[Bone density in type 2 diabetes as related to obesity and adrenal function].

    The purpose of the study was to analyse bone density in patients with noninsulin-dependent diabetes mellitus with respect to such factors as the magnitude of obesity, serum cortisol concentration and 17-ketosteroid excretion and insulinemia level. Bone density was measured by ultrasonography, insulin was determined by radioimmunoassay, and steroid level by using the method of Zimmerman. The patients had imminent and overt osteoporosis. The highest serum cortisol concentration was in groups with lower T-score significant negative correlation between cortisol and SOS index. The lowest 17-ketosteroid level was detected in the patients with the lowest T-score; a significant correlation was observed between 17-ketosteroids and BUA index. The present findings indicate an imbalance between bone protective and resorptive factors. This leads to osteopenia in patients with noninsulin-dependent diabetes mellitus despite a protective effect of obesity.

Serum free triiodothyronine: determinant of the pseudohypercortisolism in obesity?

    Eighteen euthyroid, grossly-obese females without characteristics of Cushing's syndrome were studied. The absolute urinary excretion rate of 17- ketogenic steroids was increased in 13 patients, whereas the excretion rate of urinary free cortisol was slightly elevated in four out of 15 patients. The urinary excretion rate of 17-ketogenic steroids was significantly (P less than 0.005) positively correlated (rs = 0.66) with the serum concentration of free triiodothyronine, whereas no correlation with the serum concentration of free thyroxine could be demonstrated. The urinary excretion rate of urinary free cortisol was not correlated with the serum concentration of free triiodothyronine or free thyroxine. The study indicates that the increased absolute urinary excretion rate of 17-ketogenic steroids in obesity is a result of increased cortisol metabolism and may be due to actions to triiodothyronine.

Catabolic effects and the influence on hormonal variables under treatment with Gynodian-Depot or dehydroepiandrosterone (DHEA) oenanthate.

    53 patients from a mainly climacteric population were treated monthly with 200 mg dehydroepiandrosterone (DHEA) oenanthate or with 1 ampoule Gynodian-Depot. Pronounced adiposity was present in 15 of these cases. Hormonal variables were determined before the treatment and during the depot effect of the preparations in order to study the principle which supports the oestrogenic influence and any weight-reducing influence under administration of DHEA. The elimination of low-polar oestrogens increased considerably in 4 out of 13 post-menopausal cases treated with DHEA. This effect is probably indirect and presupposes intact ovaries. The incorporation of exogenous DHEA into the excretion of 17-ketosteroids and of 17-ketogenic steroids, such as those of androsterone + aethio-cholanolone, depends on the size of the initial pool inasmuch as it is higher in small initial pools than in saturated pools- the size of the pool being age-dependent. An average weight loss of greater than 1 kg/mth was observed under DHEA treatment in 7 out of 15 adipose cases. In comparison to the other 8 adipose cases, these 7 were younger and therefore also displayed higher values for 17-ketosteroids and their individual fractions. These circumstances appeared to explain why the administration of DHEA resulted in higher levels of free plasma DHEA which, in contrast to the cases without loss of weight, also resulted in an increase of renal DHEA-sulphate clearance. It was concluded from the findings that this is the explanation for the catabolic effect of exogenous DHEA. Post-menopausally increased FSH and LH fractions were markedly suppressed in about half of the determinations after Gynodian-Depot administration, the findings indicating that DHEA is probably involved in suppression of the LH fraction.

The effect of weight loss on sex steroid secretion and binding in massively obese women.

    We have previously reported increased testosterone and androstenedione concentrations and decreased sex hormone binding globulin (SHBG) concentrations in the plasma of massively obese women. We now report that these plasma hormone concentrations return to normal in twelve of the same women after substantial weight reduction and these changes are associated with more normal menstrual cycles. We conclude that body weight and fat are important determinants of sex steroid secretion and binding and thus influence menstrual function.

Urinary steroid metabolites and the overgrowth of lean and fat tissues in obese girls.

    Studies were made of steroid metabolites excreted in the urine of 17 obese girls 11.4 to 16.8 yr and 17 normal girls 11 to 17 yr. Creatinine excretion (muscle mass), total body water (or deuterium space), lean body mass and body fat were determined in the obese girls. Extracellular volume (corrected bromide space) was also measured and by difference with body water, intracellular water or soft tissue cell mass was calculated. In normal girls 24-h creatinine excretion was determined, but body water was predicted from height and weight. It was found, as in previous studies, that the obese girls had excess muscle mass and soft tissue cell mass for height. The excess growth of muscle, lean tissue, and body length in obese girls correlated with increments in oxosteroid (17 ketosteroid) excretion. The overall weight increase correlated with increased excretion of corticosteroid metabolites--a finding of interest since a physiological Cushing's syndrome was postulated for fat girls many years ago. When the normal and obese girls were divided by age at 14 yr and the subgroups compared (normal obese) the younger girls showed differences with respect to height, weight, total body water, fat and percentage fat. Differences in steroid metabolites were not found. In older girls the same findings were made again, but here it was clear that the increments in body size, particularly muscle mass, correlated with augmented oxosteroid excretion. Evidence is cited that these findings are not just related to a larger steroid pool in obese girls.

Increased glomerular filtration rate and adrenocortical function in obese women.

    Glomerular filtration rate (GFR) was measured in 16 obese women aged 28-48 years (overweight 65-110 per cent) and compared to GFR in 16 non-obese women of the same age and height. None of the women had signs of nephrourological disorders. GFR was determined from the total 51 Cr-EDTA plasma clearance measured by a simplified single-injection technique. In the obese women GFR (median value 129.0 ml/min) was significantly (P less than 0.002) increased by 24.6 per cent. The urinary excretion rate of 17-ketogenic steroids was increased in nine out of 14 obese women, but no correlation with GFR could be demonstrated. GFR in the obese women showed a significant positive correlation with the plasma cortisol concentration (rs = 0.77; P less than 0.01) and with the urinary excretion rate of free cortisol (rs = 0.78; P less than 0.01). Whether the correlation between the plasma cortisol concentration and the increased GFR in obese women is causal needs further investigation.

Psychophysiological study on fasting therapy.

    The Tohoku University method of fasting therapy was performed on 380 patients. The clinical results revealed an efficacy rate of 87%. With regard to psychosomatic diseases, irritable colon syndrome, neurocirculatory asthenia, mild diabetes mellitus, obesity and borderline hypertension were good indications for this therapy. In order to clarify the therapeutic mechanism, several clinical examinations were administered before, during and after therapy. EEG data was analysed according to the power spectral method. The peak frequency decreased as fasting progressed, while it increased as re-fed continued. Percent energy of alpha waves after fasting therapy was significantly higher than that of the pre-fasting stage. The dexamethasone suppression rate of urine 17-OHCS after fasting therapy was significantly lower than that of the pre-fasting stage. It seems that ketone nutrition may work as a strong stressor in the brain cell, temporarily placing all biological mechanisms in a stress state and then activating the natural healing power inherent to the human body, thereby bringing about homeostasis.

Control of obesity in Avy/a mice by 5alpha-androstan-17-one.

    Treating VY/WfL-Avy/a mice with 5 alpha-androston-17-one, a mammalian glucose-6-phosphate dehydrogenase inhibitor, prevented the mice from becoming obese. The weight difference between treated and control Avy/a mice was mainly due to a decreased accumulation of triacylglycerol. The compound did not suppress appetite, had no detectable toxicity and did not affect the lipogenesis rates in the liver and carcass. The weight-controlling effect of 5alpha-androstan-17-one in Avy/a mice was reversible upon withdrawal of treatment.

Steroid hormones and human breast cancer. An hypothesis.

    The likely occurrence of two distinct types of human breast cancer is discussed. In the development of the "Western-environmental" or "adrenal" type, it is proposed that dehydroepiandrosterone sulphate (DHEAS), secreted by the adrenal, may be implicated. This steroid is metabolized by mammary tumors to active androgens and estrogens. That these metabolic pathways are very similar to that in skin is emphasized and this is possibly explained by evolution of the mammary gland from primitive sweat glands. Formation of DHEAS from DHEA in mammary tumors, claimed to be correlated with an individual subject's prognosis and response to hormone ablation, is discussed in the light of a possible regulatory role of the sulphotransferase. It is proposed furthermore that a specific control of DHEAS secretion in the adrenal may exist at the level of the sulphurylation step. Nutritional factors are also possibly implicated here and these are discussed in the light of: 1) a possible higher secretion rate of DHEAS in obese subjects; and 2) the low breast cancer incidence in the Japanese and the accompanying low blood DHEAS levels.

Free cortisol in obesity; effect of fasting.

    Plasma and urinary corticosteroids were measured in 13 obese subjects before and after high and low protein diets, and after fasting. During isocaloric high and low protein diets, urinary 17-oxogenic steroids and to a lesser extent urinary free cortisol excretion rose and fell in parallel with protein intake. Plasma unbound cortisol levels were not much changed by high or low protein intake. However, during 7 to 11 days total fasting, there was a highly significant rise in plasma unbound cortisol at 24.00. A smaller rise occurred at 09.00. The overall effect was a considerable diminution of the day-night variation of plasma unbound coritsol levels during fasting, and a rise in prevailing unbound cortisol levels and urinary free cortisol excretion. In 3 subjects tested these changes were reversed immediately by glucose re-feeding.

Hypogonadism in Prader-Willi syndrome.

    Sexual development was evaluated in 9 female and 2 male subjects with Prader-Willi syndrome. The process of sexual development and degree of genital development attained were found to be variable but abnormal in all subjects. Hypothalamic-pituitary-gonadal functions were evaluated by measurement of serum Luteinizing Hormone and plasma testosterone responses to stimulation by clomiphene citrate and plasma testosterone responses to stimulation by human chorionic gonadotrophin. The degree of vaginal estrogenization was variable. The testicular biopsies showed abnormalities mainly in the germinal epithelium. In agreement with previous studies, it was concluded that the abnormalities of sexual development in this syndrome are mainly due to a defect in the hypothalamic pituitary axis. Adrenal function was not found to be grossly abnormal. The 17 ketosteroid excretion values were low, probably explaining the rather sparse pubic and axillary hair observed in these patients. The urinary 17-hydroxycorticosteroid creatinine ratios were found to be elevated, probably due to decreased creatinine excretion, reflecting the muscular abnormalities of these subjects.

[Differential diagnosis of essential adiposity and Cushings syndrome].

According to our opinion anamnesis and clinical findings give the possibility of making a diagnosis already on a large scale. Analyses of the hormones and special X-ray examinations only serve for the preoperative ascertainment and localisation of the endocrine defective function.
