Testosterone undecanoate: a new orally active androgen.

    Oral testosterone undecanoate (TU) in arachis oil has been evaluated with a view to its possible use as a means of androgen replacement therapy. A single 100 mg dose was found to elevate plasma androgen levels and urinary 17-ketosteroid excretion in 6 normal men. Ninety mg/day and 60 mg/day doses taken by a hypogonadal man resulted in sustained levels of androgen which appeared physiological when measured by radioimmunoassay without chromatography. However, upon separation of the steroids by chromatography it was found that much of the androgen present was in fact dihydrotestosterone not testosterone. Both TU and dihydrotestosterone undecanoate were detected in plasma by gas chromatography and it is suggested that the ester is absorbed as such from the intestine and the unesterified steroid subsequently released by hydrolysis. The convenience of oral administration, the resulting prolonged elevated plasma androgen levels and the probable lack of deleterious effects on the liver may render oral TU of value where androgen replacement therapy is indicated.

[Hypothalamo-hypophyseal-adrenal and hypothalamo-hypophyseal-gonadal interactions in young persons with different forms of male hypogonadism].

    The study includes 115 men, 17 to 26 years of age, suffering from various forms of male hypogonadism: Klinefelter's syndrome--40 patients, male Turner's syndrome--2 patients, primary hypogonadism with other etiology--14 patients, cryptorchism--12 patients, anorchidism--8 patients, secondary hypogonadism--26 patients and late puberty--13 patients. A raised hydrocortisone plasma level was found. In the anorchidism cases the basic testosterone level was the lowest one combined with the highest basic hydrocortisone level. The 24 hour rhythm of hydrocortisone secretion in Klinefelter's syndrome is regular but the hour values are higher. In cases of primary hypogonadism with other etiology, cryptorchism and secondary hypogonadism the 24 hour rhythm is disturbed and the hydrocortisone values are higher.

[Functional status of the hypophyseal-gonadal system in patients with chronic bronchitis].

[Participation of gonads in pulmonary silicosis].

    A survey is given on the participation of endocrine glands in the pulmonary silicosis. At the same time a new clinical and experimental contribution to the question of the appearance of a hypogonadotropic hypogonadism is given. The frequency of this hypogonadism increases with the severity of the pulmonary silicosis. The immunological opinions as well as the neuroergonal ones concerning the pathogenesis of pulmonary silicosis gave the possibility of a better explanation of the mechanisms, by means of which also other organs may be attacked in pulmonary silicosis. The neuroendocrine system gives a defensive answer to the aggressive silicotic pulmonary process, which in general slowly progresses, but which may also be certain hormones from the group of the glucocorticosteroids in the adrenal cortex. The intervention into the axis hypothalamus-hypophysis-adrenal cortex has been provided in all defence processes of the organism against every aggression. By the functional or organic damage of the neuroendocrine system, by the immediate toxic effect of silicium dioxide or the monosilicium acid as well as by the effect of the secondary hypoxia in pulmonary silicosis an insufficiency in the stimulation and excretion of certain hormones develops. The origin of this insufficiency is found in the hypothalamo-hypophyseal centres, and the effects extend to all endocrine glands which depend on the system hypothalamus-hypophysis. This mechanism is made responsible for the appearance of a hypogonadotropic insufficiency of gonads in the course of a pulmonary silicosis and is proved clinically and experimentally in the cases investigated by the authors

Response of plasma testosterone, urinary 17-oxosteroids, oestrogens, and androsterone plus aetiocholanolone to human chorionic gonadotrophin in dexamethasone-suppressed men.

    The administration of human chorionic gonadotrophin (HCG) to dexamethasone-suppressed men caused parallel changes in the concentration of plasma testosterone and in the urinary output of androsterone+aetiocholanolone, total 17-oxosteroids and oestrogens. Discrepant results occurred in only four of the thirty-seven men tested. With these exceptions, the response to HCG could by followed as well by measuring androsterone+aetiocholanolone, 17-oxosteroid or oestrogen excretion rates as by following plasma testosterone levels. The most sensitive index of response was the rate of appearance of oestrogens in urine, and the next that of androsterone+aetiocholanolone.

Hypogonadism in Prader-Willi syndrome.

    Sexual development was evaluated in 9 female and 2 male subjects with Prader-Willi syndrome. The process of sexual development and degree of genital development attained were found to be variable but abnormal in all subjects. Hypothalamic-pituitary-gonadal functions were evaluated by measurement of serum Luteinizing Hormone and plasma testosterone responses to stimulation by clomiphene citrate and plasma testosterone responses to stimulation by human chorionic gonadotrophin. The degree of vaginal estrogenization was variable. The testicular biopsies showed abnormalities mainly in the germinal epithelium. In agreement with previous studies, it was concluded that the abnormalities of sexual development in this syndrome are mainly due to a defect in the hypothalamic pituitary axis. Adrenal function was not found to be grossly abnormal. The 17 ketosteroid excretion values were low, probably explaining the rather sparse pubic and axillary hair observed in these patients. The urinary 17-hydroxycorticosteroid creatinine ratios were found to be elevated, probably due to decreased creatinine excretion, reflecting the muscular abnormalities of these subjects.

LH and FSH in serum after intramuscular administration of LH/FSH-releasing hormone in normal and hypogonadal men.

    The increase in serum concentrations of LH and FSH after intramuscular administration of LH/FSH-RH was investigated in normal and hypogonadotrophic hypogonadal males. LH and FSH were measured by specific radio-immunoassays. To evaluate the appropriate dose of LH/FSH-RH for intramuscular administration different doses were administered and with 200 mu g an increase in LH and FSH was found comparable to the results obtained by other investigators using intravenous administration. After the initial studies a simple test including two determinations before LH/FSH-RH administration and one 60 min and 120 min after administration was developed and used in all cases. In 12 normal males a significant increase in the serum concentration of both LH and FSH was found averaging 381% and 148% respectively 60 min after 200 mu g LH/FSH-RH with a slight decrease in LH but not in FSH after 120 min. A positive response similar to that in normal men was observed in 11 hypogonadotrophic hypogonadal patients presenting different clinical pictures. This indicates that in all these patients the hypogonadotrophic state was due to a hypothalamic defect and not to a hypophyseal disorder.

Urinary steroid pattern in male hypogonadotrophic hypogonadism.

    During clinical studies on a 34-year-old man with marked hypogonadism secondary to pituitary gonadotrophin insufficiency, an attempt was made to determine whether the testes were capable of androgen production when stimulated by exogenous chorionic gonadotrophin (A.P.L.).Androgen excretion before and after chorionic gonadotrophin administration was studied. Total and fractionated 17-ketosteroid patterns suggested a severe Leydig cell deficiency which was confirmed by testicular biopsy.Panhypopituitarism was considered excluded by a normal response to metyrapone ditartrate, by a normal serum PBI and a normal radioactive iodine uptake. Genetic disorders were likewise excluded to the extent possible by determining that the chromosome pattern was normal and by determining that no similar condition existed in another member of the same family. The association of anosmia with gonadotrophin insufficiency in this case suggests a primary disorder in the hypothalamo-mamillary-tuber cinereum area.

ATAXIA-TELANGIECTASIA.

    Ataxia-telangiectasia is a syndrome of progressive cerebellar ataxia and other neurological manifestations associated with conjunctival and cutaneous telangiectases and with recurrent sino-pulmonary infections. Immunological and endocrine abnormalities occur. Two girls with this disease are described. The first had only minor respiratory infections; her serum proteins and immunity responses appeared normal. The second had recurrent pulmonary infections and bronchiectasis; she also exhibited sclerodermatous changes, poor development of secondary sexual characteristics with low urinary excretion of 17-ketosteroids, and lymphopenia. Autopsy at 17 years showed bilateral ovarian dysgerminomata and excessive cutaneous collagen as well as atrophy, and perhaps hypoplasia, of adrenals, thymus, spleen and lymphoid tissue (after steroid therapy). The cerebellum exhibited cortical degeneration. Both lungs were fibrotic with old and recent bronchopneumonia and bronchiectasis. The left lung was studied by injection of a latex preparation; no arteriovenous aneurysms were found, but the smaller pulmonary vessels showed some unusual morphological characteristics.
