Dual-Phase 18 F-FP-CIT PET in 2 Different Clinical Phenotypes of Sporadic Creutzfeldt-Jakob Disease.

    Early diagnosis of Creutzfeldt-Jakob disease (CJD) patients is often challenging due to the low sensitivity of the current clinical diagnostic criteria. We describe MRI and dual-phase 18 F-FP-CIT PET findings in 2 cases of sporadic CJD presenting different clinical phenotypes (Heidenhain variant and corticobasal syndrome). Our case series suggest that dual-phase FP-CIT-PET findings may improve the diagnosis of CJD by combining the perfusion patterns in early phase with the dopamine transporter density in delayed phase. Familiarity with these dual-phase FP-CIT PET findings is helpful for early correct diagnosis of CJD.

18F-FP-CIT PET/CT in a case of probable sporadic Creutzfeldt-Jakob disease with parkinsonism as initial symptom.

    Creutzfeldt-Jakob disease (CJD) is a low-prevalence, fatal neurodegenerative disease. Parkinsonism as first symptom of CJD is rare. We present a case manifesting difficulty falling asleep as unspecific prodromal symptom and parkinsonism as initial symptom. The patient received positron emission tomography/computed tomography (PET/CT) of dopamine transporter (DAT) using 18 F-FP-CIT. The DAT-scan demonstrated presynaptic dopaminergic deficit in bilateral posterior putamen, which supports the hypothesis of nigrostriatal pathway dysfunction in CJD.

Striatal [123I] FP-CIT SPECT demonstrates dopaminergic deficit in a sporadic case of Creutzfeldt-Jakob disease.

    The frequent occurrence of movement disorders such as myoclonus, parkinsonism and dystonia, strongly suggests an involvement of the dopaminergic system in sporadic Creutzfeldt-Jakob disease (sCJD), but this issue has not been specifically addressed yet.. We report a patient who after a sub-acute focal clinical onset, developed the full clinical picture of probable sCJD. Given the early unilateral right extrapyramidal rigidity, the patient was assessed by single-photon emission computed tomography of the dopamine transporter (DAT) using [123I] FP-CIT.. DAT-scan demonstrated reduced values of presynaptic receptorial trace in the putamen, particularly on the left side, consistent with functional putaminal dopaminergic presynaptic alteration.. The present observation emphasizes the possible role of DAT imaging studies in the investigation of the pathogenesis of movement disorders in CJD.
