Effects of intracameral administration of α-chymotrypsin on intracapsular lens extraction and postoperative outcome in clinically normal dogs.
To assess the intraoperative and postoperative clinical effects and histologic effects of intracameral administration of α-chymotrypsin in clinically normal dogs undergoing standard intracapsular lens extraction (ICLE).. 6 young adult male dogs without evidence of systemic or ocular disease.. All dogs underwent bilateral ICLE 7 minutes following injection of 75 U of α-chymotrypsin or an identical volume (0.5 mL) of a commercially available balanced saline solution (BSS) into the posterior chamber of the eye. Ease of lens extraction was subjectively assessed and intraoperative intraocular hemorrhage and fibrin accumulation scored. For 27 days after surgery, ocular hyperemia and discharge, chemosis, corneal edema, hyphema, and aqueous flare were scored, and intraocular pressure (IOP) was measured. Thirty days after surgery, histologic evidence of anterior synechia, collapse of and inflammation within the iridocorneal angle, and iritis were scored.. In 5 of 6 dogs, the surgeon was able to correctly identify the eye treated with α-chymotrypsin on the basis of ease of lens extraction. Mean intraoperative intraocular hemorrhage and fibrin scores for BSS-treated eyes were significantly higher than for α-chymotrypsin-treated eyes. Postoperatively, there were no significant differences between treatments for any clinical variables, including IOP Histologic scores were not significantly different between treatments for any variable. Vision was lost as a result of glaucoma in 1 α-chymotrypsin-treated eye and 1 BSS-treated eye.. Intracameral administration of 75 U of α-chymotrypsin 7 minutes before ICLE facilitated lensectomy without apparent adverse effects in clinically normal dogs. Effects of alpha chymotrypsin on the canine eye.
Injection of 150, 500, or 750 U of alpha chymotrypsin into the posterior chamber of clinically normal Beagles resulted in changes in intraocular pressure and iridocyclitis. In the 6 eyes treated with 500 or 750 U, lens subluxation occurred in 4 eyes, retinal detachment in 1 eye, retinal degeneration in 1 eye, and optic nerve degeneration in 6 eyes. Scanning electron microscopic changes included dose-related zonulolysis and zonulary remnants within the aqueous humor outflow pathways. Ultrastructural changes by transmission microscopy of the optic nerve degeneration revealed loss of axoplasm and organelles and disruption of the myelin sheaths. Retinal and optic nerve changes may represent the direct toxic effects of alpha chymotrypsin and/or the indirect effects of increased intraocular pressure. Histopathology of the lens capsule in fibrillopathia epitheliocapsularis (FEC) or so-called senile exfoliation or pseudoexfoliation. An electron microscopic study.
Twenty lenses from cataractous eyes have been subjected to enzymatic breakdown by collagenase and alpha-chymotrypsin in order to remove part of the lens capsule and the zonular apparatus. Twelve of the lenses had clinical fibrillopathia epitheliocapsularis (FEC syndrome). In addition four FEC lenses and four ordinary cataractous lenses served as controls. A characteristic fibrillar substance was found in the FEC lenses located to circular epithelium-near areas varying in size from 10 micrometers to 150 micrometers. The circular areas were found in zona germinativa in front of the bow region and corresponded to the discoid plaques of the deep layer. The ultrastructure of the finely fibrillar material of the deep layer were indistinguishable from that of the surface material of the peripheral band and the central disc. A connection between the discoid plaques of deep layer and the peripheral granular band were demonstrated in the form of radial "cobble-stone" sectors seen in partially digested FEC capsules. An increased amount of age dependent spindleshaped formed bodies were also found in FEC lenses. The evidence presented points towards a lenticular origin of the Busacca-bushes in the peripheral ganular band of the lens capsule. Cataract and pseudoexfoliation. A clinicopathological study.
100 lenses from 98 consecutive patients with senile cataract were investigated histologically for pseudoexfoliation. The patients came from an area known for its high incidence of pseudoexfoliation. The examiner was not aware of the clinical examination at the time of histopathological study. Pseudoexfoliation was observed in 33 lenses. There seemed to be an increasing incidence of pseudoexfoliation with increasing age of the patients. Only 16 of the cases of pseudoexfoliation had been recorded preoperatively, but routine pupillary dilatation had not been carried out. The use of alpha-chymotrypsin at cataract extraction did not preclude histological diagnosis of pseudoexfoliation.
