Disruption of the SapM locus in Mycobacterium bovis BCG improves its protective efficacy as a vaccine against M. tuberculosis.

    Mycobacterium bovis bacille Calmette-Guerin (BCG) provides only limited protection against pulmonary tuberculosis. We tested the hypothesis that BCG might have retained immunomodulatory properties from its pathogenic parent that limit its protective immunogenicity. Mutation of the molecules involved in immunomodulation might then improve its vaccine potential. We studied the vaccine potential of BCG mutants deficient in the secreted acid phosphatase, SapM, or in the capping of the immunomodulatory ManLAM cell wall component with α-1,2-oligomannoside. Both systemic and intratracheal challenge of mice with Mycobacterium tuberculosis following vaccination showed that the SapM mutant, compared to the parental BCG vaccine, provided better protection: it led to longer-term survival. Persistence of the SapM-mutated BCG in vivo resembled that of the parental BCG indicating that this mutation will likely not compromise the safety of the BCG vaccine. The SapM mutant BCG vaccine was more effective than the parental vaccine in inducing recruitment and activation of CD11c(+) MHC-II(int) CD40(int) dendritic cells (DCs) to the draining lymph nodes. Thus, SapM acts by inhibiting recruitment of DCs and their activation at the site of vaccination.

HLA-DR2 phenotype and plasma lysozyme, beta-glucuronidase and acid phosphatase levels in pulmonary tuberculosis.

To elucidate the role of HLA-Class II genes/gene products on phagocyte enzymes such as lysozyme, beta-glucuronidase and acid phosphatase in the plasma of pulmonary tuberculosis patients.. Serological determination of HLA-DR and -DQ antigens was carried out in 54 active and 84 inactive pulmonary tuberculosis (quiescent) patients and 36 healthy control subjects. The levels of lysozyme, beta-glucuronidase and acid phosphatase were measured in the plasma of tuberculosis patients and control subjects.. Increased lysozyme levels were observed in active pulmonary tuberculosis patients. beta-glucuronidase activity was higher in inactive-TB than in active-TB patients and control subjects. HLA-DR2 positive patients showed a lower lysozyme level than -DR2 negative patients.. Increase in the plasma lysozyme level in active TB reveals the active stage of the disease. Further, increase in the activity of beta-glucuronidase in inactive-TB patients reveals the quiescent stage of the disease. The low level of lysozyme in HLA-DR2 positive patients may also be one of the possible factors involved in susceptibility to tuberculosis.

Enzyme levels in bronchoalveolar lavage fluid and serum of active pulmonary tuberculosis patients.

    Bronchoalveolar lavage (BALF) was found to be a useful index of cell damage. It has been observed that the enzymes in BALF could give an idea of cell damage in a pulmonary disease. Acid phosphatase, alkaline phosphatase, leucine aminopeptidase and gamma-glutamyl-transpeptidase were assessed in mild, moderate, and severe pulmonary tuberculosis patients and Mantoux-negative normal controls. Activities of these enzymes were found to be higher in patients and were increasing with the severity of the disease. Increase in these enzymes in pulmonary tuberculosis patients could be attributed to lung tissue damage.
