Comparison of survival time between two types of orthodontic fixed retainer: a prospective randomized clinical trial.

    The aim of this prospective clinical study was to compare the mean durability and the failure rates of two types of orthodontic retainers.. Orthodontic patients (142) aged between 14 and 28 years were recruited in this study. The polyethylene woven ribbon (Ribbond, Seattle, WA, USA) retainer was compared with a 0.0175-in flexible spiral wire (Respond, Ormco, Glendora, CA, USA) retainer. When treatment was completed, the retainers were bonded from canine to canine in the maxillary and the mandibular arches of the participants. In the follow-up visits, the patients were re-evaluated every 3 months over a period of 18 months. The time taken for the retainers to remain without any fracture was appraised. Kaplan-Meier analysis and the logrank test were employed to identify significant differences in the survival functions between the groups. The rates of the retainers' failure between the groups were analyzed using Chi-square test.. It was revealed that the mean survival of the flexible spiral wire retainer was 15.34±0.47 and 15.60±0.42 months in the maxillary and mandibular arches, respectively. The mean survival of the ribbon retainer was 13.95±0.55 and 14.26±0.57 months in the maxillary and mandibular arches, respectively. Ribbon retainers showed a failure rate of 50% in the maxillary and 42.6% in the mandibular arches. Flexible spiral retainers showed a failure rate of 36.5% in the maxillary and 37.8% in the mandibular arches. The differences were not statistically significant. Regarding the evaluation period, the differences had limited clinical significance.. The mean survival time and the failure rates of the polyethylene woven ribbon retainer were comparable to the flexible spiral wire retainer during the 18 months after orthodontic treatment.

Shear bond strength of orthodontic brackets bonded with different self-etching adhesives.

    The purpose of this study was to compare the shear bond strength (SBS) of orthodontic brackets bonded with 4 self-etching adhesives.. A total of 175 extracted premolars were randomly divided into 5 groups (n = 35). Group I was the control, in which the enamel was etched with 37% phosphoric acid, and stainless steel brackets were bonded with Transbond XT (3M Unitek, Monrovia, Calif). In the remaining 4 groups, the enamel was conditioned with the following self-etching primers and adhesives: group II, Transbond Plus and Transbond XT (3M Unitek); group III, Clearfil Mega Bond FA and Kurasper F (Kuraray Medical, Tokyo, Japan); group IV, Primers A and B, and BeautyOrtho Bond (Shofu, Kyoto, Japan); and group V, AdheSE and Heliosit Orthodontic (Ivoclar Vivadent AG, Liechtenstein). The teeth were stored in distilled water at 37 degrees C for 24 hours and debonded with a universal testing machine. The adhesive remnant index (ARI) including enamel fracture score was also evaluated. Additionally, the conditioned enamel surfaces were observed under a scanning electron microscope.. The SBS values of groups I (19.0 +/- 6.7 MPa) and II (16.6 +/- 7.3 MPa) were significantly higher than those of groups III (11.0 +/- 3.9 MPa), IV (10.1 +/- 3.7 MPa), and V (11.8 +/- 3.5 MPa). Fluoride-releasing adhesives (Kurasper F and BeautyOrtho Bond) showed clinically acceptable SBS values. Significant differences were found in the ARI and enamel fracture scores between groups I and II.. The 4 self-etching adhesives yielded SBS values higher than the bond strength (5.9 to 7.8 MPa) suggested for routine clinical treatment, indicating that orthodontic brackets can be successfully bonded with any of these self-etching adhesives.

Bonding of brackets using a caries-protective adhesive patch.

    The purpose of this study was to assess the shear bond strength (SBS) of metal brackets when placed with a caries protective adhesive patch.. Forty stainless steel brackets (Ormco) were bonded to 40 bovine enamel samples according to the following conditioning/bonding procedures using a resin-based orthodontic luting material (Heliosit Orthodontic, N=10 per group): (A) 35% phosphoric acid (30s), rinse and dry; (B) as in A but additional placement of a prototype adhesive patch (Ivoclar Vivadent) using a bonding agent (Heliobond); (C) application of a two-step self-etch adhesive (AdheSE); (D) as in C but additional patch placement. Samples were stored at 37 degrees C for 24h. SBS was measured with a universal testing machine with a crosshead speed of 0.5mm/min and the adhesive remnant index (ARI) was determined under a stereomicroscope (16x).. Mean SBS (standard deviation) values were as follows: (A) 16.6 (6.4)MPa; (B) 12.2 (5.8); (C) 12.9 (5.0); and (D) 10.5 (4.7). Analysis of variance followed by Bonferroni correction revealed no statistically significant differences. In 2 (B) and 4 (D) specimens, complete retention of the adhesive patch was observed.. All treatment groups showed adequate bond strength values. The adhesive patch could therefore be applied in combination with orthodontic brackets and seal the enamel adjacent to the bracket.
