Virilizing adrenocortical tumors in childhood: eight cases and a review of the literature.

    Eight cases of adrenocortical tumor are presented with a review of the literature. Although such tumors are rare, they are important causes of inappropriate virilization and Cushing's syndrome in childhood. Clinical virilization with or without hypercortisolism was found in all eight children, who were 5 years old or younger. Excessive linear growth was noted, despite evidence of hypercortisolism. Serum levels of dehydroepiandrosterone, dehydroepiandrosterone sulfate, testosterone, and cortisol were elevated in all cases tested and appear to be useful diagnostic alternatives to the more traditional determinations of urine 17-ketosteroids and 17-hydroxycorticosteroids. Abdominal sonography and computed tomography have proven to be reliable tools for tumor localization. Surgical resection was the definitive therapy in all patients, and perioperative steroid replacement was essential. Histologic diagnosis appeared to have little bearing on prognosis, and the majority of pediatric patients have had clinically benign disease. At a mean follow-up of 3 years, seven of the eight children were alive and had no evidence of tumor recurrence.

The effect of alpha-difluoromethyl-ornithine on tumor growth, acute phase reactants, beta-2-microglobulin and hydroxyproline in kidney and bladder carcinomas.

    Multiple oral doses of alpha-difluoromethyl-ornithine (alpha-DFMO), 18-24 g/day for up to 2 months, were administered to 2 patients with invasive and metastatic carcinoma of the bladder and to 3 patients with metastatic renal cancer in an open study. The moderate antigrowth effect of alpha-DFMO in these patients was concluded from the results of serial kidney, liver and bone scintigrams, from the low frequency of a local recurrence of bladder carcinoma and from the survival rate of 4 of 5 patients after 12 months. Increased excretion of hydroxyproline and beta2-microglobulin were measured 1 month following alpha-DFMO treatment suggesting initial tumor necrosis and degradation. Treatment was poorly tolerated with gastrointestinal side effects (e.g. gastritis, vomiting, diarrhea) in all subjects, erythropenia in 2 subjects (associated with thrombocytopenia and leucopenia in 1 subject each), and audiovestibular symptoms in 2 patients. All side effects were reversible and normal function returned 2-4 months after stopping therapy.

[Steroid balance in breast cancers of varying histological structure].

    Patients with breast cancer in stage I--II of various histological structure showed varied hormonal disturbances. In adenocarcinoma a high level of classic estrogens or total phenolsteroids was noted. In low differentiated cancers (solid, scirrhus) the values of estrogens excretion were considerably lower, but, if correlated with other hormones (androsterone), a relative hyperestrogenization is observed. Scirrhous cancers are characterized by the increased 17-ketogenic steroids excretion. The character of hormonal disturbances concomitant with the predominant development of certain breast tumor structures (adenocarcinoma, solid cancer, scirrhus) is identical for the patients being in their reproductive and menopausal period.

C19-steroid metabolism by spontaneous adenocarcinoma of the A X C rat ventral prostate.

    Specimens of ventral prostate from 37- to 46-month-old Andradurin-treated A X C rats demonstrated three primary morphologic patterns, each being divisible into two sub-groups based upon the extent of glandular alteration. The principal groups were: group 1, hyperplasia; group 2, atypical hyperplasia; and group 3, adenocarcinoma. The secondary classifications were: subgroup (+), few glands involved; and subgroup (++), most glands involved. Multiple parameters of ventral prostate testosterone metabolic potential failed to distinguish the morphologically diverse prostate specimens of groups (1+). 1(++), 2(+), 2(++), and 3(+) which predominantly metabolized testosterone to 5alpha-reduced 17beta-hydroxysteroids. By contrast, testosterone metabolism by ventral prostate specimens predominantly composed of neoplastic epithelium, group 3(++), was distinct from all other prostate specimens. The distinguishing feature was a shift to more prominent elaboration of 17-ketosteroids, principally delta4-androstenedione, and a cteroids. The change in this biochemical parameter of prostate epithelial cell function was indicated to be an early manifestation of the neoplastic transformation.

[Sclerocystic ovaries and cancer of the uterine body].

    As a result of investigation and treatment of 5000 patients with disorders in menstrual function and endocrinous sterility sclerocystic ovaries were diagnosed in 1.8-2% (90 patients). In 6 of them ovarian changes were associated with cancer of the uterine body. In uterine body cancer and sclerocystic ovaries opsomenorrhea and menorrhagia, obesity were of great persistance, the duration of the disease being 12-20 years. Based on the conducted investigations it is considered rational to perform a wedge resection as early as possible, but not following many years of unsuccessful hormone therapy, and to study histologically endometrial curettage specimens. In recurrence of the disease and enlargement even of one ovary a repeat wedge ovariectomy should be performed.

The possible mechanisms of action of progestins on endometrial adenocarcinoma.

    To investigate the effect of medroxyprogesterone acetate therapy upon the immune response and endogenous status of patients with primary or recurrent endometrial adenocarcinoma, 8 patients were treated with progestin therapy as inpatients for 33-80 days. Patients were studied intermittently for up to 72 months. Serum protein electrophoresis during treatment showed no change in protein pattern. In all 8 patients, there was a suppression of pituitary gonadotrophin. There was no evidence that the adenocarcinomas were suppressed by either an immunologic mechanism or by the variation in endogenous hormone production. Investigation should be continued to study the effects of medroxyprogesterone acetate on the cellular level.

Androgen dynamics in vitro in the human prostate gland. Effect of oestradiol-17 .

    Normal, hyperplastic and adenocarcinomatous human prostatic tissue was perfused in vitro with radioactively labelled androstenedione, testosterone and 5alpha-dihydrotestosterone with and without added oestradiol-17beta. Various parameters of tissue-steroid relationship were measured at the steady state. When oestradiol (0.11 or 0.22mumol/l) was added to the perfusing medium, the entry of the steroids into the tissue and their metabolism was increased in the majority of the glands studied. The ;uptake' of all the steroids varied, in response to the addition of oestradiol, in both normal and adenocarcinomatous glands in a way differing from the response of hyperplastic glands. As a consequence, the tissue clearance of the steroids, particularly of androstenedione and testosterone, increased in normal and adenocarcinomatous glands in the presence of oestradiol, and decreased in the hyperplastic tissues. At a concentration 0.33mumol/l, oestradiol decreased the entry of the steroids in all the tissues studied, while the clearance of steroids tended to decrease. The significance of these findings in terms of the regulation of androgen dynamics in vivo in the normal and diseased human prostate, with particular regard to the response to oestrogen treatment, is discussed.
