Feminizing interstitial cell tumor of the testis: personal observations and a review of the literature.

    A 28-year-old man with evidence of feminization was demonstrated after 4 years of investigation to have a estrogen-secreting interstitial cell tumor. Such feminizing neoplasms are uncommon, only 37 having been described. They are usually benign and are characterized by gynecomastia, a testicular mass and, with lesser frequency, by decreased libido and potency and poor semen quality. The urinary excretion and plasma levels of estrogen are increased and, by selective testicular catheterization, the site of increased estrogen production can be localized. Secondary histologic changes occur in the nontumorous portions of the testis as well as in the contralateral testis; they are most marked in the area immediately adjacent to the tumor. Postoperatively, the gynecomastia regresses, the excessive levels of estrogen return to normal, libido improves, and the sperm count increases to normal.

    As the sexuality of the aged man is concerned it can be said, that the potentia generandi may be maintained up to a very advanced age if there is no restriction because of illness or injury and that, on the other hand, the potentia coeundi will constantly decrease mainly after the age of 60. However, it is possible that in the case of regular sexual activity during the middle life and of a adequate partnership the ability of practicising a satisfaying coitus for able partners may exist during the 7th and 8th decade of life.

[Endocrinologic and ophthalmologic findings before and after transnasal surgery in so-called chromophobe pituitary adenomas].

    Pre- and postoperative visual and endocrine functions were examined in 54 patients with so-called chromophobe pituitary adenomas operated on by the transnasal approach. The preoperative examination showed visual disturbances in 41 of 48 patients. A loss of gonadal function was present in 93% of the women and 69% of the men. Impairment of adrenal function was found in 37% and of thyroid function in 18% of patients. Hyperprolactinemia was present in 16 of 29 patients examined. The visual findings remained unchanged, normal or improved in 75% of patients. Impairment occurred in 3%. Eight patients experienced improvement of gonadal function and 3 of adrenal function, whereas no postoperative improvement of the thyroid function was observed. Deterioration of gonadal function occurred in 4, of adrenal function in 7, and of thyroid function in 9 patients. Hyperprolactinemia was reduced in all cases.

[Participation of gonads in pulmonary silicosis].

    A survey is given on the participation of endocrine glands in the pulmonary silicosis. At the same time a new clinical and experimental contribution to the question of the appearance of a hypogonadotropic hypogonadism is given. The frequency of this hypogonadism increases with the severity of the pulmonary silicosis. The immunological opinions as well as the neuroergonal ones concerning the pathogenesis of pulmonary silicosis gave the possibility of a better explanation of the mechanisms, by means of which also other organs may be attacked in pulmonary silicosis. The neuroendocrine system gives a defensive answer to the aggressive silicotic pulmonary process, which in general slowly progresses, but which may also be certain hormones from the group of the glucocorticosteroids in the adrenal cortex. The intervention into the axis hypothalamus-hypophysis-adrenal cortex has been provided in all defence processes of the organism against every aggression. By the functional or organic damage of the neuroendocrine system, by the immediate toxic effect of silicium dioxide or the monosilicium acid as well as by the effect of the secondary hypoxia in pulmonary silicosis an insufficiency in the stimulation and excretion of certain hormones develops. The origin of this insufficiency is found in the hypothalamo-hypophyseal centres, and the effects extend to all endocrine glands which depend on the system hypothalamus-hypophysis. This mechanism is made responsible for the appearance of a hypogonadotropic insufficiency of gonads in the course of a pulmonary silicosis and is proved clinically and experimentally in the cases investigated by the authors
