Evaluation of an injectable progestin-estrogen as a contraceptive.

    A study involving an injectable contraceptive regimen utilizing 2 commercially available hormonal preparations, medroxyprogesterone acetate (Depo-Provera, The Upjohn Company) and estradiol-17 beta-cyclopentylpropionate (Depo-Estradiol Cypionate, The Upjohn Company), is described. 90 multigravidas received intramuscular injections of 50 mg of medroxyprogesterone acetate and 10 mg of estradiol-17 beta-cyclopentylpropionate as a contraceptive every 5 weeks, for a total of 1155 periods. The injections were found to be safe and completely effective. In 15% of the therapeutic cycles, bleeding did not appear for a period of 30 or more days. In 14%, bleeding and/or spotting lasted for longer than 9 days during a 30 day period. The incidence of aminorrhea increased somewhat after the first 10 months of treatment. Endometrial biopsies showed a "suppressed" endometrium. Adrenal pituitary responsiveness to metyrapone was found to be normal in 17 of 21 patients tested. Although almost half of the patients presented some complaints during the study, these were mostly transient and minor in nature. Headache was the complaint most frequently observed (14%), but it was generally limited to 1 cycle. The monthly injectable contraceptive regimen may be useful in a particular population of patients who, for various reasons, cannot tolerate or cannot be relied upon to take oral contraceptives. The authors believe that because of the high incidence of alterations in the bleeding pattern while on the therapeutic regimen, as well as the frequent occurrence of aminorrhea following discontinuation of therapy, the use of such a contraceptive regimen in the general population might not be desirable.

Steroid secretory characteristics of a virilizing adrenal adenoma in a woman.

    A tumour of the left adrenal gland was identified in a woman who presented with virilization and secondary amenorrhea. Preoperatively, the plasma levels of dehydroepiandrosterone sulphate, dehydroepiandrosterone, androstenedione, testosterone, 5 alpha-dihydrotestosterone and 5-androstene-3 beta,17 beta-diol were elevated two- to fourfold whereas those of urinary 17-ketosteroids were elevated more than tenfold. The production rate of dehydroepiandrosterone sulphate was more than 16 times that in normal women whereas those of dehydroepiandrosterone, testosterone and androstenedione were approximately twofold greater; plasma testosterone was derived almost entirely from the peripheral conversion of androstenedione. Blood was obtained by catheterization of the ovarian veins, left adrenal gland vein and inferior vena cava (at two different sites) and plasma steroid levels were determined: testosterone and cortisol levels were elevated in all blood samples whereas those of androstenedione, dehydroepiandrosterone sulphate and 11-desoxycortisol were approximately six- to eightfold, 1.5-fold and nine- to 22-fold higher in the effluent on the left adrenal gland/tumour compared with the levels in the other compartments. Blood was collected hourly for 24 h to determine steroid levels under basal conditions and, also, after ACTH treatment. Plasma cortisol levels increased markedly upon ACTH administration and fell to very low levels 11 h later, but those of androstenedione, testosterone, dehydroepiandrosterone, 5-androstene-3 beta,17 beta-diol and dehydroepiandrosterone sulphate were not affected by ACTH treatment. A histological diagnosis of cortical adenoma of the extirpated tumour was made. Tissue explants and adenoma cells were maintained in culture to characterize the steroid-metabolizing properties of the tumour. The secretion of dehydroepiandrosterone sulphate by tissue explants was highly initially, but declined to almost undetectable levels after 5 days in culture. In the presence of ACTH, dehydroepiandrosterone sulphate secretion remained elevated throughout the entire study up to 5 days. Basal secretion of dehydroepiandrosterone sulphate, androstenedione, 11-desoxycortisol, cortisol, testosterone and 11 beta-hydroxyandrostenedione by adenoma cells was either very low or undetectable. [Endocrine and ultrasound examination of hirsutism].

    One hundred and fourteen nontumorous hirsute women were investigated. The degree of hirsutism, the menstrual abnormalities and the level of 17-ketosteroids, of DHEA, of 17-OH-corticoids in urine and RIA of total and free testosterone and of sex binding globulins (SHBG) in blood were performed. On the basis of endocrinological and ultrasonographic determinations 3 patients were diagnosed as congenital adrenal hyperplasia, 35 patients as late onset adrenal hyperplasia and 57 women as polycystic ovary syndrome (PCO). The maximal mean level (X + SD) of total and free testosterone above 0.5 ng/ml relatively above 5.05 pg/ml were performed in 94.3% relatively 97.1% in late onset adrenal hyperplasia and in 78.9% relatively 87.7% in PCO. The diagnosis of PCO is possible among 75% of all hirsute patients after exclusion the patients with late onset adrenal hyperplasia. The PCO is confirmed by hormonal or/and ultrasonographic examination. The diagnosis of late onset adrenal hyperplasia is allowable only by hormonal investigation.

Role of androgens in menstrual disorders of nonhirsute and hirsute women, and the effect of glucocorticoid therapy on androgen levels in hirsute hyperandrogenic women.

    The plasma concentrations of total testosterone, free testosterone index, androstenedione, 17 beta-estradiol, luteinizing hormone, follicle-stimulating hormone, prolactin, and urinary 17-ketosteroid and 17-ketogenic steroid excretion were measured in 48 nonhirsute and 119 hirsute patients. Hormone data were compared within and between groups according to whether the menstrual cycles were eumenorrheic, amenorrheic, or oligomenorrheic. Eleven hirsute women treated with prednisone were followed for 6 months. It was concluded that: (1) androstenedione, testosterone, free testosterone index, and adrenal androgens alone or in combination play a role in the pathogenesis of the hirsutism observed in eumenorrheic women and in the amenorrhea and oligomenorrhea of both hirsute and nonhirsute women; (2) body weight correlated with adrenal adrogens (17-ketosteroids) in nonhirsute women and with androstenedione in hirsute women; (3) prednisone significantly suppressed androstenedione and 17-ketosteroids (p less than 0.05), with a decline of testosterone to 65% and luteinizing hormone to 51% of pretreatment values, with favorable clinical effects on the hirsutism, menstrual dysfunction, and infertility; (4) concentrations of 17 beta-estradiol were lower in amenorrheic than in eumenorrheic and oligomenorrheic women of both groups.

Menarche and puberty in daughters of amenorrheic women.

    Twenty-six daughters born to amenorrheic women after gonadotropin-induced ovulation were studied at 10 to 16 years of age. The aim of the study was to assess whether the mothers' condition, namely, amenorrhea and infertility followed by the pharmacologic induction of ovulation, had any effect on their female offspring in terms of endocrine disorders at puberty. The daughters were found to have normal onset of puberty as well as normal physical and mental development. The mean age at menarche, body weight, and height were similar to those of the general female population in Israel. A functioning hypothalamic-pituitary-ovarian axis was evidenced by the appearance of menarche followed by regular cycles. These data form a reassuring sample for the clinicians, the treated mothers, and their offspring.

[Androgenetic amenorrhea].

    Virilism in females is often associated with menstrual disorders. There is an evident correlation between the severity of virilism, the level of androgens and the degree of menstrual disorder. A study was carried out between January 1978 and December 1980 on 75 women with various degrees of virilism. The majority of women with severe hirsutism and male-type alopecia were found in the group with amenorrhoea. Furthermore, the four cases with the full-blown picture of severe virilism were found exclusively in this group. Clarification of the virilizing manifestations on the basis of a determination of the basic hormonal status (including the steroid fractions), the dexamethasone suppression test (including the HCG stimulation test) and the oestrogen-gestagen suppression test, diagnostic laparoscopy and phlebography of the adrenal gland is discussed and the methods evaluated.

A decade's experience with an individualized clomiphene treatment regimen including its effect on the postcoital test.

    During a 10-year period, 428 women received clomiphene citrate according to a graduated therapeutic regimen in which the dose of clomiphene and the laboratory studies were individualized according to each patient's history, examination and response. Of the 428 patients, 85.3% ovulated and 42.8% conceived. The great majority of those who conceived did so during the first three ovulatory cycles. There was no evidence that clomiphene therapy was associated with the induction of another cause of infertility. Overall, 88.2% of those with no other causes for infertility who ovulated also conceived. However, only 7.8% of those who had one or more factors in addition to anovulation became pregnant. There was no evidence that clomiphene adversely affected the postcoital test, as only 15% of the patients had poor cervical mucus. The low rate of complications of this treatment, 5.1% cyst formation as well as the 14% abortion rate and the 2.6% congenital anomaly rate and the excellent gestational outcome in those who conceived support the use of this treatment regimen.

[The problem of extirpation of gonads in "testicular feminisation"].

    The 15-year old female patient G. G. was referred to the outpatient department for child gynaecology on account of a primary amenorrhoea. The endocrinal parameters found (testosterone, oestradiol, 17 OHCS, 17 KS), chromosome analysis, vaginoscopy and the result of a laparoscopy led to the clinical diagnosis of "testicular feminisation". Satisfactory development of secondary female sex characteristics and sudden increase of complaints caused by an inguinal hernia on the left (gonad situated in the inner inguinal ring) prompted us to perform bilateral extirpation of gonads with simultaneous treatment of the inguinal hernia. Histological examination revealed the existence of a seminoma of the right gonad. This case confirm our stand in respect of extirpation of gonads at the onset of puberty, but it also raised the question as to whether it would be advisable to operate at an even earlier stage in order to avoid the risk of malignant degeneration of intra-abdominally positioned testes.

Antepartum pituitary infarction.

    Antepartum pituitary infarction occurs only in insulin-dependent diabetic patients. It is manifested by severe headache, followed by decreasing insulin requirements. Delivery is frequently premature, with high fetal wastage and increased maternal mortality. During the puerperium, the first manifestation of pituitary deficiency, other than a lower insulin requirement than would be expected, is failure to lactate. Subsequent evaluation of pituitary function reveals variable deficiencies with loss of growth hormone and gonadotropins being most frequent. This case is the eighth report of this entity, and it represents the first patient to survive a pituitary infarction prior to the third trimester of pregnancy. Recognition of this syndrome is critical in order to ensure that the mother's health and the viability of the offspring be preserved.

[Raised serum PRL and androgen levels associated with hyrsutism, amenorrhoea and galactorrhoea].

    This is a report of a case of a case of amenorrhoea-galactorrhoea syndrome with hyperprolactinaemia associated with increase of plasma androstenedione and urinary 17-ketosteroids, gradually developed in a sterile subject. Plasma LH and testosterone levels and the results of adrenal suppression and ovarian stimulation tests seem to prove, point towards, the adrenal as the likely source of androgens. Treatment with bromocryptin for 20 weeks brought about a reduction to normal of both PRL and androgens. Resumption of ovulatory cycles followed thereafter and a pregnancy eventually occurred.

The treatment of mild adrenal hyperplasia and associated infertility with prednisone.

    Thirty patients with mild post-pubertal adrenal hyperplasia, characterized by raised urinary 17-oxosteroid levels and variable combinations of irregular menses, hirsuties, infertility, and spontaneous abortion, were treated with 2.5 to 10 mg of prednisone per day and all conceived (55 pregnancies). With this treatment, regular, ovulatory cycles occurred immediately in 25 patients, and after two to six months, in the rest. Treatment reduced raised 17-oxosteroid levels to normal and brought about some improvement in hirsuties and acne. Forty-seven pregnancies ended in the birth of liveborn infants; one of these died of prematurity and another had congenital emphysema. One pregnancy was terminated, two were of unknown outcome and five (9.4%) ended in abortion. Before treatment, 20 out of 22 pregnancies (91%) had ended in abortion.

Lipid cell tumor of the ovary: an ultrastructural study.

    An ovarian lipid cell tumor without Reinke's crystalloids in a woman with secondary amenorrhea, minimal hirsutism, and elevated 17-ketosteroid excretion was studied by light and electron microscopy. Tumor cells were found in small clumps or scattered singly within a collagenous matrix. The cytoplasm of the tumor cells contained abundant smooth endoplasmic reticula, numerous mitochondria with tubular cristae, lipid droplets, lysosomal dense bodies, and concentric membranous whorls, characteristic of steroidogenic cells. In addition, "peripheral canalicular systems" were found at the outer margins of the nests of the tumor cells. These "peripheral canalicular systems" were bordered by the cell membranes and the surrounding collagenous stroma into which microvilli projected. Since the intercellular canalicular system present between the tumor cells was continuous with the "peripheral canalicular system," both systems probably have a common function related to steroid metabolism. The intercellular and "peripheral" canalicular systems and cytoplasmic microfilaments found in this tumor suggest that this ovarian lipid cell tumor was derived from the ovarian stroma.

Review of 18 years' experience with pituitary tumors.

    The presenting signs, symptoms, roentgenographic findings, endocrine evaluations, treatment, and results in 68 cases of presumed pituitary adenomas treated over an 18-year period are discussed. The most common symptoms were headache, acromegalic changes, visual symptoms, and amenorrhea. Most common physical findings were obesity, acromegaly, and visual field defects, usually bitemporal hemianopsia. Roentgenographic evidence of sellar erosion was almost universal but angiography and pneumoencephalography were required to evaluate suprasellar extension. Brain scan was not considered a particularly useful diagnostic tool. Endocrine status was best evaluated by a battery of tests including 17-OH, 17-KS, T3, T4, PBI, ACTH stimulation, and FSH and STH levels. (Prolactin levels are currently being obtained, also). Surgical specimens were obtained in 29 patients, with subsequent diagnoses of 22 chromophobe adenomas, five eosinophilie adenomas, one cystic adenoma, and one necrotic tumor. All five eosinophilic tumors came from acromegalic patients. Patients treated by operation alone or operation followed by radiotherapy generally had less "medical morbidity" than did patients who received radiotherapy alone.

A case of adrenogenital syndrome with aberrant 11beta-hydroxylation.

    A 17 year old female patient with hypertension, amenorrhoea and hirsutism was found to have subnormal levels of plasma and urinary cortisol, significant plasma levels of Reichstein's compound S and 21-deoxycortisol, high urinary levels of THS and pregnanetriolone as well as elevated levels of plasma and urinary testosterone. Treatment with 0.5 mg/day of dexamethasone or 25 mg/day cortisone reduced her hypertension and restored her menstrual cycles, but also resulted in the development of moon face, body striae and a gain in weight. Lower doses of cortisone were without effect. Selection of patients for clomiphene citrate therapy.

    Ninety-three infertile women were treated with clomiphene citrate alone or in combination with human chorionic gonadotropin (hCG) for absent or infrequent ovulation. The patients were divided into eight categories according to the diagnosis obtained: ovarian androgenic hyperplasia, adrenal androgenic hyperplasia, mixed ovarian and adrenal androgenic hyperplasia, hypothalamic anovulation, postpill anovulation, follicular phase defect, luteal phase defect, and amenorrhea-galactorrhea syndrome. Each group was analyzed individually to compare the ovulation and conception rates and the complications involved. A survey of the data presented in this study shows that the best response was noted in patients with ovarian androgenic hyperplasia. Patients with a functional pathologic adrenal component responded favorably when dexamethasone was used as an adjuvant to clomiphene therapy. Those with hypothalamic anovulation responded better when hCG was added to clomiphene therapy. Women with postpill anovulation as well as those with follicular phase defect were found to be good candidates for clomiphene therapy. In properly selected patients with poor luteal phase defect, hCG secured excellent results both in ovulation and conception. Patients with lactation amenorrhea failed to ovulate when treated with clomiphene alone.

The significance of the combined dexamethasone-HCG test for the assessment of hyperandrogenism.

    The combined dexamethasone-HCG test was employed in 57 patients with excessive androgen secretion in order to differentiate the Stein-Leventhal syndrome from other disorders associated with increased androgen production. The patients were classified according to the test results. All except two of the patients gave a test result typical for the Stein-Leventhal syndrome.

Effect of synthetic luteinizing hormone releasing hormone (LH-RH) on the release of gonadotropins in Cushing's disease.

    The effect of intravenous synthetic luteinizing hormone releasing hormone (LH-RH) on plasma radioimmunoassayable levels of gonadotropins was investigated in 6 women suffering from Cushing's disease with bilateral adrenal hyperplasia. In five of six cases no significant variation of plasma LH levels was found following stimulation; in one case the response to LH-RH was present although slightly reduced below the normal range. By contrast in all cases the plasma FSH response was similar to that recorded in normal subjects. The explanation of impaired LH response is not clear but the possibility that endogenous hypercortisolism affects the pituitary responsiveness to LH-RH has to be considered.

Successful induction of follicular maturation and ovulation by prolonged treatment with LH-releasing hormone in women with anorexia nervosa.

    Four women with anorexia nervosa were treated with synthetic LH-releasing hormone (LRH) in an attempt to induce ovulation. All the women had very low pretreatment levels of gonadotropins and estrogens. Administration of LRH resulted in significant gonadotropin increases. The FSH response to LRH in relation to the LH response was higher than in regularly menstruating women. LRH (500 mug) was administered parenterally three times daily over about 4 weeks. During this period there were no significant effects on mood, eating behavior, weight, or libido. All the women responded with follicular maturation and ovulation to the prolonged LRH treatment.

Gynandroblastoma of the ovary.

    A gynandroblastoma of the ovary in a 17-year-old girl is reported. Primary amenorrhoea, hirsuties and slight clitoral enlargement were associated with the tumour and high levels of circulating testosterone were found. This is the first gynandroblastoma in which Reinke crystalloids have been identified in the Leydig cell component. The criteria for the diagnosis of this tumour and its histogenesis are discussed.

Menstrual dysfunction following use of oral contraceptives.

    An evaluation of 13 patients with menstrual dysfunction following discontinuation of oral contraceptives is presented. All had normal ovulatory cycles of 26-34 days before beginning oral contraceptive medication. The duration of therapy was 3-42 months with 14 months the mean. Relationship between occurrence of menstrual dysfunction and duration of therapy or type of agent administered was not noted. 9 were classified as amenorrheic (6 months or more of secondary amenorrhea) and their condition lasted from 6 to 36 months. Cervical mucus from 6 demonstrated a 3 to 4+ fern phenomena, indicating significant endogenous estrogen production. Gynecologic endocrine status was evaluated by basal body temperature, endometrial biopsy, and cervical mucus arborization. Total gonadotropins were measured in those demonstrating poor or absent fern phenomenon. Adrenal and thyroid functions were measured by 17-ketosteroid and 17-hydroxysteroid excretion and PBI respectively. Patients were screened for pituitary tumors by X-ray and visual examinations. Clomiphene (Clomid) was administered to 6 patients in 5 day courses of 50 or 100 mg per day. Of these 4 responded with ovulatory cycles. One became pregnant. 2 patients not responding to several such courses, were given human menopausal gonadotropin (HMG Pregova) in doses of 140 I.U. a day until cervical mucus demonstrated 4+ fern phenomenon. Human chorionic gonadotropin (HCG) was then given in a single dose of 5000 I.U. One responded to this treatment and became pregnant. 3 were not treated because they did not desire conception or ovulation induction. The 4 patients who were amenorrheic for less than 6 months were considered oligomenorrheic. 2 responded to clomiphene but the pattern of prolonged follicular phase has recurred in both. Their menstrual dysfunction is thought to be due to delayed recovery from suppression of some hypothalamic-pituitary mechanisms involved in normal gonadotropin regulation. Gonadtropin therapy is the only effective method of ovulation induction in those patients who fail to respond to clomiphene.

CLINICAL AND LABORATORY ASSESSMENT OF A NEW PROGESTATIONAL AGENT.

    Studies of the effects of megestrol (6-methyl, 6-dehydro, 17alpha-acetoxy progesterone) on human subjects indicated that this drug is a potent, orally effective progestational agent. The progestational effect was enhanced by estrogen. When combined with ethinyl estradiol satisfactory control of menstrual function and inhibition of ovulation were achieved. Increasing doses of this combination were effective in four patients with endometriosis. Side effects of the drug were minimal. No toxic effects on adrenal, hepatic or hematopoietic function were noted.
