectomy.

[Experimental studies of the prevention of postoperative adhesions].

    Female wistar rats were used for the testing of different substances to prevent postoperative intraabdominal adhesions. It could be demonstrated that the best results were obtained by dextran 70 with a concentration of 10%. Good effects were seen also by contrykal (aprotinin) and hylase (hyaluronidase). By the combination of dextran 70 30% with contrykal the spread of adhesions was very low but the effect was not significant better than with dextran 70 10% alone. For the prevention of intraabdominal adhesions operative techniques with minimal lesions are important too of the peritoneal epithelium.

Ligneous conjunctivitis after pingueculae removal in an adult.

    A 45-year-old man who had uneventful excision of bilateral pingueculae developed bilateral membranous lesions involving the bulbar conjunctivae and corneas. Histologically, the membranes were composed mainly of large fibrinous deposits intermixed with acute and chronic inflammatory cells with areas of fibroblastic and capillary proliferation resembling granulation tissue. By electronmicroscopy the amorphous acidophilic masses were composed of electron-dense, fibrillar material with a periodicity of 10-12 mm, which was consistent with fibrin. Despite mechanical removal of the membranes, they continued to recur rapidly over a period of several months. The lesions apparently responded slowly to topical enzymatic therapy that consisted of hyaluronidase (175 U/ml) and alpha-chymotrypsin (1:5000) drops. Follow-up examination, approximately 1 year after surgery, revealed that the patient was asymptomatic. Ocular examination disclosed slight persistence of gelatinous membranes on the bulbar conjunctivae, most prominent in the left eye.

Vitreous haemorrhage after cataract extraction.

    One hundred eyes undergoing intracapsular cataract extraction and 100 undergoing extracapsular extraction were examined prospectively within one week postoperatively and again at 6-10 weeks postoperatively. Indirect ophthalmoscopy showed vitreous haemorrhage in 36% of the intracapsular group and 13% of the extracapsular group. Vitreous haemorrhage was significantly related to the occurrence of operative hyphaema (p less than 0.01) but not to iridectomy (p greater than 0.05). In the intracapsular group the use of chymotrypsin significantly reduced the incidence of vitreous haemorrhage (p less than 0.01). Three different types of vitreous haemorrhage were identified--streaks, diffuse haze, and beads in the vitreous base. Wound haemorrhage and operative hyphaema account for most cases, but it is suggested that zonular traction may be responsible for the bead haemorrhages and also for the retinal haemorrhages reported by other authors. Although no difference in visual result or incidence of cystoid macular oedema was observed in this study, a relationship to the vitreoretinal pathology of aphakia is suggested.

[Digestive function of the residual pancreas after partial duodenopancreatectomy for chronic pancreatitis (author's transl)].

    Forty-eight patients with chronic recurrent pancreatitis treated by resection of the head of the pancreas were restudied more than a year post-operatively. In addition to general features such as symptoms, alcohol consumption and work ability, faecal weight, its fat content, fat and fatty acid balance and faecal chymotrypsin were measured. According to the patients' own estimate, late results were good or very good in 70-90%. Faecal fat content and balance indicated high-grade exocrine pancreatic insufficiency in 80 and 90%, respectively. But it was easily controlled by drugs. In a third of the cases there was the need to supplement the diet with medium-chain triglycerides. Its components are satisfactorily absorbed even when the fat utilisation is severely abnormal.

[Effect of timolol and acetazolamide on intraocular hypertension after intracapsular lens extraction with alpha-chymotrypsin (author's transl)].

    After intracapsular lens extraction with alpha-chymotrypsin three groups of 14 patients were formed. The first group was given Timolol 0.25% 2 x 1 drops, the second acetazolamide in sustained release form 1 x 500 mg and the third served as control. In all three groups the postoperatively elevated pressure diminished significantly in the first five postoperative days. The pressure diminution on the fourth and fifth postoperative day was accelerated by Timolol but not by acetazolamide.

[Animal experiments on the prevention of peritoneal adhesions (author's transl)].

    Several possibilities for the prevention of adhesions were checked in animal experiments. In rats adhesions were produced by a standardized operative procedure. For the prevention of adhesions an artificial ascites with normal saline solution or Dextran was tried. To maintain ascites an intraperitoneal catheter was left in place for the first three post-operative days and refilled. The proteolytic effect of Chymotrypsin, the proteinase inhibitor Trasylol and of Glucocorticoids was investigated. All three drugs showed a definite therapeutic effect. In 45-50% of the rats adhesions were prevented. A combination of Trasylol, Glucocorticoids and Dextran prevented adhesions in 65% of the animals. When the ascites was maintained for four days 80% of the animals remained free of adhesions.

The effect of corneal section on early increased intraocular pressue after cataract extraction.

    Increased intraocular pressure in the immediate postoperative period commonly occurs after cataract extraction. We compared a series of patients operated on through a limbal section to a series operated on through a corneal section. The incidence of increased IOP was significantly lower in the corneal section group. This supports the theory that trabecular damage is a major factor in causing early increased IOP after cataract extraction.

Prevention of intraperitoneal adhesions by dextran. Hydrocortisone and chymotrypsin. An experimental study.

    The purpose of an experimental study in guineapigs was to prevent adhesion formation in the intra-abdominal space by simultaneous intraperitoneal infusion of hydrocortisone, dextran and chymotrypsin. An examination made three weeks after the operation revealed a highly significant (p less than 0.001) difference in favour of the dextran, dextran-chymotrypsin and dextran-chymotrypsin-hydrocortisone groups when compared with the NaC1 and control groups.

[Prevention and treatment of postoperative broncho-pulmonary complications with ultrasonic aerosols].

    The method and results of applying ultrasonic aerosols in 485 patients, operated upon on abdominal organs, are presented. Ultrasonic aerosols were included in the complex of prophylactic and therapeutic measures in the postoperative period. Prophylactic measures were started immediately after patient's being returned from the operating room. Special attention was given to patients in whom ventilatory disturbances developed during anesthesia, to patients with obesity, chronic bronchitis, emphysema, pneumosclerosis and also those, who negated the rationality of prophylactic measures. The use of finely dispersed aerosols after the method suggested by the authors allowed a 6 times reduction in the incidence of pulmonary complications and more than twice shortening of the postoperative pneumonia course.

The influence of various drugs on artificial oedema of the rat's bladder.

    An experimental oedema of the bladder wall was produced by electric coagulation current in 130 rats. The amount of oedema was calculated by determination of the difference between the weight of the freshly excised wet bladder and that after drying to constant weight. Thus, oedema of a fairly constant and reproducible amount could be produced. The effect of three different drugs on such oedema was tested (Na-Aescinat, 6-methylprednisolone, alpha-Chymotrase). After the application of Na-Aescinat and 6-methylprednisolone no significant oedema could be measured after 48 h. Using alpha-Chymotrase, there was no measurable influence on the oedema.

[In vitro studies on the effect of alpha-chymotrypsin on hemostasis].

    Postoperative bleeding was observed in 2 patients for whom alpha-chymotrypsin had been prescribed to prevent haematoma or oedema formation. In vitro experiments with citrated blood added with alpha-chymotrypsin evidenced (thrombelastogram, hirudin tolerance test, euglobulin-lysis time) an action on haemostasis. As compared to blanks, the application of an alpha-chymotrypsin dose which was 100-fold higher than those currently used in therapy resulted in a significant shortening of the r-time (thrombelastogram), delayed blood clotting (hirudin tolerance test) and a shortening of the euglobulin-lysis time. In vivo experiments are needed for further elucidation.

[Digestive and endocrine functions after partial duodeno-pancreatectomy].

    Pancreatico-duodenectomy was performed in 11 patients for malignant or inflammatory tumours of the head of the pancreas or the region of the papilla. Digestive and endocrine functions were determined after the operation. In all cases faecal fat values were abnormal, indicating a 90% loss of pancreas. 14C-exhalation measurement, chymotrypsin determination in stool, and amylose tolerance test were also performed. Oral glucose-tolerance tests with plasma-insulin measurement indicated asymptomatic diabetes mellitus in the majority of patients. Two patients whose diabetes was controlled by tablets before the operation required insulin treatment afterwards. A decreased serum-gastrin level proved the existence of gastric and extragastric sources of gastrin.

Bulbar ligneous conjunctivitis after pterygium removal in an elderly man.

    Ligneous conjunctivitis occurred unilaterally in a 74-year-old man after pterygium excision and only involved the bulbar conjunctiva. Histochemical studies confirmed the presence of mucopolysaccharides in the lesion sensitive to hyaluronidase. The patient was treated successfully with topical hyaluronidase and alpha-chymotrypsin, with no recurrence 20 months after cessation of therapy.
