Effect of cyproterone acetate and its metabolites on the adrenal function in man, rhesus monkey and rat.

    The effect of oral administration of 17 alpha-acetoxy-6-chloro-1,2 alpha-methylene-4,6]pregnadiene-3,20-dione (cyproterone acetate, Androcur) on the adrenal function in male hypersexual subjects and in female rhesus monkey was investigated on the basis of cortisol and ACTH levels in serum or plasma and excretion of cortisol and 17-ketosteroids. In addition, cyproterone acetate and its main metabolite alcohol of the main metabolite were characterized for treatment of male hypersexual subjects and female rhesus monkeys did not reveal any signs of adrenal suppression. Cyproterone acetate and its metabolites gave on indication of any appreciable anti-inflammatory effect in the adjuvant edema test in rats. However, there was a general increase in the level of blood glucose and liver glycogen as well as a reduction in body weight and organ weight (spleen, thymus and adrenal) in rats, in which 15 beta-hydroxy cyproterone was slightly more active with the exception of adrenal weight tests. It can be concluded that adult man and rhesus monkey are much less sensitive, is so at all, to some corticosteroid-like activities of cyproterone acetate and its main metabolites than the rat.

Iatrogenic and nurisgenic effects of prolonged immobilization of the ill aged.

    The kinesio, phychopathologic and psychosocial effects of prolonged immobilization of the ill aged are outlined. The iatrogenic (physician-induced) and nurisgenic (nurse-induced) factors related to such functional disabilities are described. Illustrative case histories are given. The syndrome is reversible. Thus physicians and nurses have a continued responsibility to support a sustained rehabilitation program for these patients. The biochemical effects of prolonged inactivity indicate that immobilization of the elderly patient results in adverse physical and psychologic phenomena.

[Hormonal patterns in breast cancers].

    Urinary outpout of oestrogens and 17-ketosteroids were estimated in 200 women with breast cancer; 60 of them were cases with rapid evolution (malignant evolutive push). The majority of cases analysed had abnormal hormonal levels, especially of oestrogens. The average oestrogenic levels are higher than the normal values; the highest values correspond to the most severe and rapidly evolutive type of breast cancer, the inflammatory breast carcinoma. In the patients without evolutive push, the proportion of cases with absolute or relative hypooestrogeny is approximately equal toor exceeds the proportion of hyperoestrogenic cases. In the presence of the evolutive push and especially of his most severe type, inflammatory carcinoma, the cases with hyperoestrogeny, both absolute and relative, by hypoandrogeny, are dominant. The proportion of cases with higher 17-ketosteroid urinary outputs than the normal mean is greater in the patients without evolutive push, while in those with evolutive push, the majority of cases have lower values of 17-ketosteroids than the normal mean.
